Same story here. I’m 55 and had a complete cardio test done because I had heart palpitations(turned out to be nothing). My cardiologist tells me that I had the cardiovascular system of a healthy 25 year old but because my LDL was slightly elevated he wanted to put me on statins. He didn’t mention, not one single time, anything about changing my diet or exercise, just pushed a drug. So he began to negotiate with me as if I had stepped out on a ledge and he had to talk me down, this went on for 10 minutes and still no discussion on nutrition. When he said “So Mr. Smith, how long do you want to live?”…..I walked out. I received about 10 calls from his office demanding that I come back and pick up my prescription until the message said they called it and to go pick it up, those pills can rot there. Over the last 4 years of living LC and really studying it and how my biology works the more I understand SOME doctors just don’t get it. Thanks for keeping us educated.

They try to push statins on everyone above a certain level, no matter diet, lifestyle or otherwise good health. All they know are numbers and drugs. It's just lazy. Statins have awful side effects. I guess "first do no harm" doesn't apply anymore.

I'm 58 now and I have had high LDL since the time I had my first blood test 30 years ago, I have never taken statins even if doctors insist. I have never been obese and I have always made exercise and played soccer all this time. Now I'm still playing soccer at a very good level. I had some health issues until I understood that I had chronic inflammation due to the high consumption of carbs. I began fasting, then Keto and now I'm a carnivore and continue doing fast (16 to 24 hours daily). After my last blood test, my numbers are like the ones discussed in this presentation (my LDL is lower less than 200), triglycerides 60, A1C 4, no inflammation, and my blood pressure is 110/65 lower than when I was 40 (after a soccer game is 98/60), my weight is like when I was 25, and last year I had a complete heart check-up with no evidence of atherosclerosis. The cardiologist told me to continue doing what I was doing. Evens o, my family doctor continues saying that I have to take statins... It's incredible!!

My doctor had a fit about my “dangerous” cholesterol levels. I even printed studies and he wouldn’t even look at it or listen to me. He warned me and told me I needed to go off keto. Yet he wouldn’t even acknowledge my bmi (from 24.0 to 21.6) or how my fasting glucose went from 99 to 83. My good cholesterol went up as so did my triglycerides (still in normal range. I did drink black coffee the morning of my blood work which could have contributed to that). The thing that irritated me the most is that he wouldn’t order the advanced lipid panel blood test for me. I guess I will have to do it in my own and pay out of pocket which is ridiculous because I have and pay for health insurance. I wish I could find a doctor who believes in and is knowledgeable in this. 😞

I had EXACTLY the same reaction and discussion with my PCP! Except that when I pointed out the HDL and triglycerides he said they were not important. I did an independent NMR that showed the LDL was due to large nonatherogenic particles and showed it to him on the next visit. He refused to even look at it.

I believe that nothing is more annoying to a doctor than having a patient arguing his opinion. They are not gods, rather above average educated people. And I'm sure most of them do try to keep us going. Having said that, they do show a particular lack of knowledge regarding some issues, like which lipids run through our arteries and which are indeed dangerous. Dave and his team are doing a stellar work. Hopefully more doctors get on board and help with the findings.

I have 5 lipid panels going back 14 years to 2008 with LDL levels ranging from the lowest of 167 (in 2011) to the highest of 264 (in 2021) with the LDL levels overall gradually increasing with time (and trigs dropping and HLD increasing) as I progressively lost weight (about 30 lbs) and have been within a normal BMI the last 2 years. Just had a cardiac CT calcium score done. It was ZERO! Chest xray was normal, 12 lead EKG was normal and so was the treadmill stress test along with all kinds of blood tests for heart, kidney and thyroid functioning - all normal. Despite this, 4 docs want to put me on a statin and I'm still holding my ground, adamantly against it. This research, independent of big Pharma influence is so badly needed. Thank you for doing it. If I can contribute as a participant in any of your studies, I'm happy to do so.

I've been Keto for 2 1/2 years. I'm 59 and this lifestyle has done great things for me. I lost 20 lbs, lowered my A1C from 59 to 53, cut my blood pressure meds 80% and yet stabilized and improved my blood pressure, raised my HDL and lowered my Triglycerides. Lots to be happy about but my LDL'S doubled from 100 to 200. My triglyceride/HDL ratio is 1.34 which I think is good BUT I must confess that the LDL does scare me some. I had a complete heart check up this year and came through with flying colors. I wish that I had a doctor that I could go over my numbers with who understood or supported a Keto lifestyle better but I don't.

HDL: trig ratio is best at 1:1 or higher base on mg/dl. Base on mmol HDL:trig is best at 1.5:1 or higher. Takes 1.5 years to loose 17kg and another 2.5kg to go. Not on keto but low carb. 😂😂😂.

The best way to know is to have a CAC test, and the heart usually does not fail due to high cholesterol or clogged arteries in most cases it’s the muscle due to stress combined with other existing issues.

