Thanks for the tutorial apart from lack of PPE (mask), no identification with 2 items, tape left on non sanitized surface, no inversion, that tutorial is teaching us many good steps and techniques in a very calming tone.

lots going on. personally i wear gloves for the entire procedure to reduce contamination risk. he left the tourniquet on too long, didn’t immediately invert tubes (that’s what i personally do). you should NEVER put your bandage onto a potentially contaminated surface like that before drawing. he also did not identify the patient, ask about allergies, fasting, fainting, etc. i also noticed he slipped out of the vein potentially while a tube was engaged but it was hard to see? also not wearing proper ppe. typically at the very least a MASK. but most do a gown as well.

Apart from allergy to tape adhesive and latex(which NO ONE should be using) there is nothing a phlebotomist needs to know about allergies. Should be asking about past difficulties drawing their blood.

@@hbic3 yes there is. depending on the tubes if edta is accidentally touched to the skin or chlorprep or iodine are used as cleansers it’s important to ask.

He didn’t verify the patient’s identity. Didn’t ask the pt about history of fainting or lightheadedness. Should’ve had gloves on when touching the gauze and assembling needle and hub. He said sharps go in the biohazard trash and that is incorrect, they only go in the sharps container!! He touched the alcohol pad with his bare hands. He should’ve removed the tourniquet when the final tube was half filled and he left it on longer than 60secs. He didn’t invert the tube within 30secs. He didn’t have the patient apply pressure long enough. He didn’t have the pt verify the info on the labels were correct (name, DOB). Lastly, where is the requisition form?!

Didn't verify name, DOB, ask about allery to latex or if he had been fasting. Opened Needle facing towards patient. Needle angle was a bit high. Did not invert properly either. Thanks for the video!

You really only need to ask about fasting if the ordered tests require fasting. By the time you get to draw prep, name, dob,etc has already been established. His angle wasn't bad, some have to be at this angle. HOWEVER, he didn't advance the needle far enough, that would have prevented the "overflow" because he really is "just the tip" unnecessarily. SOMETIMES you have to pull back to just the bevel, but that is so precarious, if the patient moves even the slightest, that needle is OUT!

He didn’t identify name and dob, he didn’t check for a wristband to make sure the requisition match the band to right person. He didn’task if the patient was allergic to latex or if he fasted if noted on requisition. He should have washed his hands and then put on gloves. He poked his nail on the vein site. He went around in circles with alcohol. He had the tourniquet on too long. After removing needle and placing gauge he didn’t tell the patient to put pressure on the site. He didn’t check the site for hemostasis before placing tape. He shook the bottles fast even though he told us not to. He didn’t place the labels on immediately after drawing the tubes.

ok. I teach this. 32 years now. Never sanitized nhis hands. Spoke about it BUT did NOT do it. Handled ALL equipment without gloves YOU NEVER pre-cut tape and hang on table (let all of those aerobes and pathogens just cling to the tape !!) When you anchor the arm you NEVER anchor on side of venipuncture site !!! anchor low on arm....believe it or not, your veins do reach down there ! You NEVER mark a vein. That is old school-lazy..and NOT protocol, anywhere. Not with a needle cap, NOT with your finger nail (gross) not with anything. OMG..so MANY things to list. This is awful Big NO NO also, when he switched tube (you saw that spurt of blood) that means the needle LEFT THE VEIN...watch carefully..he pushes it back in. So air got in that tube when he adjusted. Ridiculous. I am just surprised he didn't lean over and blow on the arm to make the alcohol dry quicker...I was waiting for it. So bad... And ALWAYS make your patient extend their arm straight (even if they must make a fist and prop under their elbow...The straighter that arm, the straighter the vein...the straighter the vein....the LESS PAIN the patient feels...

@bo bo It’s probably late by now, but that was probably a Hematoma. That happens when the needle is partially in the vein and partially outside of it, the blood will start to pool right under the skin and cause some darker than normal bruising and maybe a slight bulge in the skin as well. This usually happens when the veins are really deep and the needle isn’t in far enough. This can also happen after the blood is drawn, if you bend your arm before the bleeding stops. The phlebotomist should tell you not to bend your arm too much for at least 10 minutes or so. If a Hematoma does occur It’s not harmful, just put some ice and massage like you did and it will eventually go away.

I precut tape and put it on the back of my gloved hand. Sometimes you don't have two hands to get that tape going. Your patient can't always hold pressure for you. But, I guess you'd have to have actually drawn blood at hospital or nursing home bedside to know that.

You are supposed to invert your tubes immediately after removing them from the vacutainer, do not wait. Also those were not complete inversions

Let the tubes lay down for awhile before inverting, and also the red one did not need to be inverted.

Identify the patient, blue tube invert 4 times, red tube need to rest for 30 min or centrifuge, he move the needle that's way pt bleeds a little bit, check the site before putting tape to make sure is no more bleeding,

Did not verify patients name or date of birth or realise tourniquet after 60 seconds and did not ask the patient if the tourniquet was comfortable not to tight I would have my gloves already on pulled the cap of tord the patient and did not sanitize his hands and you should wash your hands thoroughly than sanitize then put on your gloves

FACTS. Sometimes you HAVE to PULL BACK to just the bevel, but those are specific instances. He surely didn't advance it far enough. Patient arm wasn't straight enough, but sometimes they just won't, or CAN'T make it straighter.

Don't really have to. This is BEST PRACTICE, but depending on the difficulty of the draw and how tight you've tied the tournie(this guy BARELY ties, it might as well not be there), it CAN stay on longer, as long as it's not TOO tight and you sense that taking it off will cause flow to stop(yes, that happens).

There really weren't that many. A patient who has had their blood drawn many times but had never been trained in phlebotomy does NOT know how to do the job better than the trained Phleb. Keep that in mind.
