So the hands need to be over the top when entering?

Not quite. It is more the angle of the arm to you shoulder and their palm height. If on entering your hand/glove is below their hand or glove it is easy for them to tap and elbow. If your entering the clinch by reaching into the inside it is generaly preferable that only one hand is reaching and one hand in a tight guard. When reaching with one arm make sure the angle of your arm adds strength to the position and that your shoulder on the same side is tight to your cheek. This will make it hard to push down and also add some protection if the elbow does get through.