@@247drycleaners9 don't put too much faith in calcium scores. I went from 0 to 85% blockage in LAD in 3 years. NO lifestyle changes - fit and healthy. Apparently can occur in 5-7% of cases and calcium tests are not that reliable

It seems obvious after watching years of experts discuss mechanisms and studies that a diet of fat and carbs together results in arterial disease (due to damage from glycated and or oxidized fats) ... but either high fat alone or high carbs alone does not. So, this research goes some distance in showing the former may be an almost completely ignored heart-healthy option (that comes with dozens of other benefits, from intestinal health to brains and hearts using ketones, an efficient, less metabolically damaging energy source). You guys are true health pioneers, regardless of how the studies pan out. Thank you!

Your work is very intriguing and fascinating. One thought I have always had over the years has been, what context do the studies on cholesterol exist within? I would argue that the specific cohorts and proposed outcomes in the studies exist within the context of standard diets. I’ve had this debate with my colleagues in cardiology. Usually occurs in the lounge at breakfast. I’m eating my eggs and bacon, and they’re asking me if I’m not concerned about my heart disease risk, all while they’re eating pancakes and French toast doused in processed syrup, drinking a sugary Minute Maid orange juice etc. I’ve been strict carnivore for the past 18 months or so. Feel the best I ever had. Boundless energy. Leanest I’ve been. 53 yo. 6’5”. Hover around 210 lbs and 11-12% body fat. Resting heart rate in 40’s. Exercise in some form daily. Usually weight training 4x per week and daily 15-20K steps outside in sun as much as possible. My recent labs 2021: Total cholesterol 347 LDL-C 240 HDL 100 TG 51 Apo B 130 LP (a) 25 hsCRP 100 etc. I guess epigenetically my genes were switched off back then? The other argument I make is, I know a person who is 400 lbs with a BMI of 75. Their A1c is 11. Hypertensive. And their most recent numbers were: Total cholesterol 160 LDL-C 110 HDL 35 TG >100 So based on the common dogma of the lipid hypothesis, this person would have a lower CVD risk than me, and I would be the one needing a statin. I point to studies like Framingham, and others, which suggest that the issue of LDL-C and CVD risk essentially disappears in the setting of metabolic health and insulin sensitivity, high HDL etc. Would be interesting to see long term data on cholesterol in the setting of metabolically healthy, clean eating cohorts. It would seem anecdotally that the overall health is the main metric. Thoughts?

I’ve always had healthy cholesterol and triglycerides until age 57. My triglycerides was 600 and cholesterol was 300. I had an adverse reaction to statins and ended up in the hospital. My blood sugar was elevated. The blood test was positive for hypothyroidism. Once I began Thyroxine, Lipitor, Gemfibrozil, and avoided high fructose corn syrup, and modified my diet, everything went back to normal blood range. I’m now 66 and my blood chemistry is normal.

Yup. Just remember folks, Your Doctor works for you . He is just like anyone else that you hire, plumbers, mechanics etc. If you don't like his work, fire them and find another. especially the ones who wont listen or can only write prescriptions.

I've been on statins for about two years now. I'm not convinced I need to be. But, I don't want to have a heart attack in the future, and be told "I told you so." I'm doing the best I can with the information I can understand. It sucks that money and greed gets in the way of solid research.

I started Keto and IF about 1 1/2 years ago. I was obese and prediabetic with a good amount of inflammation. I lost 50lbs and I am within my weight range now. My total cholesterol and LDL shot up. But my REMNANT cholesterol or VDL is 5, which is the lowest number. My carotid arteries were pretty clear about 9 months ago and they are probably clearer now. My inflammation is gone. I just turned 60 and I feel like I am 25.

Dr Paul Mason has a great lecture on this. Also Dr. THEUT of Wausau wellness. First LDL ISN’T Cholesterol at all, it’s a lipo-protein, so calling it LDL Cholesterol is incorrect. Second, Having high LDL clearly does not equate to having CAD. Having high TRIGLYCERIDES does however. Triglycerides are made up of Three Sugars and one fat. Having SMALL DENSE LDL clearly causes CAD. Small dense LDL happens when LDL becomes glycated and oxidized. That is caused by a diet high in carbohydrates and low in antioxidants. Smaller denser LDL sinks farther into the epithelial layer of arteries where it isn’t supposed to go, and when that happens your bodies macrophages consume the LDL and turn into foam cells, which grow into plaques in an attempt to plug the “hole” in your arteries. These foam cells grow out of control when your diet is low in K2 (mk4/7), which is carried on your HDL and is designed to sink that far into the epithelial layers of arteries and deposit K vitamins into the area, which kills the foam cells if you have them, so its a multi-headed issue of poor diet consisting of a high carbohydrate diet low in K2 and antioxidants. If you eat TRUE keto, you’ll likely not only never develop CAD, you’ll also heal and reverse any pre-existing CAD. Bottom line, the high carb low fat diet is a recipe for CAD and a high fat low carb diet is a recipe for curing it. Something to note, the diet for the child was not annotated, and likely contained high carb low fat, and even WITH conventional western medical interventional STILL developed CAD, while the assumed untreated adult on a lower carb higher fat diet did NOT develop CAD. Only when the medical field truly wants to know the answers and truly cure the conditions will it be found and published. Our current medical system and medical professionals, despite having a fiduciary responsibility in opposition to it, do NOT seek to CURE these diseases, they seek to TREAT them with continual pharmaceutical interventions, which turn profits. That being said, I highly doubt any upcoming studies to provide truly beneficial data on the Low Carb High Fat (ketogenic) nutritional profile in any subsequent reports, which will leave the medical community still “in the dark” on proper treatment of these “diseases” with a proper human diet.

