Hi, medical professional here. The "Tourniquets are a last resort" thing is outdated, doctrine has changed and EMTs are nowadays much quicker at applying them, as it is fairly easy for a trained person to determine an aterial bleed which usually can`t be stopped by wound packing or pressure bandaging, thus making the quick response of applying the tourniquet a more viable option. Tourniquets are also fairly safe up to 2+ hours (according to recent studies), especially the flexible one`s like the Swat-T, tissue damage can be repaired, and the loss of an extremity in order to safe a life is considered acceptable. The hemostatic gauze has actually not been proven to be superior for woundpacking than normal gauze as far as I know, so you could safe some money there, but on the other hand the hemostatic gauze also might potentially have increased effect, we simply don`t know for sure.

The C.A.T. and the SOF-T are the only two recommended by TCCC. Celox is better because it's not affected by blood thinners while quick clot is.

I was an EMT for 6 yrs, a few things are handy in an emergency such as small bandages and a tourniquet. Honestly, most of the stuff included in commercial IFAK isn't useful to the average person unless they get training. If you need more than a band aide.......YOU NEED HELP. Time is the enemy when there is trauma, in my opinion your phone is your most valuable piece of first aide equipment, have it with you and keep track of the last place you had signal if you are in the back country. Your kit seems pretty reasonable, knowledge and a bit of creativity is THE BEST thing to have at your disposal.

Great video! Good suggestions for what to put in any first aid trauma kit. My go to first aid kit packs for the last couple years or so has been Vanquests FATPacks. I have the largest and smallest sizes and I'm going to get at least one in the new first responder red color. I love how easy they are to open and how much stuff they can carry that is easily accessible. Yeah they are a little more pricey maybe, but they are top notch quality and super useful for their purpose.

@THE_OG_MP I have a dark angel medical ankle ifak i wear on duty its comfortable and is well made. I wear it for 12hrs no issue. I also wear it off duty when i have pants on

Great video!! I totally believe in carrying a IFAK kit with you and knowing HOW to use the contents! I get it’s not as fun... as knives, flashlights, and guns.... but saving your life or someone’s else’s is pretty cool too! 👊🏼 good stuff Aaron 👊🏼

EMT here. Awesome kit, good to see people ready for medical emergencies. Very well covered on trauma and bleeding. My only ideas for improvement are to add more supplies for medical emergencies, like a CPR face shield, Naloxone, an NPA, etc. You'll run into a lot of non-trauma emergencies too. Great video!

The only two things I would add are both small, can save a life, so I would recommend a small CPR mask and one set of Steri strips and you would be good to good.

Vaseline gauze pad. You can use it as a chest seal if needed. You can even open the packaging completely till it comes apart, and use the sterile "foil wrapper" for a chest seal.

Combat gauze and Celox can take up 3 minutes to work. Celox Rapid, under a minute. A newer, faster acting Combat Gauze should be available maybe this summer or fall.

You know a large portion of the population doesn’t respond to Tylenol/Acetaminophen but they do to Ibuprofen/Advil, or vice versa. Additionally, Ibuprofen is generally considered to be more broadly effective for pain relief because of its roll as an inflammation reducer. Since a lot of discomfort such as back and joint pain is due to inflammation , you may consider splitting the storage container 50/50 Tylenol/Advil and ask whoever is using the med which one has historically worked more effectively for them. Per drugs.com- “Either acetaminophen or ibuprofen may be used for headaches, fever and minor aches and pains, but ibuprofen is more effective for strains or sprains.” Additionally as for the first camo ifak I was wondering if you had some flat Z folded gauze in there for wound packing? Some of the newer T3 Izzy bandages come with integrated gauze for wound packing but I’m not sure if the one you had in there did or not? Either way stand alone gauze is endlessly useful and flexible, plus it could be integrated with a lot of the other wraps you have in there to address some pretty serious wounds that don’t quite meet the tourniquet level of bleeding. Great vid- for anyone who enjoyed this vid I would highly recommend nutnfancy level 2 kit vids 1-5 for ideas, as well as any content from Skinny Medic for further coverage of first aid.

I think his thought process when titling the video was "why bother" well let me tell you why you should bother". although he doesn't speak in a persuasive manner he speaks in a informative one which will hopefully achieve his goal in helping others build a useful kit which before watching may not have even been considered.

Some tourniquets will not work on children with small arms and legs. If you have children I would suggest a SWAT-T tourniquet. Also remember your can use a belt, shemaugh or even a large bandana and a stick for a tourniquet. Some type of flashlight/headlamp is critical as well, accidents happen at night. Great video, thumbs up.

Chest Seal and NPH really don`t make sense, you`d never need the chest seal because it`s use is too specific. The NPH is a hygiene item that just takes up space, just wipe the puke out of the mouth. An Endotracheal Tube Kit would be much better, but also waaayyy bulkier. A chest seal can also fairly easily be improvised with a blastic bag and some duct tape, so in case you win the bad-day-lottery and get shot, you can rig one up, or use vaseline gauze. Safe the $$ you`d spend on a premade one.

My biggest fear is an ankle injury if you’re hiking and being stuck in the middle of nowhere. I believe in self rescue and calling the Calvary would be my absolute last resort unless I’m bleeding out. Can anyone recommend some kind of lightweight compact Velcro ankle support wrap in case of this type of injury?

For the last few months now I've been belt carrying a Gen 7 CAT tourniquet, gloves, and a sharpie, along with my cellphone and notepad, in an M48 tactical pouch that I picked up from BudK: www.budk.com/M48-Gear-Tactical-Belt-Pouch-Black-30364 Then, in my rear pocket I have the Maxpedition micro pocket organizer that you showed in the video. In that I carry a 4" T-3 Israeli bandage (which includes some non-hemostatic compressed gauze sealed in the same package), and a separate pack of Celox ribbon gauze.

Man, I appreciate your videos but a swat T is not a life saving device. I don’t care what NTOA says about it or what some other guys say about it. The fact is that it does not meet the standards for a TQ by the Committee on TCCC, they recommend two TQs, the CAT in its various generations(gen 7 now) and the Softt wide. There have been numerous studies about the efficacy of TQs and the swat just does not even come close to being as effective, especially under stress. I’m willing to provide my credentials and why I’m qualified to speak on the subject as well as links to the studies I’m taking about. As a KZitemr talking about this type of thing you have an obligation to put out top quality information and I just can’t let this sort of thing persist. People buy the swat t or the rats to try and save a buck but they simply aren’t as effective, they also really don’t save you that much space either when compared to a properly stored CAT or SOFTT Wide. What is your life worth and what is your family’s life worth.
