In case two is it predominately lamellar bone as opposed to woven? I;m pretty sure we are seeing foamy macrophages in case 2 as well.

That is correct - it is lamellar (or mature) bone in this case. There are a few scattered macrophages but the predominant histologic feature is the acute inflammation, which trumps the foamy macrophages (a feature we associate more commonly with chronic inflammation).