Dave - Congratulations to you and your team on another publication. This accompanying video is also very well done. First and foremost, I commend the use of conversational English in presenting this technical report. I wish all researchers would take the extra step to present in this format rather than obfuscating the message with extraneous jargon. Secondly, I appreciate LM's willingness to volunteer for this study and the publication of his data. Granted, this is still an "n=1" "anecdote," but it helps push the science forward. Thirdly, LM is 5'7" tall and weighs 119-129 pounds. That BMI is equivalent to 138-149 pounds for me at 6'0". Holy cow. I can't even imagine that. His reported calorie intake of 2,783-3,272 is equally impressive, and suggests I may need to walk a bit faster... Although not specifically tied to the high LDL-C topic in question, I wish you could have included his HbA1c and body fat % metrics for comparison as well.

Page 4 left column notes HbA1c, as well as other markers. Copied below. As for body composition, it wasn't clinically indicated to get a DXA at each timepoint. Weight was monitored with a withings bioimpedence scale, which isn't accurate but has decent precision. The issue was that the error gets amplified at very low body fats and at the beginning the patient's body fat was below level of detection, increasing to ~4.5% at peak BMI. We also had 7-skin-fold caliper measurements confirming a modest increase in body fat with BMI. But, again, the error was sufficiently high given the absolute low levels of fat - and the trend consistent w/ BMI - that it was determined to be 'distracting' or superfluous information. @@nicknorwitzPhD Thanks Nick. I'd still estimate 3% essential BF at the low BMI, which would put his 10 pound weight gain at 2 pounds fat and 8 pounds LBM. Your paper suggests a correlation of LDL-C with weight/BMI whereas other researchers have postulated a correlation of T2D with (edit - not subcutaneous) visceral fat. I guess I was wondering if there was a corresponding increase in his HbA1c and fasting insulin over this time inversely correlated to the LDL-C reduction. This would bring in the concept of nutrient partitioning on his hypercaloric diet phase, and perhaps narrow down the zone where primary carb energy trafficking intersects fat energy trafficking, almost like a long term RQ score. I'm looking for the Theory of Everything. Good luck.

@@bidnow2946 I like your thoughts. Here are some responses: First, I don't think other researchers have postulated much with respect to the LEM. That's largely the domain of myself, Dave and our collaborators. That said, I would speculate high VAT and IR has "veto power" over LDL-C increases, but that in metabolically healthy persons SubQ is quite influential. As for HbA1c and fasting insulin in this case report, no meaningful changes in part because they were/remain so low (and the patient isn't IR), the diet is CRD, and the 1w tracking periods prior to each lipid measurement don't represent dynamic weight gain/loss necessarily. RQ near 0.7 throughout. Carb intake consistently in ketogenic range.

High LDL is just a datapoint. Cholesterol buildup in arteries is as a result of inflammation of the arterial walls (cholesterol acts as a bandaid), caused by an excess glucose or insulin or other dietary or pathogenic sources of inflammation. A person consuming large amounts of carbohydrates will more likely have a high A1C from chronic high blood glucose. A person consuming a ketogenic diet heavily favoring fats will have high cholesterol numbers simply because they are transporting fats in their bloodstream (cholesterol is the transport mechanism). Clean Keto emphasizes not consuming inflammatory foods (including sugar/glucose) so high LDL/HDL in the absence of inflammation markers (A1C) has no correlation with arterial blockage. High LDL AND high A1C is a cause for concern but otherwise they are just datapoints that must be considered together.

My LDL has been over 300 in every blood test I've had since I started low carb/keto several years ago, but my HDL has also been relatively high, and my triglycerides low, as has been my IR score, and most markers of inflammation. Plus, my LDL keeps testing as nearly all large pattern A, with hardly any small pattern B. I have yet to get a CT scan and score though. I am currently at the 3 month mark of carnivore diet, and am having extensive blood tests done next week, so it will be interesting to see the new results. Also, I am 60 years old, 5'11" and currently 148 lbs.

Your results look good, and with a BMI of 20.6 perhaps you can also be classified as a lean mass hyper responder. Plus your LDLs are mostly of the good type. You should consider the S-Apo B and S-Apo-A1 tests and then check your Apo B/Apo a-1 ratio. Many consider this to be a far better indication than LDL. Also check your Lipoprotein (a).

Recent studies suggest that particle size has very little bearing on cardiovascular risk. The large buoyant argument is largely a myth to justify a diet that has regularly been shown to increase LDL. High LDL numbers have routinely been shown, in studies that come out regularly, to be very correlative to cardiovascular risk and particle size is a nearly meaningless marker.

