As a president of a residents union here in India I approve this message! We formed a union, negotiated hard and got a 25% stipend increase for all residents. The last raise was 8 years ago!

the phrase "form a union" is weird to me. in germany it would be "joined a union" as there are already unions a lot of fields - and unions are nationwide mostly, not confined to a single workplace. the concept of a single workplace union feels extremly alien to me mh. BUT nontheless, good for you. i'm a firm believer that unions improve the work place performance overall - not just for the employes but for the companys as well.

Story time: A few years back all the residents and fellows refused to round on or write orders for a certain spine surgeon at our hospital ( the guy was a total asshole). First he tried to bully, threaten, and shout his way out of the situation. However, once he could see that the housestaff wasn’t going to cave in, it was amazing how quickly he unlearned 35 years of malignant behavior and changed the way he was treating his colleagues and trainees.

Unfortunately spinal surgeons really do seem to be set up to be the most arrogant. It makes sense with neurosurgery being one of the most inaccessible fields of medicine. Even if you can get through all of the years leading to actually becoming a physician, you get to deal with a terrible work/life balance. All of the neurosurgeons I work with seem stressed tf out all the time. But I honestly have very few complaints about them. There's plenty who are remarkably nice even when it's obvious they haven't stopped moving in 38 hours. 😂 I'm sorry that you had to deal with this asshole, though. Definitely not making excuses for someone like that

A word of caution to medical students/residents: the abuse doesn't end when residency is completed. Life as a practicing physician can still be GREAT, but employers, from private practices hospitals and/or health care systems will still do everything in their power to take advantage of you. To paraphrase Terry Wall, MD, JD who has lectured to residents, myself included, "NO ONE wants to make new physician rich." His advice: Hire a competent attorney and have them review any employment agreement BEFORE you sign.

I agree with this sentiment, and think we could extrapolate a bit here: No one in any commodified field wants to make new members of said field wealthy enough to have control over their career prospects. Residents in my country just returned from a five day warming strike. It was hell because we had emergency surgeries and had to prepare the patients and do all the heavy lifting ourselves. After the first day, I had to inform ER to stop admitting patients for my unit. Residents are very important to the system.

As a consequence of that, the implementation of changes has to go through the political system, which will have an apparatus with lots of bloat and inertia. Corruption becomes very easy in such a structure aswell. If all you want to do is to abolish wage slavery and make health care accessible to everyone, then regular 'social democracy' is enough. If, on top of that, you want all means of production to be democratically organized and owned in equal parts by the people who work there, socialism it is.

from personal experiences of the uk junior doc strikes, the wards go to shit but A&E is actually fine because the consultants that are left have been there too long to give a shit about potentially being taken to court over someones 4 week cough and just keep telling people to go home and contact their GP lmao. Its very bizarre

Fair pay and benefits are great but it's also important to push for safe working hours. Junior doctors in my country had a successful strike a while ago that was mostly around that. No amount of pay will make 80 hour work weeks less dangerous for both patients and doctors, so it's gotta be both!

As an American, I'd GLADLY pay what I already am, if the $ cut out all the middleman bullshit and inflated CEO pay, and went toward increased medical staffing, and fair and safe labor practices - including 40 hour work weeks, shift maximums of 10 hours, fair wages and benefits for medical and support staff including residents...bc they're people too.., and safe staffing ratios. Unionizing means shifting a little bit of power away from the employer, hopefully enough power to get a fair employment deal. An HR director once told me that a company has to work awfully hard at it to get a union in it. Hospitals seem to work very hard indeed at abusing their power over residents, so good on them, those who choose to unionize!

Coming from the UK, we healthcare providers could join the union, CoHSE - the Confederation of Healthcare Service Employees. It encompassed all in the healthcare environment. It gave us bargaining power for doctors down to the orderlies. Great power in numbers of like minded people. All for one and one for all. I came to the US and was appalled at the lack of power anyone in the healthcare environment had. I left and went into law representing MDs. I’m experiencing lacrimal stenosis and am being treated by my great ophthalmologist. I told him about you and now he’s a addict. Thank you for you content, especially the challenges that rural MDs face. I love Texaco Mike and his MRI.

My dad knew four people who crashed their cars falling asleep at the wheel during the course of his residency, since everyone was working 100+hours a week for 22k a year He also knew a guy who was outed as having falsified his med school records once the Soviet Union collapsed, and the hospital could actually check them. Also, their chief resident for internal med mever finished his last year of med school because he bribed an administrator in the Dominican Republic the early 90s were nuts

I had the fortune to be part of an Interns and Residents union during my training. Thank goodness. We still got abused, don’t get me wrong, but not as badly as many of my friends who went to different programs. It was a good union, so we got better pay and more vacation than average, and someone to turn to when the administration tried playing dirty tricks. It has made me very pro-union (with the understanding that some unions are better than others).

Having worked in a family medicine practice where the doctors, NPs and PAs led the union effort, and were successful, I applaud the residents. Currently working at a facility where hospital nurses are union, clinical are not (I am clinical). The “do more with less staffing” is a real contributor to provider and support staff burnout. Thank you Dr. G for bringing these issues to light.

Being a junior resident who is currently posted in a periphery hospital...this strikes so much hard that literally tears coming to my eyes...but here no one to protest..no one cares..our principal has given us multiple threats to deduct our salaries which is not even a salary but a minimal stipend...she even refused to give us hostels...have to travel 100kms up and down every week to stay 3 days in a duty room...not a adjustment disorder but every f time i regret taking mbbs as my career...

