This is one of the best explanations of SI joint issues and their clinical workup. Trying to find a local specialist here in the Austin area who will work through these steps, but the two specialists I have seen so far must have slept through SI Joint Dysfunction 101. Will keep up until I get to a specialist who read the textbook.

My Orthopedic Dr recommended that I see a pain mgt.dr. for RFA. I've been getting RFA for one year now. When I get the Ablations, it takes 4 weeks for the nerves to die off. After that only half the pain is gone for 4 months. Then the pain comes back to the extreme. I get the RFA every 6 months on each side of the SI Joints. One on one side, then the next month the other side. I have a hard time sitting over 20 minutes and standing on concrete floors. The pain is worse on the right side with buttock pain and sciatica down the outside of that rt leg to the knee sometimes to the big toe. Issue is L4 & 5 & S1. I have an one inch difference in my legs due to surgery from one of the accidents. (Broken femur - pin was never put in place above knee, just in the hip and rod in the femur)Therefore, I have a shoe lift on the outside of the shoe, one inch. I've done Prolotherapy, PT, Chiropractor, meds. etc. Nothing last. This is all from 2 drunk drivers. I've been progressively getting worse over 27 years since the 1st car accident. Is there another procedure you'd recommend that would take away all the pain and last longer? What do you think about cutting the nerves, I believe it's called DVR? Tina Thank you

My MRI AND XRAYS, showed, sacroilitis!!!!tested negative for RA and Ankylosis Spondylitis came back negative as well! I’m in horrible pain, especially after sitting and any twisting! My inflammation markers are high, and X-rays show erosion as well😢

Very informative and helpful. I'm currently dealing with what we think is SI joint pain. Had 1 injection for the nerves and sciatica. Did not help, so next step next month is an injection of the SI joint. Hoping it works!

This is so thorough. I have bilateral sacroiliitis that manifested as lumbar pain and inability to go into flexion. L2 to T1 basically dont move. But nobody can figure out the relationship between the two

Good video. I think I am a similar case to the last guy. I had my hip replaced over 7 months ago. I now have pain shooting down my femor and lower back pain with pain along my glutes. An MRI showed bulging disc, so I had 2 epidurals that didn't work. I had an epidural in sacroiliac joint, and I was normal again for 3 weeks. I get another epidural this week in the sacroiliac joint. I hope it works. I have been diagnosed with sacroilliitis and piriformas. I use a walker or cane now. I was better off before hip replacement. The pain prevents me from walking. Hurts worse when I get up from sleeping. I was jogging down the road after my shot in the sacroiliac joint.

I have this problem and I had an injection and because I have fusion from T9 to L5 the doc had a hard time getting the angle past the hardware from the fusions. I was relieved from pain for 3months assuming it was the L5-S1 joint bulge. It came back and the next one lasted a week. Another injection a month later and it lasted 1 day. Neurosurgeon thinks my S1 is the cause and is willing to pin the SI joint after the fusion of S1 to the L5 fusion..ì do the exercises and it feels worse later the afternoon after PT. The next morning I have trouble getting to role over in bed and when I stand I feel better. My pain goes through my left crotch area down the front of my left leg and knee to the balls of my feet.Not down the back of my leg. Like it did for the L1L2L3L4L5 DID.after seeing this I will recommend they inject my SI joint and I am sire this will fix it. Thanks doc for your help and work 😊👍😉

This was so very informative. God bless you, Dr.Cooper. I've been diagnosed with severe sacraliliac dysfunctioning. I've had spine surgery several months ago which was successful. However the Sacroliliac pain began 3 months ago. Will have an injection soon. Feeling hopeful

I have learned a lot in this video. I am being treated for Sacroiliac Joint pain. Had my first injection 3 weeks ago and the pain was gone but returned about 7 days later. I am seeing the Dr tomorrow and he had told me that another injection may be necessary. So, I am on that path at the moment. However, I watched your video and was puzzled that you never mentioned a "fusion". My Dr had told me that should the injections fail to be successful, that a fusion would be considered. I was just curious as to why you never even mentioned that.

I think he did............he said it's not recommended anymore bc of micro fractures bc the joint has to slightly move.

