According to National Geographic, it was unmanned and ordered to self-destruct when a rocket started to act awry. (in before "What happened?" comments)

I can't find any reference to a self destruct option for the antares, it there was one, nasa would have placed charges all over the rocket, blowing it to pieces. The reason stated by Orbital Sciences was that they bought an old soviet rocket motor that had been sitting in a shed for fifty years, and a turbopump fractured, causing the explosion you see at 0:20

I think I was in third or fourth grade when this happened, watching with my mom and brother from the cemetery in Onancock (it’s the only place in town where you can see the launches) and we saw it go up into the sky and just explode and go back down; I remember crying because I thought there were people in the rocket, thank god it was just a supply load 😅