Similar situation to many here: Having to order my own advanced lipid panels because Dr. will only prescribe statins...no serious testing done. He blew a gasket because of my test numbers after 3 months carnivore, but I'm not panicking. Cholesterol: 480 HDL: 121 Triglycerides: 175 LDL: 325 Don't like the triglycerides numbers and seriously need to know the small particle LDL...but I don't smoke, rarely drink, I'm not overweight, I exercise, my A1C numbers are low 5's...not a time to panic into a bad decision.

I've seen some doctors talking about Carnivore diet. Are you low carb, or Carnivore? The randle cycle is responsible for your triglyceride level. Your cells have two highways going into it. One for carbs, one for fat, but there is only one processing plant (The Mitochondria). When you eat a meal with both carbs and fat combined, OR eat a meal overloaded with carbs/Fat, your Mitochondria will start to throttle the highways. This causes your blood glucose to rise with carbs, but also causes your body to send excess fat away to be stored. Excess fat is carried by Triglycerides until it reaches a place to be deposited. Basically, your body in its current state cannot handle the amount of fat you are putting into it. This could be because of age (Our Mitochondria get less efficient with age), genetic factors, or because you are also eating carbs while eating fat. I've seen doctors recommend changing your fat/protein ratio and it should clear up your triglyceride problem.

My brother has always had low cholesterol but his doctor wanted him to take statins as a preventive measure. this is crazy. He is 70 and is in great health. I told him don’t do it. I had high cholesterol on the other hand and started keto and IF . Lowered my A1C and lost 20 pounds in 3 weeks.

You didn't mention anything about the size of the cholesterol molecules found in this study. I think I read that although one may have a high cholesterol level it is the size of the cholesterol that matters most where the larger the cholesterol molecule size the easier it is for the body to flush out these molecules. The smaller the size the more likely the cholesterol molecules will tend to stick together and cause plaque. My understanding from what I've read is that people on a high fat low carb diet will produce large size cholesterol molecules that are not prone to sticking together to cause plaque. I think I read this in the book "Wheat Belly".

This data is the correct terminology. I don't know why Americans are getting this confused all of a sudden. Data is group of datums - a set, a collection, and thus is singular (one piece of information is a datum). Even if you have several 'lots' of data you don't call it just data, they're called data sets. Real life example: So when you have a group of dogs the term is pack. You would then say 'this pack', not 'these pack'. So dog is the single name and pack is the group name. Same with datum and data. Hope that helps! :)

I just did my blood work after two years of more keto diet. Total went up from 169 - 183. But HDL went up a lot from 54-71. TGL went down from 56-35! And LDL about the same!

64 years old, white, 5' 10" starting weight 225LBS - Been on a Keto diet for 10 months basically I have one meal/day & that meal is 4 eggs with either steak or bacon. Plus 5 days/week of 1 hour/day weight training. End results: 35LB weight loss, cholesterol went from 260 to 215 morning boners came back & my wife is so excited over my changes that we went from a sexless marriage to her jumping my bones twice/day & she is also trying to lose weight & improve herself. Bottom line: The Keto diet works & it's very inexpensive to follow.

The guy is a lean 26 year old. He still has a good 15-20 years before heart disease becomes a major risk factor. All you middle aged men with high cholesterol and possibly hypertension are kidding yourselves if you think you can compare your health to this person. But what ever helps you sleep at night.

