Thank you!! this helped a lot for my upcoming exam. I only have a question about case nr 5, in this patient I can see that reversibility test is positive with increase in FEV1 >12% and volume > 200 ml. This can also be diagnostic for asthma, so do I differentiate between the two based on the patient history (age, onset, history of smoking etc)?

That's a great question! It's been a while since I made these videos, and I'd need to rewatch them to see if I made this point during them or not, but PFTs are not sufficient alone to distinguish asthma from COPD from other forms of lung disease, or from patients with more than one process - they are only one (though prob the most important) consideration. In this case, even though the reversibility (i.e. bronchodilator response) might point a little towards asthma, COPD is still strong suggested might the low DLCO, age of onset, smoking history, and signs of right heart failure.

Loved how Daenerys is an animal trainer, Bran has a spinal injury and Baratheon is obese =)) the last one in particular really cracked me up. Also, thank you so much for all your hard work!! It's incredible how well taught these videos are. And solving the cases gave me great confidence. I never really liked pneumology before (im a 4th year medical student - out of 6 years overall) but now I actually find it quite entertaining and fun - after 2 hours of your video series and a great cup of coffee. You really are inspiring - I think it's quite something to make a student so enthusiastic about a subject :)

Hi,in case two, why we say to FEV1/FVC elevated ?i see refe is 84 % , but one can not have exactly 84 % to be considered normal , right? so which number considered normal ?should the ratio number exactly be ref number to be considered normal ?

Dr. Strong, when you draw the flow volume loops for the restrictive patient in case 2, should the patient's outline be shifted rightwards? Having it near the left end of the graph makes it appear that the patient has a massive residual volume and normal TLC. Thank you for the videos, you are a very gifted teacher!

Hello, by making post-bronchodilation test, you give B-adrenergic inhalation with salbultamol ect and wait for 15 minutes and repeat the test , if the FEV1/FVC improves then the disease Obstruction is reversible which means it is most likely Asthma and not COPD.

An ABG to determine the A-a gradient provides roughly analogous information as the DLCO (and is much more widely available).

Thanks for making these videos! In the final case I noticed the patient has a relative increase in FEV1 of around 25% after bronchodilators. Would we say this patient has asthma in addition to COPD? or just that the patient has a reversible component to their COPD-related obstruction?

Noted the same. Bronchodilator response meeting criteria of 200 mL and 12%.

Super especially the Cases, however Case 5 especially x-ray was in reality probably Euron Grayjoy, with a skinny hyper-inflated chest, not Ned Stark!! As for the animal trainer, would the case be similar with chronic inhalation of particulates, especially close proximity to burning?

Hello, if TLC is low, does it mean that VC should be also always low? if so,do they both show restriction ?

I don't want to say anything is impossible, but I imagine it's very unusual for a patient with a significantly low TLC (i.e. not just 1% below normal) with a completely normal VC. And yes, that would suggest restrictive lung disease, but I think most clinicians wouldn't consider such a diagnosis as definitive without also having abnormal spirometry. (It's easier to imagine a patient with mixed obstruction/restriction having low TLC with normal, or even high, RV)

@@StrongMed Thank you. Regarding the Diffusing capacity parameters DLCO, DL Adj , I see they are not available in my chinese spirometer machine. i just see about 40 types of parameters of FVC, VC, MVV, MV. So how to get or calculate them?are there other alternative names for them?Thank you.

The normal values for PFT parameters vary based on sex, age, and height. When you receive a set of PFT results, the expected values for that individual (i.e. the normal values for someone of the same sex, age, and height) are usually included for easy comparison.

The 5 videos are super! especially the practice cases. Case 1 is most consistent with Euron Greyjoy, skinny and hyperinflated lungs... The Animal Trainer interstitial picture, would that be similar to damage from particulates, especially chronic exposure to high levels of smoke?

I am wondering why the final patient has such good reversibility on his spirometry? He has signs of right heart failure which might suggest Cor Pulmonale as well.

Ok another question, what would the pft values for tracheal stenosis look like. I understand the graph and why you see the flattened shape due to the fixed obstruction but would tlc be reduced, would fev1/fvc be reduced. I'm guessing it would look like severe obstruction but normal dlco, but I'm not sure.
