Clinical trials show antidepressants work no better than placebos and pharmaceutical companies do several studies and cherry pick results to submit the favorable results to the FDA when it comes to getting these drugs approved. Antidepressants have not been studied for long term safety and clinical trials for most antidepressants are a few weeks or months at best. Many have the notion that anxiety or depression is like diabetes and like a good diabetic they need to take their antidepressants for life. Nothing could be further from the truth. Even mainstream psychiatry will admit the chemical imbalance myth is just that....a myth. Unlike a disease like diabetes, high cholesterol, etc, psychiatry diagnoses are only diagnosed by excluding other issues that could cause symptoms and the DSM. The DSM has come under fire recently as it's very subjective and at any given time in a person's lifetime, they could be labeled depressed or anxious or even schizophrenic (who hasn't had an auditory hallucination and thought someone called their name??) simply because they could check off a symptom in a manual. Psychiatry is the only branch of medicine where there are no tests to diagnose a disease but rather it's based on a list of emotions. Anxiety and depression have a root cause that needs to be addressed and it's not the mythical chemical imbalance. Antidepressants have serious side effects which are not rare. Side effects are vastly under reported but not rare. Antidepressants do not prevent suicides. In fact, study after study has shown they increase the risk of suicide in all ages. Even the FDA required antidepressants to have a black box warning (the strongest warning the FDA gives) because of the suicide risk. While the black box warning specifically mentions suicide risk in young adults, studies have shown antidepressants increase suicide risks in people of all ages. Furthermore, antidepressants have been linked to violence, worst long term outcomes, birth defects and learning disabilities when taken while pregnant, increased risk of death, diabetes, cancer, and so much more. Antidepressants have been shown to actually cause anxiety and depression and increase the risk of future depressive episodes. Some of the side effects can be permanent and continue long after the drug is stopped.

Dr. Mcmullen, is it true that higher doses of SSRI are used to treat anxiety? Such as 30-40mg or lexapro?

Hi Dr. McMullen, I was wondering if maybe you could do another video on SSRIs (maybe other antidepressants as well) in terms of which ones are relatively more "sedating" as opposed to "activating" (for example they say fluvoxamine and paroxetine are sedating, while fluoxetine and sertraline are activating). Also if you could relate this issue to bipolar disorder, if the sedating antidepressants are relatively safer to use than the activating ones, for people with bipolar (if used in addition to lithium, of course), i.e. more or less likely to cause 'manic switching'. Maybe more generally talk about who could benefit from a relatively more sedating vs. activating antidepressant. Thanks, Alex.

I have just starting taking an SSRI. I have had major depression for over 20 years and never treated it until now. Paroxetine at 10mg and im 2 weeks into the treatment and i have noticed an increase in my mood. I do get ringing in my ears that never goes away and an occasional insomnia but its overall an improvement from the depressed state prior. I use to self medicate with Marijuana and that was a lot more effective than Paroxetine and the feelings i get are similar but with MMJ they would peak and valley where as Paroxetine they are constant but at low levels of a similar feeling. MMJ also gave effects that would affect vision and appetite that were beneficial. focusing was more difficult or id be hyperfocused with MMJ but overall was a much better treatment from my experience so far. I am now educating myself on this treatment because i no longer want to live this way and I appreciate the information in helping me understand the medicine i am taking. Thank you. I am sure that on my next visit after 21 days of the medicine that the doctor will either change or increase the medication. I am also taking Hydroxyzine for my anxiety and that is pretty much worthless even when i double the dose, it has not helped my acute anxiety when i try to go into public or when anxiety occurs. I remember not feeling like this and over time the depression has got worse and i hope that this will help fix this condition. i first thought MMJ was causing my depression but after many times quitting for multiple months to over a year at different times i realized i was self-medicating and that MMJ was helping but i needed professional help. MMJ was not the issue but was helping me cope with the condition. If MMJ was legal and I could take it, I would in addition to the medication prescribed, because i can see how MMJ could greatly help people with depression but id prefer not to because of the smoking part affecting my lungs since i enjoy exercise when i can and the stigma in society that MMJ has been given and that its illegal. Thank you for helping me be more informed and hope my comment contributes to someone else in my shoes or helps in other ways. thank you.

Prozac is the only antidepressant that has worked for me and I have tried so many since 1987. However, the longest it worked for me was 2 1/2 days. The few times it did start to work was always with just a few days of starting it. My psychiatrist theorizes that when I start taking the Prozac it catches my brain off guard and starts to do it's "thing" right away but then my brain recognizes what's happening and wants to readjust itself to it's abnormal way of function so it shuts down the Prozac. No matter how long or how high a dose of Prozac I take it never starts to work again. We have tried up to 120mg and it still wouldn't start working again. One positive side effect of the Prozac that I do get is it makes my edema in my lower legs go away completely. My psychiatrists, podiatrist, cardiologist & nephrologist have never heard of this happening but that's what it does for me. Exercising and elevating my legs & taking diuretics does not reduce the swelling. I would suggest giving Prozac a shot if you have edema but it will cause sexual side effects.

I have the exact same problem! It works incredible for a week or two, and then poops out. A lot of people rate lexapro as being effective for many people and I may try that. I hope your mental and physical health is doing well.

Hi Doc,just want to pick your brain about sertraline if you dont mind.My Doctor has me on 100mg (50mg in the morning and 50mg in the evening). Is it ok to just take the 100mg once instead of 50mg x 2.I'm a bit worried as I noticed that the half life is 26hrs opposed to citalopram which is 36hrs which I still use to feel the positive effects of in the morning when i use to wake up .With sertraline when i wake up i dont feel as well as when i was on citalopram .

Don't you know that nutrition can address depression better then meds? If doctors were well trained in what actually affect the mood, the people who are said to be depressed might have a fighting chance. SSRIs should never be the first line of defense. Not even the second one. There are too many natural modalities that one should begin with. SSRIs cause an abnormal firing activity and lead to inflammation and even addictions. In fact, depression is often caused by low grade brain inflammation. This is why cannabis works so well (emphasis on CBD). We have an endocannabinoid system for a reason.. And there are no harmful side effects if used in the correct dose. Help depression while causing other symptos is NOT helping. Do you even check people's thyroid? Hormonal disturbances caused by hormonal disruptors should be investigated. Prescribing a SSRIs is no better then being a drug pusher. Seriously dude, a depressed mood is not caused by a lack of serotonin in the junction gap, it is caused by gut and immune system malfunctions.

@arron frederick Clearly you dont know what you are talking about. The problem with those chemicals is that they do so in a dangerous and unnatural manner. Diet will do the same thing but over a longer period and with no harmful side effects. Just in case you don't know, 80 to 90% of serotonin is made in the gut. What does that tell you? Must avoid sugar, processed and gluten, these all aggravate depression symptoms. Then up your fruits and veggies, good proteins and fiber. That is a good start. Then get some fresh air and do some physical activity. Walk or bike, whatever you like.
