First and foremost, picking up paper towels from the counter top is no no. Why would you ever consider this, the wall dispenser obviously has paper towels in it. Lifting a paper towel off of a countertop defeats the whole purpose of maintaining "clean " hands. Never in 25 years have I nor have I seen any physician I have worked for remove paper towels, place them on the counter then use them. Think about this, you are removing those paper towels with dirty hands and placing them on a potentially dirty countertop, omg. Second, you can peel back the opening of the sterile drape, preferably on a flat pre-cleaned surface, then don sterile gloves, remove the sterile field drape and place it onto the mayo stand. I know a lot of people do not do this as you have to remove and don an additional pair to set up, however, it does decrease any contamination even after washing your hands. Third, yes, I agree, way too many instruments in that autoclave packet but I think it appears this way because they are in a large instrument packet, allowing for them to shift and move around freely. If autoclaved in a smaller size instrument packet(s) they do not shift around. I don't know about you guys, but I was totally holding my breath when she opened that packet, any second I kept waiting for one of those instruments to catch another and flip out and over the field onto the floor ugh. Next, she did not properly demonstrate how to fold the cuffs down on the sterile gloves, it was just magically down in the next clip when she prepared to draw up the local. Everyone is taught and knows that the only way to fold the cuff of a sterile glove completely down is by using your other hand with the sterile glove on it. Hence this is why they are cuffed when you open them, allowing you to touch the folded cuff to put it on, yes you can adjust the outside cuff down somewhat but as we all know, they don't always unfold perfectly/comfortably on their own, for example when she put on the second glove, she uses her sterile gloved hand to adjust the fit and cuff. Finally, I suppose if your physician likes to have lidocaine drawn up ahead of time you can pull this off. However, in my experience, the phycisian, once he/she has donned sterile gloves, will request that the nurse/assistant hold the vial up for him/her and they withdraw the solution. Oh yeah, yes, even though the needle was technically not used, you should always demonstrate one hand technique when recapping. If you did by chance prick yourself recapping as she did well then the entire field is contaminated and you have to start all over. Also, you will find that oftentimes syringes that come with the needle attached are not twisted completely tight. Meaning they are not snug, resulting in you having to twist them snug to the luer lock tip. If you don't then your solution will leak and drip and sometimes it will not withdraw at all due to the break in vacuum connection. I am no expert by all means, just pointing out the variation in technique and ultimately how we were instructed.

Wasn't she not supposed to leave the room or turn around after placing the sterile objects on the field? She left to wash her hands. I'm a nurse student so I am pretty clueless. Also, she didn't wipe the vial before withdrawing the solution.

She didn't need to wipe the vial because she was the one who opened it..if it was used already that's when you would clean the top. That's what we were taught.

I'm a nursing student & I've noticed that her hand washing technique is wrong too. As others have mentioned, the paper towels on the table - that is a no-no. Also now with COVID-19, health care professionals need to be even more diligent in hand washing & use the proper technique.

artificial nails, no mask, no haircover! At least mention at beginning of video that you are not wearing the mask and haircover so we can see and hear you. Sorry you also need to clean vial top with alcohol wipe - as that lid cap does not guarantee asepsis. And no reminder that the outer 1 inch is considered a no-o zone.

Is it just me but she has contaminated the field cos all the equipment she was touching was not sterile ( outerlayer) and then she touched inside and putting everything on the ‘sterile’ field? She should have prepared the field as she started cleaned the surfaces open the packaging without touching the inside on the surface, then wash the hands again, open the pack to cover the field, open the rest of the necessary tools without touching the inside sterile material, then wash her hands again, place sterile gloves then arrange her tray. This way she keeps area actually sterile

You're setting up a sterile field without a face mask. Not so sterile any longer. Also, grabbing paper towels from the counter isn't good hand hygiene.

Problematic. Too many items in the peel pack. Opened instruments toward herself at first. Hair is loose which may/may not be a big deal. Recapped needle. No mask. Grabbed inside of drape instead of underside although she was in the border. Never draw up meds in your hand. Always puncture a vial downward and not in the hand. You could poke yourself. Didn't wait the dry time for the disinfectant. Hand washing demo was not 15 seconds min.
