Step by step animation showing how an Asian monolid is surgically converted to a Caucasian double eyelid using a suture technique without incisions. Caucasian upper eyelids normally have a skin fold, or supra-tarsal crease, that appears with eye opening. Asian upper eyelids typically do not have an upper eyelid crease creating what some patients feel to be an undesirable monolid.
The absence of an eyelid crease is mainly due to the lack of fibrous tissue bands between the levator aponeurosis and the overlying skin and muscle. The levator aponeurosis itself is attached between the tarsal plate and eyelid muscle. With eyelid opening, the overlying skin moves with the tarsal plate creating a skin crease. Without these fibrous bands, the skin does not move with the tarsal plate and no skin crease forms.
Asian double eyelid blepharoplasty can fabricate this upper eyelid fold by creating an artificial connection between the levator aponeurosis and skin using sutures. The suture placements can be accomplished after a formal surgical incision blepharoplasty... or without incisions as shown in this video.
