You should def do a vid breakdown on EM job market. Specifically the jobs report. Especially how the attrition rate used in the study was more than half what post Covid attrition has been and that the study actually produced a range from -2k to + 12k jobs so that’s where the average of 7K surplus came from. Also post Covid job acuity and demand is very high currently

Unfortunately, the NP/PA problem will be there for many specialties. My sibling and I are both physicians, but he is also an administrator. He says administration is definitely trying to cost cut by replacing MDs with mid levels. In some states, NPs can also practice independently as primary care providers.

Dr. Jubbal look at the amount of US MDs that matched into psychiatry this year and how few FMGs applied to the field. It's like foreign applicants have effectively given up now that the secret is out. I feel like nobody is looking at this and it's kind of massive?

The only reason compensation dropped was less hours when less patients were coming in as everyone was indoors and not coming to the hospital unless they needed to. The hours have returned to normal and the job propects from EM colleagues is good.

I work as an ED scribe, and yeah, all the docs have been talking about the drop in EM matches. They all agree that EM kind of shot themselves in the foot in recent years. With the projections of too many docs, people complaining about burnout, and being on the frontlines of a pandemic. Plus, the tedious charting to cover your ass because of the medico-legal stuff breathing down your neck (EM is in the top five specialties most likely to be sued). Can you blame people for being turned off EM? (Also, there were a bunch of documentation changes that happened at the beginning of the year. Documenting in the ED is already pretty complicated, but throw in these changes, which the doctors are STILL trying to figure out!) At the same time, this is kinda good news for me. I mean, EM is in my personal top five specialties I'm considering (neck and neck with PM&R). I have the most experience in that speciality, I know how the ER works and how the docs think and work, it's a shorter residency, and I can still do a fellowship in Sports Medicine (which is part of the ultimate dream of being a show doctor for Broadway shows). Plus, I'm used to the crazy schedule anyways.

As an fmg who successfully cleared all the steps (1 fail on step 1 in full honesty)… who was just never able to match… I’m bitter about the whole process. One exam wrecked my entire career, nobody willing to give me a chance based on 1 failure. It’s my one major regret in life… not getting into American med program and not wanting to settle for pharmacy school. Took a gamble and went abroad, studied hard, just not smart enough I guess

Don’t give up. Keep trying especially in primary care like internal medicine or family medicine. Once you’re in there, try switching. Or if you’re still young below 45, then do primary care residency, graduate and apply to your desired specialty. 6 people at U of Minnesota Medical Center did exactly that and they were FMGs with step 1 fail x 1. @@jamesamoh7340 Went through the match process 4 times… like 10 years ago. I quit and ended up getting a decent job in healthcare IT. Not my dream but more than pays the bills and gives me a great work life balance so at the end of day… whatever

FYI, I got 240+ step scores. 7 pubs, was psych tech and psych bachelors. Honored with good letters. 10 interviews and I didn’t match. Do an away rotation.

@@BigChungusthe3rd Nope. From top 40 US allopathic school. Had 19/130 students available for SOAP. Nobody known what the hell is happening out there since COVID. The entire match process is kind of a mess right now.

@@tgreetsy1201 im only an MS2. But it could also be that maybe you didnt interview as well as you thought, maybe your personal statement wasnt as convincing as others. I know you mentioned you did a lot of psych related things (pubs, volunteering, work etc) but just doing all those things doesnt mean youre a shoe-in for a specialty. Im not trying to call you out or talk down on you but just trying to add context to anyone reading this comment thread. Yes youre a superstar but theres a lot of miscellaneous things that couldve pointed to why you didnt match psych. Wish you all the best

Psychiatry is definitely more competitive but far from ROAD specialties. Psychiatry is not a back up specialty anymore

No metric is perfect but I’ve got to believe average Step 2 score is highly indicative of competitiveness of a specialty.

IMG here. What might be the reason why IMGs choose psychiatry less and less over the years? The data suggests an increased demand among MDs and DOs but IMGs applying for psych has decreased by almost 30% over the years.

Not an expert but i would guess that IMGs are more likely to apply to specialties that have a lower ratio of applicants to positions. So with the number of psychiatry applicants increasing, IMGs are less likely to apply because their chances are smaller

From what I hear, the jobs report put out for EM was wrong. There will be plenty of jobs mostly in suburban/rural areas but there will be jobs. EM is still a great specialty with a great work life balance both in residency(residency sucks for a lot of specialties, but not for EM) and as an attending. I can't find another job where I can make $250-300K/year working 12 to 15 shifts a month. Still a great bang for the buck.

So what’s going to happen when no one apples to EM residency in the next four years… does that mean there will be an actual under supply of EM docs, which the market will subsequently favor increasing salary, hours, and other benefits? Aren’t a lot of these specialties volatile?

I’m really interested in EM. I think it’s the specialty for me personally. I think the pandemic really messed up the landscape of medicine in general. It’s sad to see so many infilled spots so hopefully things turn around by the time I apply for residency

Many times these studies don’t do a good job of predicting the market. Many studies 10 years ago said radiology would have a horrible job market at this time, when in fact radiology has the best job market right now. If you like EM I would say go for it, but keep an open mind

I’d tell you to go for it because as less people keep going into EM, and more EM physicians retire or burn out, that 2030 prediction may not come to pass. The research paper from ACEP has since been heavily criticized because it makes some assumptions that simply don’t adjust for significant changes in the number of residents graduating.

Radonc and plastic surgery are overrated on his spreadsheet. I think the weight system was quite subjective.

Nah we made it rationally and let the cards land where they did. In V1 everything was weighed evenly. In the updated version we adjusted the weighting. But if you look at equal versus weighted systems, very few specialties actually changed ranking. I believe spots 3-4 switches places and a couple more towards the bottom.

@@bluethunder9102 that may be your preference! Many docs don’t love charting and outpatient clinics. With the rise of more advanced imaging techniques and procedures, many rads can also do some neat stuff.

It is all preference. I thought radiology would be boring but after going through my med school rotations, I realized radiology is actually really interesting, especially when you know what you are doing. Most other specialties require you to spend hours writing notes and dealing with social work. Also, all diagnostic radiology sub specialties have procedures as well.

@@bluethunder9102 I personally hated psychiatry, especially outpatient. Many times it’s just changing a patient’s antidepressant to another if they are still depressed. However, I understand how it can be appealing to others
