On the 22nd of June 1893, a powerful Ironclad battleship of the royal navy was lost. But it wasn't lost to gunfire or enemy torpedoes, but due to a bad order and a unwillingness to question such orders. In one of the worst peacetime tragedies of the royal navy, glaring issues in the chain of command and how orders were received were revealed.
