My dad once worked in a factory that bent and folded pipes and rods. His boss had a new girl, a teenager with two weeks at the company, operate a machine without training and a busted safety button. Two had to be pressed to activate it, but it was running with just one. The poor girl lost three fingers and half her palm. You know what management did? They sent her home, had someone wipe down the blood, and called another worker over to continue the work the girl was doing within ten minutes of the incident.

Somebody explain to me what's supposed to be locked/tagged??? It's a passive, gravity-powered roller-line that the boxes (which are clearly quite light, and moving at a blistering speed that looks like it might approach being as much as a foot or two per second) slide down to wherever they're going. The boxes are in more danger of him leaning on them and crushing them than he is of being hurt by having them hit him while he's doing whatever it is he's up to.

Hello, I hope you are well, it is important that we look at the whole picture of work, although it is true that the conveyor is serious, but let's analyze: 1. The maintenance personnel (I guess) is doing 3 simultaneous high risk jobs, work at height, apparently is doing welding and control of hazardous energies, that leads to a more risky situation. 2. The maintenance personnel is performing the work at height, if at some point by the movement of the conveyor, or jamming with several boxes this can result in a cumulative situation that can generate an incident. Remember that there is not only electrical energy considered as dangerous, but also gravity, pneumatic, hydraulic, etc. In addition, the LOTO procedure is very clear, 0 energies that can generate an incident, it is a matter of risk analysis for the prediction of incidents. I hope I have clarified a little bit the doubt. Greetings

This happens at every plant. I'm NOT defending it, but I DO understand it. I lived it for 26 years. Every plant has all the right paperwork and documentation in place in their files. EVERY worker on all the lines signed off on the training and they know to LOTO and safety is "number one" and "understands what needs to be done and takes full responsibility" and all that stuff ......... if you get my drift. LOL. In other words, the company legally covers their ass, and if something catastrophic happens, it's on the one who is injured or worse. After all, here is all the documentation signed by "mister so and so". Now, try actually adhering to those nice sounding and looking, on paper, policies and watch production go down by thirty or fifty or more percent for every single thing that should be "properly" locked out, and see how management reacts. It's just the way things are. The company, the employees, and even OSHA all are more than fully aware of how it works.

What risk? Getting his back or shoulder bumped into by a box that probably doesn't weigh much more than a pound or two as it slowly rolls (due to gravity) down the line? By the look of things, they were coming fast enough that he MIGHT have actually been able to feel them when they bumped into him. If this were a powered conveyor, it'd be a risk of some level, but what we see here is about as dangerous as sitting on a countertop to wipe cobwebs out of the bottom of the overhead cabinets above it.
