Howdy Doctor I recently had my levels checked due to the fact that my cognitive process was getting slow and when I did my physical fitness program I would not recover within the normal parameters. My T4 is at 1.7 with a total of 12.5 TSH I am 60 years of age male and I am usually physical fit until this happened. My nutrition is on the healthy side and have also learned that the nightshade family can cause a bad TSH result, I have been consuming potato, eggplant, tomato, bok choy, tofu and many other "vegetables" daily in hopes of a better nutritional intake. I am by no means a vegan or vegetarian however I could if my health depends on the commitment on doing so. My physician has prescribed levothyroxine .25mcg. I am feeling a bit better but getting back into my daily routine has been taxing. Any suggestions you can offer are appreciated.

I know it was from several months ago, but I wanted to make sure to answer it anyways in case you're still working on things. Sometimes TSH can increase as a normal function of aging, so I would be curious to see what your exact symptoms were. The T4 you described looks to be normal. For my male patients who are above 60, I like to make sure they're getting enough protein (>130 grams depending on body size), eating foods that work for them and don't cause digestive symptoms, mixing in some resistance training to tolerance, and are of a healthy weight. @@manja546 Just to be clear, is that free T4 and are the units pmol/L? To me the TSH looks a little high and so does the free T4. It would be helpful to know the other thyroid labs you had checked to give you better feedback. Things like total T4, free T3, total T3, reverse T3, and thyroid antibodies. Additionally, it may be a good idea to get another blood draw to establish a pattern and see what direction those markers are trending in. Let me know if this helps!

Hello! So last week I went to the doctor and my tsh was 4.58 and T4 15. I have a hard time breathing and my heart rate rises easily. I also feel very tired and my stomach bloats. I am on a fodmap diet and it doesnt help. I'm not overweight and I exercise a lot. My question is, can chronic stress and anxiety raise tsh levels? I have had physical stress symptoms every day for two years. There is no hypothyroidism in my family and I am 23 years old.

The main problem is doctors arent ready to find root cause. They just give medicines for symptoms. Atleast thats the case in my country.

Hi Hansi, unfortunately, that seems to be the case everywhere. Of course, there are outliers with individual doctors who are more interested in looking at the whole picture when it comes to a patient's health, but that's typically rare. It's frustrating and I don't know exactly why it happens, especially when patients come back to their doctor and tell them that they're not feeling better. I hope you can get some better help soon! Hello Doctor Brad, Iam Priya. My mother is having thyroid. Her age is 62, t3 level 95 t4 5.47 level and TSH 12.09. She has stopped the medication few months back and she is feeling better after stopping medicine. She used 50 mcg for 10 yrs. Can you please suggest what we can do. Thanks

Thank you dear doctor. My TSH is elevated at 5.3 and my doctor wants to put me on hormone medication for my thyroid, however my T3 and T4 are at normal range. I’ve never had issues with my thyroid. I just lost both my husband and my mother and I am under severe emotional stress and depression….. I am engulfed in grief. I’m 43 and I’m not eating at all, I barely eat to stay alive and I’m losing weight very quickly. I believe all these issues have caused my elevated TSH 😢

Omg!!! I have sub-clinical hypothyroidism and I have always wondered how my TSH is high (5.5) but normal T4 and T3 and adequate T4 to T3 conversion. Thank you for explaining it - you are the only one who addressed this !!

I'm happy I could help Mahasweta! If the TSH remains high on follow up with your doctor, you're going to want to continue to track your T4 and T3. For some people the TSH will resolve, for others it may be a sign that you're in the early stages of thyroid dysfunction. Hope you're doing well! Let me know if you have any questions!

@@DrBradBodleHi, my TSH last year was 12, with normal T3 and T4 and negative anti TPO antibodies. I was prescribed 25 mg Eltroxine and the TSH was normally coming down to below 1 in January 2024. But last week, it again rose to 28. Blood work and glucose are normal. No apparent problems as such too. My dose has now been increased to 50 mg Eltroxine. What is going on? Can you explain? Hey Doc Thanks for the informative video. I have been doing research watching videos and reading articles about Thyroid. My TSH is currently 18.05 and T4 0.57. I am 65 male thin and fit lost 50 lbs from 215 lbs 4 years ago to 165 lbs today. I do low carb high fat/protein diet lifestyle. My Doc just says I'm Hypothyroid and because of the numbers wants me on Thyroid Meds for the rest of my life and refuses to do a full Thyroid panel. I have no symptoms and feel great ! I need to find the root cause before I commit to such a life changing thing. Any advice would be helpful . Thanks much Michael M.