There are a few things to note on this subject. Number one the studies. HDL cholesterol is not bad and does not cause heart disease. It’s only the small, dense damaged particles that are risk factor increase to heart disease. That’s not the cholesterol. That’s the problem. It’s the inflammation the oxidative stress and inflammation caused by process, food chemicals, and Grains and other plant toxins that cause inflammation that damages these particles makes them smaller and more dense where they aren’t processed properly by the liver and those are the particles that interact with plaque and cause heart disease risk increases. If you’re eating a clean, healthy keto or carnivore or Mediterranean diet and eliminating the inflammatory agents then your cholesterol is all healthy. That’s the difference between clean keto and dirty keto come off you’re still eating soybean oil and seed, oil‘s and chemical foods that cause inflammation you’re still at a higher risk for disease. A dirty keto diet is only good for losing weight but it’s not a healthy diet. But if you’re eating a clean, healthy ketogenic diet, whether it be Mediterranean carnivore or keto, you’re eliminating the inflammation and the oxidative stress that causes inflammation and eliminating the cause of 95% of what we call disease, including that which damages your lipid protein cells, and causes them to become smaller and more dense and increase risk factor of heart disease. So if you’re eating a clean diet without the seed, oils and process food, chemicals and inflammatory plant toxins, and your cholesterol is irrelevant. Also, the death rate among people with cholesterol in the 140 range of HDL are the healthiest people and live the longest. Another factor is the medical community. They make these numbers ridiculous because they profit heavily from statins, and they like to scare people into buying drugs. They don’t really need. So 100 and your LDL is not high cholesterol that’s a very low cholesterol. 150 should be the borderline high cholesterol for LDL if you’re gonna go by the studies and the data and not the conclusions of people trying to sell your drugs you don’t need. Another medical community problem is the way they test your cholesterol. They tell you to fast and not eat anything the night before so your body is burning fat heavily when they do your blood test and they do this for blood sugar but when you’re losing weight and burning fat, which is what your body does when you’re not eating , especially if you’re fat adaptive on one of the ketogenic diet, and LDL and HDL are not actually fat or cholesterol. They are lipids proteins that carry fats/cholesterol so when you’re losing weight, your LDL levels will be elevated. When you’re fasting you’re burning fat and your body is working overtime to process stored fat which means you have a very high level of LDL Because your doctor didn’t let you eat anything and your bodies burning fat when they take your blood. This gives falls high readings. The problem is they started selling statins and calling LDL cholesterol bad and most of your doctors don’t have a clue what they’re talking about because they just learned HDL good LDL bad and go by the numbers they were told. 99.9% of the doctors out there never read the research and have no clue what they’re talking about. They’re just following policy and procedure And their educational curriculum which were all created by the pharmaceutical industry who doesn’t care about your health they care about selling drugs that they create for profit and they’re the people who teach your doctors and they don’t teach them to heal UA, teach them to treat symptoms of disease with drugs and expensive medical procedures that are very profitable. So high cholesterol isn’t a bad thing it’s a demonized and they tell us it creates heart disease mainly to sell statins which bring in tens of thousands of dollars a year per patient for the pharmaceutical companies making them. People who eat a carnivore diet or a clean, ketogenic diet, eliminating the inflammation and eliminating the small dense particles, which are the only kind of cholesterol that contribute to heart disease and even then it’s only the number eighth risk factor the number one risk factor is type two diabetes which is caused by eating excessive sugars are carbohydrates. A carbohydrates not a nutrient. It’s just a measurement of sugar in food whether it’s a monosaccharide or polysaccharide it’s still sugar, so whether it’s coming from Fruit or from bread or from a candy bar it’s still sugar and your body still rejects it and doesn’t like it and it causes damage. So a carbohydrate is not a nutrient you never have to eat it carnivores don’t eat any and they skyrocket their cholesterol to 300 or 500 or even 600 points, yet they simultaneously cure their heart disease because cholesterol does not cause heart disease and LDL cholesterol is good for you, in fact, you’ll die without it and it’s part of your basic nutritional make up. Your doctors don’t know what they’re doing. They don’t understand particle size because they’re never studying this unless they’re cardiac specialists and they were just thought the LDL was the Devil and they prescribe statins, because if your LDL is over 100 you’re about to die of a heart attack which is complete and total nonsense. What they need to tell you to do is stop eating sugary fruits and green products and cut way back on carbohydrates which will eliminate type two diabetes, 100% of the time and illuminate the number one risk factor of heart disease. You’re better off having skyrocketed LDL cholesterol and getting rid of your type two diabetes because it contributes to more heart disease than cholesterol and exponentially so it is far more dangerous.

same here, 49 years old, weighs about 60kilos and 5'7" in height. My LDL and HDL are up and my trigly is very LOW. Joint pain gone thru KETO but when I went back to carbs for 2 weeks, I got GOUT flare up and too painful! Need to go back to strict KETO to feel young again!

I think the results here can be attributed to one primary factor. When you’re burning fat, your LDL goes up because LDL is not cholesterol or fat it’s a lipid protein, cell or package that transports cholesterol/fat. When you stop losing weight, your LDL will drop naturally, because your body is not burning fat whenever your body is burning fat. Your LDL levels are accelerated. That’s why your doctors don’t know your cholesterol level even when they take it because they make you fast before they take your blood which means your cholesterol levels will be highly elevated because your bodies burning fat when they draw blood, because they tell you not to eat so you’re fasting a.k.a. burning fat so your LDL levels are going to be very high, especially if your fat adaptive on a ketogenic style diet or carnivore diet. So all the results on these charts talking about cholesterol levels can be attributed to the rate it with your burning fat. When you’re gaining weight because you’ve added carbs, your body is not burning fat and that’s why your cholesterol goes down but it doesn’t make you any healthier. It’s just not getting rid of fat a.k.a. losing weight. When you’re losing weight, your LDL is going to be elevated because that’s the transport mechanism for processing and transporting that fat, and if you eat a bunch of carbs and gain some weight you’re not burning the fat anymore and that’s why your LDL drops. LDL doesn’t cause heart disease anyway it’s small dent damaged particles that crate, heart disease and that’s from inflammation and oxidative stress not LDL.

I live in Spain and my whole family is from Spain and our diet is the so called mediterranean diet. Well I have never seen my grandparents eat avocado. Mediterranean diet has many chickpeas and lentils,vegetables,nuts,olive oil ,cured cheese and cured meat (jamon,lomo,salchichon,etc..),fruit and some meat and fish ,potatoes.... but also it has a lot bread,sweets and pastries,churros,cookies,magdalenas,and so on.

Not everyone needs to give up sugar and carbs. But hanging out in the low-carb community makes you feel that everyone should eat zero carbs. However, that's not the case. Many centenarians eat bread and sweets and take a drink every day.