@@KatherineCarothers he said old techniques caused microfractures but they now use triangular shaped anchors which allow small movements however there is always a risk that other parts of the body get affected downstream as you alter your bodies natural movement

Case 19-2023 NEJM. DESCRIBES AN 80 YEAR MAN WITH FOOT DROP AND A POSITIVE MRI SHOWING DISC IMPONGING ON L5. FINE, not really. On physical examination he has in addition to foot drop, asymmetrical muscle wasting. The final diagnosis was facioscapulohumeral muscular dystrophy. My question, is it possible that if the hip girdle muscles were involved, could this be a cause for the pain giving rise from unequal force on the sacroiloac joint? Bruce Roseman

Well, that’s not characteristic for any one particular source of back pain. It suggests more of a disc source but honestly it’s very non specific. I would definitely suggest you go and see a spine specialist and get a diagnosis and a treatment plan.

If I have SI joint pain, should I or should not do machine spinal decompression therapy? (the one where you are lying down on a bed bench). I went to a chiropractor that did spinal manipulation which made the pain worse, and they are now suggesting a round of spinal decompression therapy.

Great Video but no steroids for me. I have home health care for 40 hours. Can't lay on back, sides or stomach. I have C3-7 fused and bulging discs T1-S1. Bilateral ulnar nerve intrapment and use a walker. Only 58. Was in a very bad car accident almost 3 years ago. These doctors playing games. I don't want drugs. Just fix it.

So helpful! I've been going through PT for lumbar DDD. I've been doing exercises which relieve stiffness and pain very well in the general sense. Yet I continue to feel a localized pain in my left butt area which has plagued me for years and seems to match your description of sacroiliac pain . Thank you for clarifying this for those of us suffering with this frustrating symptom.

Thank you for such a thorough explanation. I have had one diagnostic injection prior to ablation and am discouraged because it did not relieve my pain.

If the diagnostic injection did not take away your pain for at least a couple of hours, and assuming the diagnostic injection was done under fluoroscopic guidance and done well, then you should have ruled out sacroiliac joint as the source of your pain and be looking for another source of pain. It doesn't make a ton of sense to me that you had an ablation after a failed diagnostic injection so it is very possible (likely?) that I'm missing something from the story. However, in general, if you have a diagnostic injection and there is no pain relief, then you cross that out as a possible source of pain and look elsewhere (e.g. disc, facet, piriformis, etc).

Thank you for the great video. I have been in a lot of pain for 6 weeks now. I have seen many doctors and physiotherapists. Their diagnoses have varied from the iliolumbar ligament to an L1 L2 disc compression to a hernia. No one has suggested the SI joint as the possible cause. My own investigation on the internet has led me to many videos, including yours, that best describes my symptoms. It is very disheartening because I must now go back to a doctor and convince them that it is SI pain and convince them to refer me to a specialist. I have had a CT scan, MRI and ultrasound. None of the imaging has picked up any problems at all. Anyway there’s nothing you can do for me but I appreciate the video.

All good BUT, no mention about the cause of the problem. WHY does the sacroiliac joint break down ??? Is it because of diet, does this happen because we move a certain way and the nerve becomes inflamed? WHY ? What’s up with the sacroiliac joint ? And don’t say age.

Typically the SI joint doesn’t move more than 4%. Sometimes the ligaments lose strength & the joint becomes hyper mobile & inflamed causing pain. There are other causes, but not diet. Usually structural.

The MRI showed spondylolisthesis grade 1, but I don't know when the listhesis occurred. At the same time, I have pain similar to piriformis syndrome and pain from the L5-S1. If it was caused by trauma, how can I determine the exact cause? The trauma is likely to affect both the disc and the sacroiliac joint simultaneously. Thank you."

I'm approaching my 50th and had an L5-S1 fusion at at 21. For the past 14 years I've been dealing with hip, groin, low back pain. 75% R/25% L. I went down so many roads and finally gave up. Not sure if it's psoas or now maybe SI joint. Going back at it to try and figure this out and hoping to become more active again. Thank you--great information!

What if you have both Sacroiliac pain and Disc and faucet pain? In my case I am 66 years old and suffered from A broken Tibia when I was 21 years old and Partial knee replacement when I was 45 years old? HELP