Hey Michael, thanks for the message! I'm with you on his and personally I'd also like more info before making a decision on medication. In addition to getting a full panel, I think simply getting your TSH checked again would be helpful to get a better idea of any trends or patterns. If you were feeling poorly, then I would understand more urgency to try and be proactive, but I would take things one step at a time for right now. Not much advice other than I agree with you :) Hope it's helpful though! Let me know if I can assist with anything else!

Hi! You're welcome! Yes, most doctors would have you continue with your medication is TSH is above 60 (the threshold for this is typically around 10), even with normal T4 and T3. However, it sounds like there might be something else going on here since the TSH isn't resolving with adequate hormone levels. It might not be a bad idea to get a full thyroid panel including thyroid antibodies (TPO antibodies and Tg antibodies) to see if anything else is abnormal. Low thyroid function can impact fertility as it is a very metabolically demanding process to maintain and grow the fetus. Hope this answers your questions! Thanks a lot sir, it's really explained neatly. I am diagnosed diabetes since 3 months. It's started with hba1c 9 and now reduced to 7.8 in 3 months. But I was loosing weight regularly. Yesterday when tested for thyroid. It came TSH of 13 with T3 and T4 normal. I don't any symptoms either for diabetes or thyroid until now. Please advise how to treat this TSH and my weight loss or diabetes is due to TSH? Thanks

Keep up the good work 👍 To answer your question regarding the TSH, weight loss can change our hormones in a number of ways that can be hard to predict. This is one of those scenarios where I would repeat testing to see if the TSH stays high. If it does, depending on your symptoms, you and your doctor can decide to take action or wait and watch. As long as you're not having symptoms, you might want to wait until your weight stabilizes to give you a more accurate picture. Hope this helps! Let me know if I can clarify anything for you.

Hi there! These things can take time to change, but if you're not noticing any differences or improvements, then I would reach out to your doctor and provide them with an update. Dear Dr. Bodle. I had a blood test last week and my TSH is 11.000 and my T4 1.00. I did not fast the night before, but had no breakfast. I'm 67, and was on 50mg of synthroid for years. Last year I started having sever muscle pain. It got so bad in my legs at night I'd wake up in tears. My doctor lowered my dose to 25. Still pain. I couldn't handle the pain, so I decided to take myself off synthroid. I'm sure I'm in for a lecture. I started taking a natural Thyroid suppliment containing b vitamins, sellium, iodine etc. I have been feeling great and with no pain. I'm not sure I want to go back on a drug and live with pain. What can I do? What advice can you give me?

This is incredibly helpful! I have had two blood tests (a year apart) come back as Subclinical Hypothyroid. My question is - how long do you recommend waiting to see if your body will resolve subclinical hypothyroid on its own before taking some action to intervene?

Hi Allison! I'm so happy the video was helpful to you! In my practice, I usually like follow up within 6-8 weeks, but it wouldn't be unusual for some doctors to do a 3 month check-in. Both would work fine and give you a better idea of the trend. The big thing here is correlating the labs with your symptoms. If you're not feeling well and you have back to back labs with elevated TSH, then it might be worth it to start making some changes. Hope this helps!

Dr, Just got my blood tests before I got for my yearly visit and my TSH w/reflex ULu/mL and it came back reading high the chart says 0.27 to 4,2 would normal and mine was high with a reading of 5;55. Last year it was also elevated but I don't know how much. MY Dr is going to say you need a hormone I don't take any medication and I really don't want to either. Im 75 and in very good health... So tell me if he wants me to take meds what should I do I refused before. But it's weird because I have no symptoms...thanks for your time...

Hi Doc, Male 41, 200 lbs.I was diagnosed for Hypo in nov 2020 with TSH level 48 and was put on 50 mcg thyroxine which was increased to 75mcg and then 100 mcg. In next 4 months my TSH came down to 2. I continued on 100 mcg for an year and due to some reason was not able to re test. I know it was stupid but 5 months back i changed my dose to 75mcg hoping that it will maintain some thyroid level. However two months back i gained a lot of weight with a puffy face and so when i did my labs last week the TSH was high af 214. I am scared and confused.How could the TSH level go so high in 4 months when i was taking 75mcg instead of my regular 100mcg ? Also now what should be my dose ?100 mcg or shall i increase it ? Plz advice

This is what confuses me a little because my TSH has never been higher then 5.58 and my FT4 has always been in the higher normal range. I was put on medication right away because I had symptoms, was tired for years. But when they found out my TSH was to high they also found out that I had a severe shortage of vitamin D. Which I go supplements for. And finally after many years of feeling tired and lack of energy I felt great. But now when I read into subclinical hypothyroidism I start to wonder what will happen if I stop my medication? Where my symptoms not just due my vitamin D shortage? But at the same time I don't want to quit taking that because I'm scared I will get all the symptoms back I had for years.