I’m in clerkship now, And we don’t even get paid. I had an OB attending ask me once why she had no externes and she was falling behind in her notes. She was used to having two patients scheduled at the same time and having us write the whole note for her so she could be more efficient. Just goes to show the students do contribute 😅

All my life all ive heard is how bad unions are.. i never had a union job. Until last year. I got into the steel union of ohio, and it was the best job i ever had. Due to personal reasons i had to find a new job that didnt work midnights (i had to work a rotating swing shift there) but if those issues never happened id probably be there the next 30+ years. The benefits were amazing, a pension fund and up to 6% match on a 401k, health insurance, dental insurance, vision insurance, they even had gym membership, and rec center discounts for the area just for having the job. They even had a pet insurance program! (This wasnt included in our benefits, but we got a massive discount which was like 15 bucks a month with free check ups and everything) then to go along with ALL of those benefits was the guaranteed pay increases. They have a set 10% raise every year to combat inflation, and then ontop of that there is a performance raise as well as a performance quarterly bonus based on how we do overall as a plant. We got paid double time for every hour after 8 hours worked (in our contract we are only to work 8 hour shifts but if understaffed we will make up those hours and get double pay for it) and triple pay for any hour after 13 hours worked. There was a bunch of other little bonuses as well, like we were allowed on our phones if we our work was done and waiting on a roll to finish or couches and chairs to sit at while waiting. Jist of it... unions are very important. This isnt even mentioning the MINIMUM 80 hours of vacation time per year and the fact you earned your vacation time back based on the hours you worked that week.

I worked for grief manufacturing paper mill in ohio by the way. Theyre apart of the Steel union of ohio and yoy can find most of the information I provided online in their employee handbook! Some of my information may be slightly off as the union was having meetings just prior to me leaving the company to make some changes to the benefits but I don't think they were major changes from what I remember.

Residents get unionized? Right on! How about all the other physicians? Health care worker unions can stand up up to the for profit health care industry to improve their own lives and the lives of their patients.

I remember when the 80 hour max duty hours kicked in. It was crazy. The hospital lost about 40 hrs a week per resident for over 100 residents. Keep fighting.

As someone considering going to med school but hesitating due to the widespread abuse/underpay/overwork, I only hope unionization and reform of the academic medical system keeps on rolling, for those who are in it and those who are yet still to come

Keep on hesitating, for your life balance sake, it's the same all around the globe, sadly... the abuse continues after residency, but by then, most people have spouses/babies to support, and they know it....

I'm in the same position. The suicides, the working conditions, the pay, mental and physical health down the drain. I read story after story and it just brings me down. I expect the process of becoming a MD a to be hard one, not a life threatening one. I pray that all current medical students,residents, and attendings turn this around,before the healthcare system finds itself in a place it can't bounce back from.

@@carloswilliams5670 noooooo, hell no, wish there was someway I could show little Beckah her nasty future.... I even did two specialties: Internal medicine and Nephrology, I was certain at first I did study my calling so to speak, it was truly fulfilling but at least, in my 3rd world country, that being México, practicing in a government hospital is sooo depressing because of overwork/underpay, but you have at least free medical attention and other stuff, and can't be called past your checkout....and to skip that, the other option is practicing in private hospitals, meaning one must be on entire availability on weekends, holidays and you get the picture, you pay your own medical attention, I'm not sure which one is worse....

So, at minimum Neurosurgery worked 20 hours a day, assuming he didn't get any days off. Or he had one day off and worked pretty much 24 hours a day for the rest of the week.

When i was a resident almost 20 yrs ago there was a federal lawsuit about our pay that apparently we didnt win 😐. Someone calculated what we were paid an hour based on the hours we worked a week and it was less than $3 an hour! Mind you this was when the 30 shit rule went into effect (so we weren’t supposed to work more than 30 hrs straight 🙄. Neurosurgery was the worst at violating the work hours (i.e. making residents lie about their hours). As physicians are being phased out the US is headed for a rude awakening 🥺

The horror is worse than I thought. The residents don't have even a union and they have to live there. Imagine never leaving the workplace!

You hit the nail on the head. Residents are criminally underpaid and undervalued. Unfortunately, all too often, they develop a form of Stockholm Syndrome and defend/perpetuate the malignant culture and exploitation.

The union will cheer and consider it a win when the residents get a 60 cent hourly pay bump because the hospital had only intended on a 40 cent hourly pay bump this year. Meanwhile it took 6 months of negotiating and everyone hates each other now - My hospital in a nutshell

I mean, that's hardly the union's fault, everyone would hate each other anyway over an only 40 cent pay increase, and that was a battle that needed to be fought. Really sucks that they only got up to 60 cents though

@@innocuoushappenstance6259 A step increase usually doesn't cause "unrest" in the workplace since it's written and expected, but a lot of negotiations and battles between admin and staff for a net 20 cent raise definitely does. I don't directly blame the union for the poor negotiations, but I am surprised at how proud our reps were.

I feel like the ending is a sort-of-dig at a particular CT surgeon at MGH who literally said the same thing on twitter. Said he'd just move faster without residents. He's part of the reason why residents form a union in the first place
