Calm down, people. This isn’t ear wax, it’s cholesteatoma. It’s very hard to remove and damages the ear canal. A Jobson horn wouldn’t have had any success in removing this as it adheres to the canal. The blood is due to the cholesteatoma. Dr. Zhao, I love your videos. Keep up the great work.

I feel so sorry for people who cholesteatomas. What a horrible disease/condition. Dr. Zhao, every time I watch one of these videos, I'm like... no way can he get all of that stuff out! But, amazingly you do! Thanks for sharing with us and helping this patient.

Of all the ones I've seen the doctor do this one actually appears to have left less damage. It was good to see this patient must have sought treatment more promptly. The other ones I've watched the ear canal and drum have almost being unrecognizable. Certainly is a perfect example if people don't leave things entreated.

I really like this doctors channel. His knowledge and skills is obvious. I would watch even more if I could understand what he is teaching. Maybe future videos will have English text on screen or the option of English subtitles bc I really want to hear, or rather comprehend what he's saying.

Sherri Hinton The ones who can afford it. That's why he sometimes stops ahead of being done because it's painful to remove for those who can't get anesthesia and he has to stop because of the patient's pain.

This isn't ear wax, it's a cholesteatoma, a type of tumor that can destroy the bones in your ear and even invade the skull. It's very attached to the all tissue and bone (if it's invaded that far) in the ear. 99% of patients have to be put under general anesthesia for the removal of these because it's so painful. You can't just force it quickly out, to do so would cause more damage. He has to finesse it out. If he doesn't he could cause even more damage than what the cholesteaoma has already done, that is why you'll never see him using a Jobson's horn on one of these patients.

Nicole Phipps that was an amazing description and summary many thanks. I am curious though why a jobson's horn would be more likely to cause damage. As long as he showed Extreme Caution I I would think it would be more helpful in scooping the skin rather than just breaking it up.

@@dixierae6063 good question. Besides the fact that he hardly ever uses the Jobson horne, at all. I would think that the St Barts hook, which he always uses, would cause more damage. Especially using it to scrape the canal walls... But, I may be wrong 🌞

I have bilateral cholesteatoma - both ears :-(. I sure wish my doctor was as gentle as this guy. Maybe he was, I dunno - but he did the procedure on Tuesday, and my ears still hurt five days later.

Just had surgery.Very hard to identify I guess.It took them a year to figure out what was going on.And if they tell you its not painfull there lying.I thought once I had surgery the pain would go away.Wrong I've been in horrible pain. And ZIM ONE WEEK OUT.

I can only imagine it did hurt about like hell. I hope everything is ok for you now. and it doesn't come back. 🌞

Wouldn’t it damage the ear more to push the mass down trying to get a grip on it with forceps than to keep using the hook to clear it out more?! I’m sure suction wouldn’t get a very good grip either.

Nanny B - Agreed! I think the Jobson Horne is underrated and overlooked by a lot of ENTs and Audiologists, for some reason. The St Bart's Hook has its place and works very well for Dr. Zhao in most instances...but the Jobson Horne would make his work go much faster, and as you said this ear is a perfect example!

The day will come when the Dr. Will introduce a liquid that is capable of safely dissolving wax and all other ear canal substance. Within minutes the wax will turn into a muddy slush and will be flushed out warm water.

Well in cases like these the debris is mostly keratin, dead skin. If you put something in that would dissolve that, it's going to attack the healthy skin too.