I don't want to live forever anyway. I want quality over quantity. Ketovore is right for me. My doctor is on my team and supports me. I drive over an hour to his office. It's worth it. I don't need frequent doctor visits. He writes labs on anything I ask for. I've had uncooperative doctors before and I fired them. Find a cooperative doctor. You are worth it.

My doctor want me to take a statin and go off keto because of the lipid panel results. I don't want to but will take the statin if necessary. Total cholesterol-266, Triglyceride-90, HDL-51 and LDL-197.. I am a 58 year old female.

I know we’re a few years of not decades away from finding out for sure what the LDL number means but I predict that high LDL will show to be either beneficial since it’s used for so many functions in the body or it will be transient where it either stays that way for awhile until the body fully adjusts or those that transition to a LCHF diet eventually end up with high LDL because it’s the ideal state for the body to maintain. Either way I think we’ve been tragically off in our perception of what LDL’s function is and it will be shown to be a very healthy factor by having high LDL in the presence of a LCHF or carnivore diet.

This happens to be the only objection the medical community can come up with for me trying to control my weight, inflammation, sugar levels, blood pressure etc, etc, etc. I’ll listen to my body, I’m 63, ex smoker(14 yrs) and was in enough pain daily to severely limit me at any activities. I’ve lost 45 lbs, spend most days with little pain, I have had a rather active life, and feel better than I can remember. Wonder what we’ll find out about the intermittent fasting? 😁

Cholesterol is not the problem. It's autoimmune issues and inflammatory states due to genetics in some people like individuals with RF and consumption of inflammatory foods and liquids. The great problem with the medical community is that they do not take the body as a whole. They separate and specialize in say gastroenterologist and cardiologist, when in fact both organs are associated with each other and are effected by each other. Everything is connected. And doctors do not think in those terms....

I'm still waiting for a conclusive study that shows the relation between high LDL levels and plaque build up. I've had high cholesterol all my life (around 250) and a recent angiogram showed I had clear arteries around my heart. Anecdotal evidence, I suppose, but what other evidence is there out there? I'm 75 years old BTW, and I don't take statins, contrary to my doctor's advice.....nb : I have exercised all my life in one form or another, I'm quite active and I take care of what I eat......

Last year after being on keto about 6months my ldl was really high...i was loosing so much weight and began to look too skinny...so I introduced some carbs...and ate 3x a day...starting having sweet potato.... rice...the next test 3months later my ldl had come down, the doc was happy..however...it has taken ages to put on even half a stone...but feel good still and can't help but keep up the keto...mostly!...thanx for the info...can relate to that...

I started keto in 2017 and never looked back. just got my CAC score, a very good indicator of heart arterial plaque. my score is zero, that means I have no or very little plaque and this is someone who followed the food pyramid for 46 years. I'm 52.

I wonder whether the BMI is the really issue or weight gain/loss (i.e. the direction)... A fun fact: the Uric acid ALSO seems to be high while loosing weight and be low when gaining weight (as long as you don't consume fructose/sucrose which will make the UA levels high anyway, especially when gaining weight...)

My theory is, Cholesterol is one thing if you're pumping sugar through your veins as most people are these days or.. if you're sugar free and keep a low blood sugar level (which leads to a low insulin level). Insulin is the killer, not cholesterol. It's already been shown that insulin causes heart disease and plaque accumulation as well as a BUNCH of other things that are killing people these days.

I wonder what that baby with FH was eating for a diet. Likely baby formula with a bunch of unnatural junk in it, then pureed fruit/vegetables, and on to processed junk most people eat once they get teeth. Is this factor known? If not I wonder if their arteries would have had plaques on keto/carnivore? I honestly doubt it, because damaged LDL particles and inflamed arteries is the problem and eating processed junk "food" made in factories is how that happens...

Dave, when you use the term "ketogenic diet" how do you define that? Many scientists make the mistake of calling a very low carb diet a ketogenic diet. In my opinion a ketogenic diet is a diet that induces ketosis. The only way to know if a diet is ketogenic is to measure ketones. if you don't measure ketones, you have no idea if the diet is ketogenic or just a low carb diet. Ketosis can vary with regards to a particular diet from person to person and also within the same person. Someone can be on a keto diet one day (producing ketones) and then stop exercising while remaining on the same diet and then not produce ketones. Thus, the diet does not determine if it is ketogenic or not. The production of ketones is what determines if a diet is ketogenic.

After a while you can feel when your body is making ketones. Plus if you know what has carbs and avoid it the. You don’t always have to test. I maybe test once every couple weeks. Testing often is reassuring though but not necessary.

I know a lady who thinks she is in ketosis and never tests. She is fat and not losing weight. My theory is that she is not in ketosis, but without measuring, she will never know for sure.