I am also in that grey area.....tsh is 5.41 and t3 and t4 is normal......but doctor said you won't be able to conceive woth this tsh level......i started taking 25mcg daily......after 2 months tsh level came down to 2 but than after 2 month I checked again it was 5.47, i stopped taking medicine and suddenly i gianed 4 kg in 3 days,,than my doctor increased my dose to 50 mcg......i am not sure if that is the right approach

Hi there! It wouldn't Be unusual or impossible for TSH levels to fluctuate within a day depending on changes in other aspects of our physiology. In fact, TSH normally has a diurnal rhythm with a peak at midnight and the low point in the late afternoon. An inflammatory reaction or immune system change because of increased stressors could easily impact levels. I don't think blood sausage would be a huge factor here, but I could be wrong. Hope this helps!

Hi doctor, I’m currently on 175mcg of thyroxine and 5mcg of thyronine and levels of TSH came back at 65.9 while T4 came back at 1.00 and T3 was at 2.00. I have no symptoms of hypothyroidism though, I feel great. Energy is great, no weight gain or loss, everything seems to be normal but I know this abnormally high. I’m not sure of the issue if you have any advice?

What would make TSH (other things t3/t4 are in optimal along with good lifestyle,diet,etc) fluctuate in a male from 2.3 to 4.7. Seems like mine vary a lot for some reason. Some say it's iodine,other doctors say too much iodine can cause it.

Hi Val, when there's a lot of TSH variability, I would wonder about thyroid antibodies. Have you had TPO antibodies and Tg antibodies checked before on your blood labs? Iodine can cause fluctuations, but an individuals sensitivity to iodine is often dependent on their health history and underlying conditions. As a general rule, I don't think people should be taking more than 150 mcg of iodine per day, but again, we'd need to know more about the person. Hi Dr. I am 35, eat healthy and exercise try to take care of my body 5'4" 120lbs My TSH spiked from normal range to 45 my t3 and t4 were normal. Endocrinologist put me on 100 mcg of levothyroxine and my tsh in 6 weeks went to .25. It keeps fluctuating and I feel so anxious now I hate taking medication. My TPO antibody was 160. Now I am having gallbladder and liver issues is this related?

Thank you for this video! My issue with thyroid problems is that an endo is the worst person to treat them! Yet I keep getting referred to an endo! If it could be a gut problem for example, why is it not a gastroenterologist with endo background! Is there such a thing?? Endos lack the wholistic knowledge that we need as “healthy within range” patients.

Hi Jumana, I totally agree with you! Unfortunately, in my experience, gastroenterologists can get caught in the same trap as endocrinologists. Both are can be unwilling to look beyond their niche and consider the whole picture. I've even had patients who told me that their gastro said diet had nothing to do with their gut problems, so who knows! I am a guy had problems sleeping and my face and hair was very very dry . I went to see the doctor mainly for sleep , after a blood test i was told i had thyroid problem and would have to take levothyroxine 100 mg the rest of my life .16 years later after so many blood tests over the years feel crap . How knows if i have thyroid problem

Hi Phillip, this would be one of those cases where we would want to get more lab work done to determine what's going on. Ask your doctor if you can get an expanded thyroid panel, but if that looks good, have them assess other areas that could cause these kinds of symptoms. Wishing you the best with everything!

Had my bloodwork done today due to hair loss, and my T4 is normal and TSH is over 10. I have been on Levothyroxine 100 mcg for years now. Is it not working? I am going to see my primary in November, but I am hoping you can provide advice in the meantime. Thank you for the informative video.

Hi Ritika, TSH above 10 is at the point where we might want to be more proactive, but it really depends on if she has any symptoms or not. I'd have her check with her doctor for the next steps. Hope this helps and thanks for the question!

Hi Rose, TSH should not continue to increase after being prescribed medication. It's possible that the dosage isn't high enough or you're reacting negatively to that particular form of thyroid medication. I'd follow up with your doctor to see what other options are available in addition to getting a full thyroid panel including thyroid antibodies to see if there's anything else going on. What are your thoughts on high TSh (above 10) but with someone who had subacute thyroiditis… so six weeks ago they had extremely low TSH and high T3 and t4 and now they are in hypo state. How long would you let this patient wait to see if the increase in TSH does cause the thyroid to begin producing the right amounts of T3 and T4 after the viral infection??

Hi Jenny, I think getting labs monthly at a minimum would be a good idea. More frequently if something changes and the patient starts feeling worse. Being as proactive as possible is always the best way to do things in my opinion. Let me know if this helps answer your question!

