She highlights the preventive measures that can help maintain the hair that is present on the scalp and also for the transplanted hair. She has outlined the salient features of the traction alopecia and the manner in which hair transplant modifies itself to restore hair in such cases.

Cause of Traction Alopecia
• Traction alopecia is the hair loss that occurs due to tying our hair too tightly. 00:27
• This is commonly observed in Sikh male individuals as they tie a turban too tightly because they have been tying turban tightly for quite a long period of time in the number of years. 00:31
• Also happens to those, who have tight hairstyles like Afro- American, and the Caribbean. 00:36
• What happens is Afro-Americans have a hair loss on the front side. whereas in the case of the Sikh population because they tie their hair from all the sides they will have hair loss from the front, from the sides, and from the backside also. 00:56
• This can also affect the beard area, when they tie their long beard too tightly then they can develop a patch of alopecia either on the right side on the left side. 01:07
Prevent the traction alopecia:
• First and the most important point is making a loose hairstyle they can wear a loose turban or they can adopt any kind of hairstyle but they have to consciously try to tie the turban and to make the hairstyle in such a manner that the hair roots are not pulled too tightly for a long duration of time. 01:18
• Early prevention of traction alopecia in individuals, because they are tying the turban for quite a long time now and have experienced the loss of hairline from the frontal area because of this strong mechanical full is called traction and how we can arrest traction alopecia in its early stages. 01:38
• Even after treating traction alopecia with the hair restoration surgery the transplanted hair may fall off again if the traction is continued. 02:06
Features of Traction Alopecia:
• The hair loss is usually asymmetric in such patients. It is more on the right side or on the left side and when we are doing hair restoration surgery for such patients we have to undo the asymmetry and recreate asymmetrical lines. 02:14
• There is an abrupt transition of density between the bald area and the non-bald area so there is a shaft demarcation beyond which we have a very high density of hair the reason is that they don't have concomitant androgenetic alopecia. 02:33
• When we are doing hair restoration surgery in such patients we have to create a high density for extending from hairline till the junctions and we don't have to create a density gradient which we usually do in a patient with androgenetic alopecia. 02:46
• We have to pay attention to the curl direction and angle of these here, we have to recreate the temple points which usually requires more graphs and unusual case of androgenetic alopecia. 03:08
• We have to create the side locks also and the junction between the temple points and sidelocks is very important as aesthetically there's a transition in the angle in that region and we have to pay a lot of attention to recreate in an aesthetic manner. 03:20
• There is a smooth transition of hair from the lower end of the temple point to the side lock and that smooth transition has to be recreated. 03:33
• Yet another area which we have to recreate is this zone which is above the earlobe, so this area is also bald in some patients because of the tight pulling, and because of the tight pulling, there is a bizarre angle and direction of the hair in this region so we have to recreate these areas also. 03:40
Finasteride requirement:
• Some patients of traction alopecia may be having concomitant androgenetic alopecia also in such patients they have to take medications post-surgery which mainly includes oral finasteride to maintain the pre-existing here and to prevent the progression of baldness. (04:00)
• We cannot make a low hairline in such patients and we have to consider their future potential needs because of the progression of androgenetic alopecia. 04:15
Beard Transplant:
• While doing a beard restoration surgery in such patients we have to pay a lot of attention to the direction and angle of the hair and it has to be recreated in a very aesthetic manner these angles and directions vary a lot from one person to the other and if we do it correctly at the end of the day it is very satisfactory. 04:26
Non-trim approach
• Lastly, we should know that there is no need to do a full trimming in such patients because they have long hair and we need to extract from a fixed safe donor zone we can do a window trimming on the backside which will be hidden by the long hair falling from about so there is no need to do a full trimming in such patients. 04:32
