I've had terrible piriformis pain in the past week due to hip impigment, couldn't even sleep at night from the discomfort. The posterior hip massage with the tennis ball was the first thing that brought me somw degree of relief. Thank you so much!!

I just started watching your videos and both of you have great clarity and experience. I appreciate that . I have tried a few of the exercises from your previous videos and I'm beginning to feel little bit of relief after so many years being out of whack. I have a ton of SI joint pain, very poor internal hip rotation, as well as lateral pelvic tilt. Your videos on these topics have really helped me understand in more detail what my osteopath/physiotherapist has been showing me.I have also had to learn how to walk properly and avoid duck feet. thank you for helping on all these topics.

For anyone that’s dealt with severe SI Joint Pain that comes and goes over a span of years or decades- check out the symptoms of ankylosing spondylitis to make sure that doesn’t fit you. I was just diagnosed after almost a decade of dealing with Pain, many incorrect diagnosis, physical therapy, etc. Nothing helped until I started the taking the correct medication. Changed my entire life.

OMG Coach E! I've just discovered your videos and I'm hooked!! I've done a trial psoas strengthen (couple of reps) and feel 99% better already. I've been active, tried to push on through the pain & stretch more! Then succumbed to practically giving up (also thanks to "the past few years"). I have had SI joint pain for over four years due to a car accident and nothing seemed to help. The pain was always there despite physio, staying active, stretching.etc. I tried this video abiut 3 months ago and noticed relief right away. I kept doing your exercises daily for several weeks and the pain has disappeared and has stayed away for over 2 months now. Thank you for this video.

How are you now a year on? I've damaged mine when I was 19 and been in constant pain since and I'm 33 now. 14 physios, 4 lots of spinal injections. Muscle tightness and wastage to legs etc. 😢 just wanna be pain free again

I experience severe sciatic pain that causes throbbing stabby pain mostly in my calf and thigh. It often wakes me from sleep. Ive been sitting cross legged and bending over with my head on the floor. It hurts at first as I believe my pirformis is tight. But after 10 minutes in that position... coming out and straightening my leg is very tender in my hips and often limp when I stand. The lacrosse ball is definitely a better exercise when I can get on the floor ❤ Im going to do this instead to help not to destroy my hip. Thank you subscribed

My SI feels stiff mostly, with little pain. I had a desk job ,but no longer. Got Some numbness so there may be nerve impingement, with sciatic. I performed deep tissue massage on glutes. In addition, I have pain in shoulder ball joint/ neck. Brachialgia. I read that issue affects spine. So it is related somehow. Thank you for this invaluable video.

I love your videos, how well you explain everything, and how genuine you are. You definitely stand out from the crowd. One question about the 1st exercise - the release with the ball. Should you feel better after doing this, or is it normal to have a kind of nervy - pain afterwards (it's not bad, just not the most comfortable. It's similar to how I felt before starting the exercise). Wondering if I'm bothering a nerve when I'm on the ball. I'm not being aggressive with the movement/ball and it doesn't hurt while I'm doing it, but a bit afterwards. Any advice? Thanks again!

I had an MRI and it shows thickening if the sciatic nerve near the pirformis and have pirformis pain and SI joint pain. My biggest complaint is that I have nerve pain in my foot. I have been dealing with this for over 7 years! I have been to PT's for years and they seem to know very little about the SI joint. Is it ok to start doing these exercises with inflammation on the sciatic nerve ? What would you start with? Your videos seem to make such good sense.

I just had an MRI today. I think I iterated my hips, butt and legs by thinking my problem was my hip and doing stretches and exercises for hips. My Dr. thinks it’s a disk issue, now I’m scared to do anything until I find out what I’m dealing with. But OMG does it hurt! 😩

was using a chair to get the full range of motion of the final exercises and had a sharp pain like a muscle sliping over the front of the hip. Do not think I tore anything but it hurt and I stopped on that side for the night.

I have recently been diagnosed with low bone mass / osteopenia 10-Year Fracture Risk - Moderate (10-20%) in my left hip. I am already do your piriformis and hip exercises. Would you suggest any other exercises to help in this matter

You need to do strength training. Start with bodyweight squats, lunges, knee pushups, rows, etc... and gradually add load over time. Basic strength training done with good technique and slow but consistent progression.

I've just discovered this video and am certain I've found something very useful. However when I tried to do exercise 4 ,The Hinged Knee Flex, I experienced cracking and instability in my knees in the slightly bent position. Is it advisable to carry on or is this a signal to stop?

Thank you!!! I’ve been told to wear an SI belt for a month and the slowly wean off. Can I do these exercises in the morning before putting on the belt for the day or should I wait to start these exercises until I’m ready to wean off the belt?

You are so welcome and this is a great question. The belt is passive and will not address the root cause of the issue. If anything, it will actually weaken the region further. Is there something I can for peroneal nerve entrapment? It’s causing weakness under big toe mound. My SI joint and piriformis are aggravated so thanks so much for this video but not sure if peroneal causing this pain or vice versus. It’s tight from outside of knee where nerve attaches to femur and travels down to to foot.

For sure - just don’t get distracted behind the wheel! 😉 Other big tip is to keep good pelvic/lumbar posture, perhaps with a lumbar support.

