You can see Al is drawn to the Reverend-he keeps going back and actually listening instead of blowing it all off. You can see just a glimmer of goodness behind those hard set eyes for a second.

To add to this, I think this is touching on something very personal for Al. He mentioned his brother in passing having "fits" like the reverend and I think the reverend is bringing back a lot of painful memories as well.

That is because Sopranos make it inherently possible to do series about "Anti-hero's" IE..Deadwood, Breaking Bad, The shield, ect..

That the reverend did look straight at Swearengin (whether he knew it or not) and Swearengin did look away is confirmation that Al does have a set of hard morals that causes him great distress and turmoil. Compounded by his harsh and brutal view of the world, Al reconciles his feelings through the future he builds for the people in the camp...to him, Deadwood is a sanctuary. Not perfect, but it's a place he can control enough to ensure a life for the people who need it most.

It was obvious all throughout season 1 that Al genuinely liked the Reverand Smith and felt sad and angry that he was suffering so.

There wasn’t much that could make Al turn his eyes away but that was something that he couldn’t abide.

Smith was interesting in that he had the Old Timey Religion thing going (which at its least gets people looking because) but also was portraying affliction that just wasn't manageable by the medical standards of the time - this to me was as insightful as Al's stone, in how the science/medicine of the time could (and wanted to) treat things.
