A wonderful, simple clarification. The most difficult thing to do. Thank you so much.

In free indirect speech (present tense narration), should I turn "Oh my God" into "Oh his God"? E.g. "George is breathing heavily. Oh his God, is he going to die here?"

In free indirect speech, the voice of the character mingles with the voice of the narrator. It would be something like this: “George is breathing heavily. Oh my God, am I going to die here?” So the first sentence is actually part of the narrator’s voice, and the second is part of the character’s voice. It’s up to the reader to make these distinctions! I hope this helps! :)