Heres the thing, there is fractionated LDL , subfractions 1-7. If its high in subfraction 1-2 then even with astronomically high LDL there is v little chance of Artherosclerosis. Add on the to fact that there is oxidized vs reduced LDL as well as high angstrom vs low angstrom LDL. One may have high LDL but if it’s the low subfractions (predominance) , reduced LDL and high angstrom LDL. You can walk around with high LDL and have no Artherosclerosis. The converse - low LDL , but high subfractions + oxidized LDL and low angstrom LDL. Then trouble brews. Notwithstanding. Homocysteine and Lpa1 levels- these add or subtract to the problem. Its not a deficiency of a statin that people have , it’s the SAD that worsen ANY level of chol to a lethal form.

The high levels of LDL cholesterol on keto diet is due to what type of fats a person eats. Does that person eats the type of fats that raise the LDL cholesterol levels or the type of fats that raise the HDL cholesterol levels? It's simple as that. Unfortunately there are so many people on a wrong version of Keto diet.

Mayo clinic report says high hdl and ldl, with low trygiserides means fat is used for fuel. Another study says low trygiserides allows ldl particals to increase which enriched white blood cells. 8 years for me, no flue, low blood pressure. Why don't trygiserides get more attention?

It’s so similar to my situation. I am 26 years old male. I eat no more than 60carbs a day. In my family nobody had problems with cholesterol. My great grandfather and grandmother died when they were above 90 years old. LDL 224 HDL 70 Triglyceride 57 Dear Mr Feldman, If I can be useful for your research. Just let me know. I am from Germany.

@@tomace664 I changed my diet. We have to eat everything, there should be a balance. The Keto diet sucks actually. Now I consume 250g of carbs a day and I have sooo much energy. Before that, I couldn’t train properly. I was tired all the time. No need to remove or to eat below 40g carbs. It is just wrong. That’s at least my experience. That’s to say, we shouldn’t go from one extreme to the other. A balanced lifestyle and eating habit.

Great presentation, clear and interesting. I've been keto for 2 years and my total cholesterol went from 189 to 341 (unfortunately I don't have the details of the older one) but this new one of 341 shows triglycerides at 79, HDL at 68, LDL is 218. I'm a 50yo woman, 163cm tall and weight 50kgs, so maybe I don't exactly fit the criteria of LMHR but I'm close... Anyway, I feel fine and for now I'm not worried. In the meantime I keep looking at your research 😊. Thank you very much for your work!!!

Your triglycerides to HDL ratio is 1.16, which is good. This is way more important than high LDL. My LDL is 73 but my triglyceride to HDL ratio is 2. Not real good. I got more work to do.

Unfortunately in my case, after being on a low carb diet 18 years and my cholesterol continually going up (LDL over 350), I had a heart attack and a 70% blockage in one artery and 50% in two arteries. I now have a stent and have to be on a statin which I swore I'd never take. But it is what it is. I'm still trying to figure out why. Cardiologist thinks it's probably in my genes. I don't know.

Maybe look into the book The Clot Thickens. They hypothesize that it's clotting factors that cause these issues genetically, and most FH people have clotting factor issues as well. Maybe you could get on a blood thinner or some such which may help.

I am taking Plavix, metoprolol, Losartan, Crestor and Repatha. Plavix is the blood thinner. Will probably be on it for at least a year or more.@@mikafoxx2717

The case study you presented didn't take into account the patient's age. Although his CT score was zero, that doesn't mean there was no cholesterol plaque which would turn into calcium. As you know, it takes time for that to happen, and it happens in places where cholesterol plaque had been laid down in the arteries. In other words, the calcium is the end-stage of atherosclerotic lesions.

Keto for 3 yrs, then switched to carnivore and higher saturated fat this year. 70 yo, BMI 18.8, HDL 94, Trig 86, LDL 214. CAC Agatston score 2 years ago was 3.78 and will do again this year. (If I can get insurance to pay for the better CCTA scan will do that instead of self-pay for CAC) Not quite LMHR but really close.

I don't understand the point of this. It's as if you don't understand that it's the small dense LDL that causes atherosclerosis? When not consuming high amounts of glucose the LDL is the healthy, non-damaged type (A) that just absolutely does not cause damage, hence why this guy has no plaques. When he's off carbs his LDL rises to it's NORMAL level. When on carbs it drops to it's ABNORMAL level. Please explain why none of this is mentioned?

The patient probably had low insulin levels that doctors don’t measure. That could be the reason for the absence of plaque.

Thank you for the video, I have a family history of heart disease and am a lean mass hyperresponder with very high LDL. I have some plaque, but I am hoping that my low carb approach will slow the plaque, I am one of the hundred person heart study patients so hopefully by the end of the year I will know if this low carb diet and high intake of saturated fat is helping or hurting my arteries.

I met with the doctor and I was concerned because my calcium score went from 27 to 86 or something like that. The doctor said I'm looking at your arteries from a year ago and whatever you're doing keep doing it. They haven't changed it at all. He said my calcium score is probably rising due to Plaque hardening. That was great news for me, I started lightning up a little bit on how strict I was but I'm still 90% carnivore. I added things like coffee with cream butter and a little bit of honey, I'll have milk with magic spoon cereal from time to time and it doesn't seem to cause any problems. I work a ton so I'm not able to do a whole lot of exercising so I just do push-ups at my desk every day and walk to the top of the parking structure backwards. I try to get 200 push-ups in by the end of the day and that seems to keep me in pretty good shape.