@PrecisionMovement Thanks for another high quality video. Can you do a video on shin splints? I know the topic is complicated but I feel like it really has something to do with a tight lower back or upper backside hip area like with the piriformis/si joint. I've found when i have long periods months of not working and I'm not moving as much and/or I'm sitting more then the shin splits return. As opposed to the months where I am working and constantly moving and not laying down and not sitting the shin splits are much more reduced if not there. It's the same thing with calf tightness. I'm sure the shin splits are in part caused by the calf tightness but it feels like something higher up the leg chain is causing the calf tightness. I'd love to hear your take on it.

So iv been working on this for months, every morning it take me a couple hours to be able to walk. Iv had 3 back surgery with an 8" incision l4 l5 lamanectomy then l4 l5 fusion then a combination 3 weeks after a "subdermial hematoma" but everyone that saw it said it was infected but the hospital says no. Struggling for my life back, probably going to see how I can get involved with you guys and see what you can help. Im in ND USA where are you located?

On the split squat, after the (sore leg) right knee is in front, when I try to stand from the squat (left knee to floor) what can I do to stop an impingement in that right inner hip socket that prevents me from standing? It feels like it’s literally hung on something and is very painful if I try to stand all the way up. The only way to unhook it is to bend forward and bring the left leg under me to stabilize and then push my right foot and leg backward to unhook it before it can stand straight. Are there stretches to get whatever ligament is taught to relax?

I have SI joint blockages on the right side and pain in the outer part of my hip (probably the glute medius). Apart from using the ball on my glute max, I also use it on the tender spot on my right side, Then I also strengthen specificially the glute med, similar as in the video on gluteal tendiopathy, since this is also an issue with me. I find that combining SI exercises with abduction exercises and expecially loosening the fascia with the ball help me on with my problems. Thank you for the exercises! But my question is, is the glue med pain and stiffness (and therefore the tendency to get gluteal tendiopathy) also likely related to the SI joint? I am not particularly weak in the hip area, but when the glute medius is supertight, it doesn't seem to work properly. Can this contribute to trochantiriac pain?

Thanks for following along. What you are experiencing is not uncommon and hopefully, these exercises help. The SI Joint cannot move much but if it cannot even achieve that small amount of movement then the ramifications can be large. Try them out 2 - 3 times per week for 4 weeks and get back to us with a progress report :) - Coach Joshua, Team PM

So here I am with the progress report (a bit late, I know ;)) These exercise give me an immediate relief and improve function, mobility and pain level. So thanks again for that. However I must admit, that I have not been really consistent with all these exercises. The problems come back, when I neglect them for a couple of days. And I have also found that some activities (biking and heavy leg work) often make the situation worse again. So my question is, how can I integrate them in the long term and how do they work together with regular strength training and other activities? Especially squats and leg presses seem to make it worse - strict glute, hamstring and adductor/abductor exercises tend to have a positive effect, however. I just don't want to lose strength in the quad exercises, either..Is it better to focus on hip strength for a while and progressively build strength in these aread and do only a little or no quad training? Or do the SI/glute/psoas etc. exercises on off-days?

So my butt tightness came from using a tall Home Depot orange stair ladder. I brought down several cases of spray paint over a stretch of two days. Now my left Butt cheek is super tight. Actually, my whole left leg is super tight. It feels like the perfect storm.

@@PrecisionMovementCoach Just finished my workout. Definitely feels like this my left cheek tightness extends from my Achilles to the top inner cheek, on the way to the lumbar. Straightening my left leg during an exercise is a challenge and my hip flexor on my left is either weak or tight. The workout was great and left me feeling like this left-to-right imbalance is going to be a real problem. I have co-workers and associates that are going through the same thing and can’t afford to deal with the issue medically. Thank you for the feedback.

@@PrecisionMovementCoach thank you for the support. Using a medium-firm foam roller, I just finished working over my glute tissues from the inner cheek to the hamstrings (both sides). It loosened my left side all the way up , but I will keep up the routine for the next week.

It could be a combination of issues and it's hard to say. Start by analyzing when it happens and if it's something that you can correct by modifying the technique or your alignment. Thanks for the video, great content. What do you think about my situation? I was holding a large box (light, but enough to block most of my vision), stepped on someone's shoe left on steep stairs. since I was holing the box, I just left kept my leg straight as the shoe rolled off the step and then my foot caught it again, forcing me down another step where I jarred that hole side pretty hard but stayed upright. This was probably around October of 2021, the end result was that I injured my tibialis anterior (presumably, it hurt from the top of my foot up to my knee) which healed after about 8 weeks, and what I also assume is my SI joint has been in mild pain since. SI pain fluctuates a bit but never hurt as much as the muscle/tendon at it's worst. A few weeks back I used a tennis ball on similar to the video but more on my back and that helped for a couple weeks it seemed, but the achiness returned. I have a job that requires me to sit for long hours though, so do you think this matches up with these SI exercises?

I am in agony ! My physio was so unhelpful that I changed her recommended excercices and do water jogging instead . I practice qi qong , Tai chi for 3 hours a week and swim daily . Nothing help

Just an update, been doing this routine for almost two months now, and the results are excellent. Still have a little pain in my left buttock region whenever I wake up in the morning if I slept a little longer than 7-8hrs. But really this routine helped me with my lower back pain and now I can do isolation workouts 5 times a week using bands. Thank you so much Precision Movement!
