Here's the simple system I use to track my fitness goals. When in the gym, I record my workouts using the LOCHBY Pocket Journal. It's small and fits in a pocket.
I use the Dot Grid format to record reps/weight. When I fill out a refill, I store it for later. That way, I can look back on past workouts and see where I need to adjust.