LDL is your aircraft carrier and HDL is your aircraft, you need both . My Keto experience led to 3.8 times the normal LDL levels although the HDL and Triglyserides were good. So for a couple of years I had too many aircraft carriers. Now LDL is normal as is everything else , and Doctors still push statins ? Cardiologist could not believe the ole heart was functioning as it should.

Instead of trying to understand why the cholesterol is high, they just try to give you meds to lower it. Damage to the arterial walls are result of friction caused by gyration and oxidation. The cholesterol is there as a reparative measure. It then hardens into plaque. Physicians are basically taught, diagnostics and treatment of symptoms either pharmaceutical and surgical interventions or both…

LDL as we know is a poor indicator of atherosclerosis. Latest evidence points to high apoB. Regardless, it's pretty clear that saturated fat is not the culprit but excess sugar/refined carbs, smoking and excessive alcohol.

My reading of this is that with more body fat, you mobilize less LDL from the liver. Would that be an over-simplification?

I used Keto for about a year to lose weight and it worked (25lbs). But and there is always a but, I really started to feel bad physically after a year on Keto. At the year and 1/2 mark I switched to a "whole food plant base diet." Now three years later, the weight is still off and I feel great. I am not 100 % plant based. I eat a small amount of meat (4oz) twice per month and a little ice cream (2oz - plain vanilla) each night. Got to have at least one vice. Lol. I am 60 and in good health. My experience with Keto in a nut shell is that it helped to lose weight but I should have switched to the WFPB at the 8 month mark. I can see where specific individuals need to be on Keto and if that works for them then that is great. It seems like a majority of studies that are published and peer reviewed show significant increase of disease process with the more animal product you eat and associated saturated fat. Especially as you get older. Just a thought.

Hey Dave, can you tell us where the magical 200 or less total cholesterol actually came from. I read a story way back when that it was pulled out of thin air by a cardiologist at a heart doctors convention ? I've been studying cholesterol since 1991. Because my father had a quadruple bypass , due to what the docs called high cholesterol blockage. Which we now know is bologna. I believe that if we keep our blood sugar low at all times, with a low carb higher fat diet, everything else will take care of itself.

I would expect that the FH children were not on a high fat keto type diet as it would be counter intuitive to mainstream medicine. You can imagine the child’s doctor’s responses to the parents..”you want to do WHAT!!!”. Really, we need a FH parent to step up and put the child on a keto diet and review.

I also wonder about variances in men vs women….most of the keto/carnivore channels are from a male perspective….I wonder if the complexities of female bodies (e.g. hormone cycles) have an impact on BMI and fat metabolization in a different manner? It seems like men lose weight much quicker on these diets than women, particularly post menopausal women who already ate a healthy whole food diet before switching to keto or carnivore….

I wonder if keto might help people with FH to prevent plaque, as it seems to be possible to have high LDL without getting heart disease. Possibly historically there has been little natural selection against FH genes, because it is considered one of the most comon genetic disorders in the world. Perhaps it is much like type 2 diabetes, which also has a genetic component, but doesn't manifest itself unless a person follows an unhealthy lifestyle.

I wonder what the patients HDL and TG were. Having those labs would be extremely helpful in determining his risk for atherosclerosis. High HDL and low TG ratios would mean high LDL isn't a concern.

It's reported in the paper in Table 1. PUFA ranged from 9% to 20% of fat intake, the former on this high saturated fat diet (timepoint4) and the latter on the high PUFA diet w/ sesame oil (timepoint3). For the two highest LDL-C readings, when BMI was lowest, PUFA was 13-14%.

My wife's cholesterol was slightly high 3 years ago when she changed her diet. Lower carb diet. Last December she had it checked and it has gone up to 851 total cholesterol. Her triglycerides are normal, blood sugar low, Oxygen 100%, blood pressure is awesome consistent of a runner, she's lost 25lbs.+. She had a CT scan of her heart and had a "0" score. She had gone Keto about 2 years ago. After seeing videos like this she has added some carbs back. Now we are waiting a while to see if it her cholesterol will come down.

I should also add that both her general practice and heart doctor want her on statin's. She said "NO" to many side effects and they really don't do much.

What about non lean mass responders? I went keto, was in ketosis for 1 month, didn't lose a pound, felt horrible. Nauseous just looking at my plate by the end of the month. I have a hereditary thing where my liver makes too much cholesterol. Huge heart disease my family on both sides. Had a 3x bypass at age 58. I really think I do best on mostly plant based, low sat-fat diet now. All this information and keto stuff is very confusing. I think everyone is different. Glad they seem to be teasing this out a bit more.

Paul Mason vids on cholesterol statins I'm 70. followed SAD, AHD Diet recommendations and earned my CABG in 2015. been keto/ carnivore since 1/2022. PHD cured my NAFLD, pre-diabeties, CKD, high BP, increased testosterone, and lost 40lbs.
