The full-endoscopic technique is the least invasive approach in spine surgery. With less scaring and less tissue trauma, there are no reported complications in terms of scaring or tissue trauma. This approach also requires less bone drilling. As long as the bone structure is generally healthy, it will not cause instability.

Thank you, Why not Transforaminal approach or what is the cons if its done by Transforaminal corridor? and what is the angle of your endoscope?

The sequester is slightly migrated caudaly. A transforaminal approach has a risk of compressing the exiting nerve root whenever the trajectory of the endoscope is changed dramatically, or a substantial part of the caudal pedicle would have to be removed. Mobility towards caudal or cranial is larger with an interlaminar approach.

Do any professionals know if there are any special reason a 5mm AP fragment of disc would be be deliberately left behind after a L5/S1 discectomy? I have one of those cuddling up to the S1 nerve root. My GP said she does not think any surgeon would leave it there on purpose, i.e., it may not have been seen during surgery when they removed the rest of it. I did a release of information process to get a scan report that showed it. It might make a big difference, because lots of doctors did not seem to know about it, or about this particular scan having been done. They also removed a spur into that nerve and some ossified disc pressing my spinal cord, etc. The ossified disc did not show on previous MRIs. It was only discovered during surgery according to his notes. No wonder I was so sore for three years years between the first MRI and the surgery. It was a very hard time. It is not fused (the first surgeon I saw said he could not fuse due to multilevel damage. That checked out back in the days I thought fixing spines was like fixing engines etc). Surgery was 2014. Private hospital in NZ. A post surgery scan was done about ten days later when I had a spinal epidural haematoma. I do not recall seeing the fragment mentioned on that one. The MRI scan we discussed that shows the fragment was 2017. For some reason everybody missed this scan report. My GP had not even been sent it. She immediately began doing referral things on her computer saying she wants it looked into. She was quite animated about it and seemed a little angry at not having received it. This was two days ago. What has been the worst thing for me is how I had actually seen this scan report in 2017, while others who should have did not. I saw it saying how that nerve is impeded, but I did not understand about the disc fragment at the time. I also knew there were other possible pain causes, but not the details (marked foriminal stenosis at L5/S1 plus several others). So many people were telling me I was wrong and that this nerve compression did not exist. It was neuropathy according to them all. I had begun to wonder if I had dreamt seeing that report or something. When I was classified as CPS only after being referred yet again, this scan was not seen by pain specialists and their PTs. They were saying for years I just have a disc bulge. They did not even know I had a discectomy and the laminectomy etc. I tired of it and had to ask them to actually read my file while I made sure they did in person. However, they stuck to their guns saying I have no potential mechanical causes for pain. They lectured me over and over for years about phantom leg pain and neuropathy, like they thought I did not understand how ongoing pain arises and how it can be a factor in a case like mine, i.e., treating me like I am an idiot and a stubborn fool. Then they began head shrinking me trying to find events in my childhood they could blame for my not recovering, while feeding me anti-depressants for my mood and pregabalin for nerve pain. I did it all anyway and continue to do all the PT routines they gave me to do at home etc. Another wrote a report saying I was obsessed with having surgery (silly, because the only time I would want surgery on my spine is if it would help). Another made me flinch when he prodded me. He wrote that I am hyper-sensitive to pain. No other professional agrees, but the pain clinic reports carry a lot of weight here for some reason, perhaps because of the multi-discipline thing, but my more cynical side would have other theories. My dentist said just last week that I seem normal to pain, but if anything hypo-sensitive to pain. She would not put that in writing after she extracted a wisdom tooth and a molar. One of them even tried to make me feel guilty for not recovering by saying the country can not afford me not overcoming CPS. That is really not on. I do not resent the surgeon if he made an error, because it happens to the best of them. I do resent how the pain clinics treated me like I was wonky in the head. Another used euphemisms to accuse me of malingering in a report that was used to deny me accident compensation cover I had from the original work injury to that disc (another specialist pointed that out to me when my engineering union assigned a lawyer to it). Hopefully now this MRI showing the fragment has been found and it is referred to our NZ public health care system, I might get past this. I want my life back. After not being able to return to my work at sea, I began part time study at university. I got nearly half way through the degree with high GPA -- somehow while limping around and on tramadol, i.e., sheer determination. After surgery I tried two semesters in a row to get back to study. I could no longer do all the walking and sitting. I have been trying ever since for years, but I get worse instead of better. I can no longer walk more than a minute or two before my legs weaken and get numb with sciatica etc. I had to get a special disability card to get half price taxis, because I can not walk to the bus stop, or drive a car. We'll see what happens next. I also have a urologist just giving me two weeks to decide if I want a TURP surgery. Two marriages, but no kids. My dad would not be happy. TURP means probably not being able to spawn brats. He already half jests about me making his 1200 year old Italian family tree extinct, like most of the other branches are now. I was picture of health until this spine thing. Then Graves Disease, CVD, diabetes, BHP and chronic allergies etc. I am hoping that if I can even get a 50% reduction in pain, I might do better with all the other stuff. I am 51 now, so not ready to be thrown on the scrapheap of expended human labour. P.S. My wife was German. Her family used to send over these chocolate beans full of brandy. Nice. I can't have sugar or alcohol these days, but I sure enjoyed them then. The only German I remember really well is the rude thing to say a person's face looks like a butt with ears on it. I can still say that one quite well, but have not used it at the pain clinic. :)
