Very well explained... I too diagnosed with Otoscelerosis.. before lock down.... now thinking for surgery.

@@aqrab18 yes I undergone surgery in last March '21...my sound level boost 20db... It was previous both ear 40db....Now I will go to right ear surgery..

Vikrant I have one Right ear with 40db and left with 50db. What do u suggest as I am hesitating for surgery

@@aqrab18 ENT will do Rinnie Test with fork... If you have CHL means Conductive hearing loss and difference is more than 20 dB then ENT will suggest surgery... It can be test by putting vibrating fork near ear and behind ear... Direct signal to brain... You may check video on KZitem about Rinnie test

If the sclerosis is in cochlea where the endochondral layer is present shouldn't this be sensorineural hearing loss.. Stapes isn't affected rather it can't function due to the anomaly in cochlea???

You said that sclerosis of spongy bone will stop sound, how? Sound should travel faster in sclerosis as sound conduction in solids is greater than in liquids as it is a longitudinal wave

there may be 2 reasons. 1.when there is otosclerosis, it leads to fixation of foot plate of stapes with oval window so that when sound vibrations reach from stapes to oval window it can't pull oval window inwards so no movement of perilymph ,no movement of basilar membrane and no sound conduction. 2.spongy bone is unorganized bone while the endochondral bone is organized and conduct sound waves more efficiently so when sound waves enter from stapes to spongy bone in between stapes and oval window then it can't transfer sound waves forwards..
