This is incredibly interesting to me. I was born without a functioning vestibular system, I have no inner ear balance fluid at all. I cannot ride a bike, ice skate, etc. I didn’t learn to walk until well over 2 years old. So if I close my eyes I fall over.

Gosh that’s rare - thank you for sharing, esp as you’ve brilliantly highlighted the point that we need two of your three balance systems available to keep balanced, and by closing your eyes, you are down to one

That sounds terrible. So, if you get on a motorcycle, you fall off the very next second? I suffer from deafferentation, which has caused me to lose a lot of sensations throughout my body, but it's not that bad. If I close my eyes, I can still move a bit without falling.

Thank you for an amazing video as always, Dr. James! I do have a question, and it is that can Rombergs test help in determination of whether the person is intoxicated with alcohol, or not?
@DrJamesGill
Жыл бұрын
It is often used less, but here is the limitation - Romberg’s simply identifies there is a problem OTHER than vision, it is UNABLE to identify whether vestibular or proprioceptive. cerebellar is a different issue again

Fantastic video! Proprioception is one of the most interesting things about the body I've found. Have you per chance come across the case study of The Disembodied Lady by Dr Oliver Sacks? I found it equal parts facinating and heartwrenching.

As someone with proprioception issues caused by sensory processing disorder this makes a lot of sense! When I’m overloaded often times my proprioception issues arise/get worse and it makes me more prone to tripping over my own feet or injuring myself somehow because my body doesn’t have enough spoons/energy so to speak to understand exactly where my limbs are in space and it sort of guesses but guesses wrong and it’s really annoying because I don’t always know when it will happen or when it will go away so I have to pay extra attention when it does to prevent injury because I’ve had some spectacular/hilarious moments as a result of it when I didn’t…

Could you do Unterberg's test to differentiate between vestibular and propioception after a positive Romberg's?
@DrJamesGill
Жыл бұрын
I’d need to double check with an ENT specialist, but I’d assume there would be a problem - if a patient is failing Romberg’s test when closing their eyes, they may not be safe to perform Unterberg’s I’m honestly not sure

I think it’s an interesting issue, as alcohol I can cause long term damage to the cerebellum, but TECHNICALLY Romberg’s is about dorsal columns and proprioception

Hi, I had this test today but I had to lift my leg slightly and close my eyes.I could not balance on both right and left foot.What would that indicate? I have tinnitus and was told no cure just yoga and meditation to help.Can you advise me .

Hello, Thank you for the video. I was wondering if you could advise me whether a positive rhomburgs with eyes open is a sign of cerebellar dysfunction? I know that it is not a cerebellar exam per se, but there seems to be some conflicting information as to what a +ve result with eyes open may point to. Any clarification would be much appreciated. Thanks

Not something that I intend to do often, HOWEVER, there are some videos I know i NEED to do for medical completion, but at the same time will not be popular with the algorithm. I have changed some of my working to be able to do more videos - i) i enjoy them ii) it's less stress than other things, iii) I've had excellent feedback from students saying how they have helped them. Unfortunately, less "popular videos" also mean less in all other metrics, so I need to find a way to balance that, and given that I use Blinkist, and highlight it to patients and students ANYWAY, seemed reasonably a win for many without significant harm. But to be very clear, it was not a decision taken lightly
