The cervical spine functions to position the head over the body in space and to maintain horizontal gaze. Under physiologic conditions, these functions occur without excess recruitment of soft tissue structures or fatiguing of cervical musculature. As cervical alignment deviates from normal, increased energy is required to maintain horizontal gaze. Cervical spine deformity is associated with increased disability, decreased health-related quality of life, and myelopathy, and cervical alignment is closely related to global sagittal alignment.
