In 2018 we issued a Patient Safety Alert ‘Resources to support safer modification of food and drink’ to eliminate use of the imprecise term ‘soft diet’ improvement.nh....
This alert followed reports of patients choking or coming to other harm after imprecise terms such as ‘soft diet’ led to them being given unsuitable food. It asks providers to introduce standard terminology to describe texture modification for food and drink (the International Dysphagia Diet Standardisation Initiative (IDDSI) framework iddsi.org/). The deadline for the actions in the alert to be completed was April 2019.
If you work with patients with dysphagia, or other patients that need the texture of their diet to be modified, and haven’t seen the changes outlined in this video and the alert, you are encouraged to speak to your manager and ask why.
