Oh no, these scenarios were great and so truthful. In all the example there was a huge misunderstanding between providers and "the interpreter " who are not qualified. I am so glad that I am studying translation and interpretation, and my feel will be a medical interpreter. These are great case materials to teach. Thank you

I've been a medical interpreter for 4 years and this is sadly a common scenario, we want to advocate for the patient and we can even cuote the 1557 affordable care act legislation and even then, some doctors and patients choose to use family members instead. It's on them at that point and it still won't let you sleep at night knowing that you could've done better.

(1)The patient's brother was not an effective interpreter as for one he is not a professional medical interpreter. Therefore when you have a person that is not familiar in the field things can go wrong as in that particular case were the brother omitted a lot of the questions asked by the doctor, and he also was assuming things as saying that he probably just have gas or air nothing else.(2) He offered himself to interpret for his brother as he said he has done so for him many times before. The physian did had a choice and should have called for a trained medical interpreter. In that case if I were the physician I would have called for a trained medical interpreter because the fact that you are able to speak two different languages that does not make you an expert on the medical field per say in this case. The limitations were obvious the brother "interpreter" was very limited at the interpretation of both languages he omitted basically all the questions asked by the physician. He also added things to the interpretation (3) No I don't believe it was accurate. (4) He's speech pattern was not the greatest but he spoke very clearly to the brother "interpreter" Great video!!! Excellent for all new Medical Interpreters in the field. Thanks for posting it.

Bilsan Abdi - Sorry, but this example is TRUE to life. Family interpreters always "know better" than the patient. Notice the patient's thousand-word response to a question, and notice the dishonesty of the relative who interpreted a thousand words as: "No". As a medical interpreter I have witnessed this flouting of HIPAA law, and sometimes have argued with crooked relatives who always "know the patient better" than does the patient himself.

doctor should of have got the medical interpreter to begin with. I think he took the easy route by just using the family member- but, being the first time seeing that patient- he should't have done that. (May be if he had known the patient for a while it could have been fine but NOT on a first time encounter. Speaking of the brother- he did not interpret to the full content of the patient needs- he omitted so much Important and critical information!! The doctor mentioned the high blood pressure vital sign but the brother did not even bother to ask for the numbers or how critical it was. In my opinion, I can sense the brother is not familiar with medical terms- so this makes the information being interpreted more difficult for all involved.

In many situations, some so called trained interpreters don't even understand English. A friend of mine called his doctor, they gave him an interpreter who would not accurately translate what was being said, he kept beating around the bush until I had to interact to save my friend. Bear in mind that so many people might be able to interpret even better than some certified interpreters. If your proficiency in both languages is not adequate, your certification will not get the job done. If you are not gifted to be professional and smart, your certification will betray your career. This is what they call soft skills and hard skills.

After reading and learning about the rules of simultaneous. You should use speaking in one person, while in this video, the interpreter uses "He" for it. And for body positioning, interpreter should be next to someone who speaks a foreign language (not who's speaks english) not un front of the doctor. So that, the view is still carried between doctor and patient.

I did not like when the doctor asked and or inquire About the patients sexual history- the comments were derogative (in my opinion) and quite judgmental. There were so many more factors in this patient- doctor interview that makes it potential for error.

very unprofessional interpretation. the patient said that he was ever sick when he was about 15 years and the translator said never. he is saying opposite of what is being said by the patient in a very risky matter.

In this video we see the potential ethical pitfalls with the use of family members as medical translators instead of trained translational professionals. Using the patient’s brother to translate may be the easiest and quickest option available, however it is not the best option for ensuring accurate medical information is being relayed to the patient from the physician, and from physician to the patient. As we see in this video, the brother of the patient relayed incorrect medical information which could potentially cause harm or decrease the quality of care that the patient receives. This practice brings into question the ethics of patient autonomy, or the right of the patient to make their own decisions about their healthcare. If they are permitted to use a family member instead of a professional medical translator, then the medical information may end up being filtered through the family member’s opinions before reaching the patient. If this is the case, the patient may not receive all the pertinent information necessary for them to make the most informed decision about their healthcare. In that situation their right to healthcare autonomy is being infringed upon and therefore is not ethical based care. Additionally, the idea of justice in healthcare is brought into question because those patients who need an interpreter are subject to the availability of specific medical interpreters. If the patient speaks a language that is not spoken by a professional interpreter, then they could face a much harder time receiving quality care due to the inability for effective communication with their healthcare team. This could be a problem ethically because it creates an unfair discrepancy between the level of care that is being provided to those who do not need interpreters and those who do. As such, it is important that healthcare settings are prepared and able to use medical interpretation services to their fullest capabilities whenever possible, to provide the highest level of quality care to all potential patients.
