Thanmathra was really really tragic man, I watched it two years back and I feel I am brave enough to hold my tears in the dark moments, especially the one when Mohanlal bites his finger and bleeds and even in the death scene, he was shown to die in sleep but the BGM was enough to haunt anyone. The even more emotional part was that his son was praising him in the IAS interview unknown to the reality that he lost his father.😭 Mohanlal somehow saved Priyadarshan by noticing symptoms of memory loss in him and Priyadarshan got treated and made a comeback in life, otherwise Lal's darkest film would have been his bestie's dark reality.

I watched Thanmaatra after i lost my grandfather to alzheimers. I bawled through the second half. Mohanlal portrayed the helplessness of a person who lost memory beautifully. Its too difficult for me to revisit this movie. My another suggestion would be Thalavattom

For me, GARGI starring Sai Pallavi will also make the list for its extreme emotionally haunting feature. The shock and emotions were too hard to take. 💔💔 CHITTHA and UGLY can make an entry too. From my mother tongue Bengali, I can put PATHER PANCHALI and DRACULA SIR, though Dracula sir was not much disturbing, but it was very hard to see the protagonist played by Anirban suffer.

You should watch 'Hostel' (which released in 2011) starring Vatsal sheth. Woh film dekhne ke baad mujhe 3 raat theek se neend nhi aayi thi. It's impact was such. Really, not for the weak hearted audience.

I recommend you to watch Kireedam, Chenkol, Dashratham and finally i highly recommended you to watch Akashdoothu 1993 it will disturb you so much, the emotional effect of the movie will be for days. I think the movie got a national award.

I saw Thaniyavarthanam at the age of 5 . N even a kid of that age could clearly understand the turmoil of the actors esp the protagonist(Mammootty) and the pain it created in ones mind was immense. The way how a society can make a perfectly sane man to question his own sanity. I didn't watch it a 2nd time coz its truly heartbreaking!!!

Mahanadi was an accidental watch telecasted in tv AND IT. HAUNTS me till this day.the thought of the film itself by name throws shivers down my spine. One such accidental watch was" Hello darling" starring naresh in Telugu .I hve definitely promised myself I would never let my children watch these films as they scar them fr life.

There is an office scene in "Thanmathra" where Mohanlal's character starts behaving like he is at home. I could never watch the movie beyond that point. There is another Malayalam movie called " Aakashadooth". Basil Joseph recently told in an interview that he could never watch that movie again. Even I feel the same way. It is a really heartbreaking movie.

Dushman and Sangharsh both directed by Tanuja Chandra and both film's villain was Asotosh Rana. Dushman's rape and murder scene was shown in Zee Cinama at 1:15 pm at that time. I had luchbreak and that scene made food come out of my mouth and i returned school empty stomach yet all i can feel Asotosh Rana's remorseless face and Kajol's fear. And Sangharsh was seen after a wedding..... And the obvious couldn’t sleep for days.

The story mathrubhoomi itself is so disturbing that you couldn't watch it in the first place. I've never watched thaniyaavarthanam and thanmaathra second time. A recent movie is ADU JEEVITHAM & some of Padmarajan's movies are so haunting that you'll get sleepless nights. Trapped is another disturbing movie which I couldn't bear with.

A big miss on the list is "Maya". The movie centers around an innocent young girl and her happy life in rural India. However, when she enters puberty at 12, her parents takes her to the village temple for the coming-of-age tradition. The brutal climax of the ritual is shocking. The girl, Maya, is raped by numerous holy men. This is part of the ritual, and guests feast outside the temple while it's happening. The adults in the story see such rapes as rites of passage and celebrations of family prestige.

If you're willing to try something different, I'd recommend seeing the Telugu film "Republic," which terrified me to no end. The selfishness of society was depicted in this movie. This movie is not a realistic one or with great acting skills, but the final product genuinely frightened me.

Try watching Sadayam. I can give you a list of Mohanlal movies alone for this countdown. The unfortunate part is Mohanlal is such a good actor that you are left with a pang in your heart for days after you watch the movie.
Jammy you should have added the movies Visaranai, Viduthalai and Asuran all 3 movies directed by Vetrimaaran they also would fit in this list.

As far as tamil movies goes, i feel jammy has missed some of the cult and gut-wrenching ones starting from the historic "moondram pirai", and mid-age "pithamagan" & "anbe shivam" to the modern "Chitha!" All were absolute gem of movies. Throw "nooravathu naal" into it as well!

There are lots of films in this type of genre(?) performed by Mammootty and Mohanlal. There are so many malayalam movies that have traumatised me in my childhood that I don't dare turn back to yet as an adult. The most difficult/memorable one was when I, as a kid, was forced to endure this Mohanlal movie 'Mizhikal Sakshi' in the movie hall because my family thought it would be an alright family movie to watch on a weekend, thanks to the lack of trailers/promotions and the habit of blindly going for the movie that's just playing locaclly in the halls nearby in that era of film-making/film-watching.

I never want to watch Danger (Telugu, 2005) directed by Krishna Vamsi starring a bunch fresh faces back then. Its a chase movie with a lot of comedy and excellent pacing, but when the bombshell drops on why our heroes are running, all the laughter and fun, gets flipped on its head. That twist makes me sick even today

Anurag Kashyap's Paanch (not even released) and Raman Raghav 2.0 top my list. It's just the effectiveness of the performances (KayKay and his gang in the first, an absolutely top-of-their-game duo of Nawaz and Vicky in the second) and AK's uncompromised filmmaking that pushes these just over the edge. Haven't dared to revisit either. Sriram Raghavan's Ek Hasina Thi is another one that I found pretty tough to revisit (I did, though), because after a point you just can't sit through what Urmila's character was dealing with. And that end to Saif's character is one of the most creepily horrifying things I have ever seen on film. Tumbbad is another one I have managed to rewatch with some difficulty, especially the climactic portions, because of just how catastrophically disturbing it is. The film that I have vowed never to revisit (any language) is Requiem For A Dream, kyunki I just know ki nahi ho paayega. Too raw and dark.

I saw Raman Raghav 2.0 snippets and immediately the last scene of the forced home entry with the cellphone bait came into my mind. I had seen it in theater for the 1st time and although I have rewatched it many times, I can never get myself to watch the last scene. All of the horror in it is implied and yet it's the most disturbing for me, more than all of the portrayals of incest, murders of children, s€xu@l and drug abuse in the whole movie.

Watch this 1991 Marathi movie called Chaukat Raja starring legendary actors like Dilip Prabhavalkar and Ashok Saraf. I wouldn't call it a disturbing movie but a few scenes are difficult to watch. Hats off to Dilip Prabhavalkar for pulling off such a difficult role.