My TSH was around 5.5 and I hadn't noticed any symptoms beyond fertility issues when I was first diagnosed but because I had antibodies, low T4 and was trying to conceive I started a small dose of generic levothyroxine. Within a few months my TSH was a 30, and other labs simliarly out of whack. I always wonder whether it's possible that taking the medication caused the drastic changes I saw in such a short amount of time? Or does the fact that they increased show that I indeed needed medication?

Hi Kelsey, thanks for the comment! It's hard for me to say without knowing all the details, but it is very possible for people to not react well to certain medications (I have some patients who don't tolerate most thyroid meds). Whatever the cause, I would not say that having the TSH increase is a good outcome or that it shows you really needed it. Let me know if you have any questions! I'd be happy to discuss this more.

I came across this video doing research! I have all the symptoms of hypo but my endo just said I have all "normal" labs and there is nothing they can do for me. My TSH was 3.49, my t4 was 0.54, and my t3 was 3.9 .. what do you think?

Hi Sara, so glad you found me! It might be worth your time to get a full thyroid panel to see if any of the other markers are abnormal. If it's one of my patients, I like to check TSH, total T4, free T4, total T3, free T3, T3 Uptake, reverse T3, TPO antibodies, and Tg (thyroglobulin) antibodies. If those all look good, then it's possible your symptoms are being caused by something other than thyroid. I also have a few videos explaining some of these markers and reasons why you have normal TSH, but continue to have symptoms. My TSH test was 4.550 is that a concern? Dr. gave RX of ROVOTHYROXIN 25 mg. I am 76 years old and DO NOT WANT TO TAKE THIS. I was diabetic and brought my count down just by watching sugar and carb intake, this shocked Dr. NO Metformin ever for me. If you do the work diabetes IS curable. I am 6.3 from 8. I was diagnosed with subacute thyroiditis this past weekend. I just received my blood work results and my TSH lvl is 50.79, T3 free is 2.2, and my T4 free is 0.6. What does this mean?? Hi there! As I mentioned in the video, TSH that is below 10 might resolve on its own if you're not experiencing any symptoms. If you are having symptoms, then I think it would warrant a full thyroid panel to better understand why the TSH is high. From there you and your doctor can give you a more specific recommendation to help you improve. Weight loss can be helpful for improving thyroid symptoms, but it's only one piece of the puzzle. Hope this helps!

@@DrBradBodle yeah the fatigue & hair shedding is also a symptom of the low iron stores & anemia that I'm being treated for with iron so it's hard to tell if those symptoms are from thyroid. but I'll ask for the full panel anyway..thanks

@@diamondsandlipgloss Absolutely, and it's possible that the low iron is also the cause of the elevated TSH. I'd continue to work on supporting the iron and track how things change, then once you get the additional thyroid information you can add anything else that you might need.

I got High TSH after taking 450mg aktive absorb magnesium. after 4 days i stopped magnesium and the day 7 i got blodwork and it was 6.8. and normal range t4. Very sad cause i had it so good the 3 first days of the tree types of magnesium i toke. i toke 300mg ashwaganda a week up to some days before i stopped magnesium, but i don't think it was it i got sick from. Maybe i can take like 200mg magnesium and work normally, any suggestion for that :) ?

My tsh is 10.4 will my pituitary dry up and become a raisin? 6 weeks ago my tsh was 6.4 My t3 and t4 is normal. When they tested this week my tsh was 10.4 but they did not test my t3 and t4. I’m on keytruda for reoccurrence of cancer that was in remission for 4 years .My dr put me on thyroid med thst begins with L and I can’t pronounce it nor spell it. Is my dr making a mistake? I don’t understand why my high intake of iodine didn’t prevent this problem. I’m 50. My tsh was constantly 3 or higher for 5 years which I think is high and explained a lot of my symptoms

Hello sir ,, last year my fiance suffer from tuberculosis neck lump , she get medication about 6 to 8 month and get cure .. Hey doc hope ur good So 2 months I tested for vitamin d and thyroid The level of vit d was toxic 102 and they asked me to stop taking vit d and for the first time my tsh was 4 something, however, 4 months ago I tested for tsh it was 1. Something. I took ultrasound for neck today due to other reason, they told me I have my right thyroid prominent a little check ur tsh so I showed them the previous tsh which was 4 something she said yeah that’s fine Should I be worried about something doc pls? Hi there! For most labs, the typical range is .45 to 4.5. However, it's possible to have normal TSH and still not feel well. So you'll want to look at the other thyroid markers too to determine how things are going. Hope this helps!
