My last 4 doctors say my levels are exceptable, I've had no sex drive,erections, weakness,etc. for YEARS. They have all just shrugged their shoulders and said hormone replacement isn't done anymore because they can cause cancer

Well that would be a uninformed answer the doctors are giving in regards to not being done and causing cancer. We do talk about that in the video but also a great book for that is testosterone for life by Dr Morgentaler, a Harvard trained urologist, that is a leader in the area of testosterone and men.

I don't understand how he can say creams don't work. My testosterone was 240 and is now 1123 on cream. And I'm 61. But sounds like it works to me. And it's steady I apply at two times a day

Cream works very well for me. I use a compounded cream that is perfect. The skin on some individuals do not absorb the testosterone, so they need to try another method. Try it first and and see.

Testosterone in cream has an erratic absorption. Most do not feel great because the levels are not consistent. A pt can be 300 one day and 1200 after taking but it drops back down. It is consistent optimal levels that result in the most benefit. That is why he shared his experience to be that. There are exceptions but you do not know until you compare the difference. Essentially we have no men on creams in our practice. Hope that helps!

For the last three months I have been visiting a cadre of doctors, including my internist (GP), trying to get to the bottom of the many symptoms I have been experiencing. They include shortness of breath, fatigue, brain fog and weakness. My GP started with an X-Ray that showed large lymph nodes. I have been scanned with CT & PET and had a broncoscopy. Bottom line is they think it might be lymphoma or sarcoidosis. A couple of weeks ago my GP scheduled me for a blood test to get my ACE levels. I know that in the past I have had lowT so I asked to get those tested as well. My T came back

I am 60 and have been doing pellets for 7 years. They Work. It’s not all about libido, sex , etc., although that aspect of my life is Great due to the pellets. The Main benefit to me is energy level. As we age it goes down. Our gas tanks get smaller and we run out easier during the day. Testosterone via pellets provides the gasoline needed to get through the day.

Awesome and informative interview! I’m a 59 year old male that has been on injectable Testosterone for just over a year and must admit it has changed my life habits immensely. I suggest anyone over age 50 to get a blood test and see your levels . My Dr referenced trt levels like a 8 cylinder car …. Why would you want to run on only 4 cylinders ? You would take it in and get it fixed ! Lol

@@drdebradurst My Urologist told me that I need to have two low readings per the FDA to start TRT. The first one was low out side the reference range and the second one came back within the reference range. So at 60 years of age doing everything the Urologist suggested, Better overall health, loosing 25 lbs, exercise, proper sleep using a VED, etc, my sex drive is lacking. I thought optimization would be a personal choice but the FDA says otherwise. Any suggestions? I love your show!

Non - optimal levels and symptoms can dictate treatment plans. Look for an HRT specialist in your area and you can search on A4M for one that is trained, boarded and / or a fellowship www.a4m.com/find-a-doctor.html. Does that help??

@@drdebradurst Yes, Thank you. My Urologist has put me on DHEA 25 MG once a day and MACA 525 MG twice a day. She will re-check levels in 6 months as Total and Free Testosterone levels are a little low but above the level where they can be treated with TRT. Thanks for your reply and God bless!

Weird how each practiconer is different. Most I have listened to dont recommend pellets because you cant adjust the dose after the implant, most dont recommended creams but most by far go with injections and depending upon SHBG levels and pt response inject 1x/wk , 2x, or every other day so blood levels stay stable. .any are using insulin suringes and microdosing subQ to slow absorbtion. Personally I injest 3x/wk IM. I have a friend who currently injects it had used creams and actually really liked them and his blood levels went way up from them.

The best way of administering Testosterone is sub Q to the fatty area of the stomach. You don’t need a huge intramuscular needle.

Bullshit regarding cremes. Creme works just fine as long as it's concentrated enough... Like 15% or so. I use an 18% creme and my levels are between 900 and 1200. Apply the creme to any area free of hair or shaved, and alternate locations. The greatest absorption will be when applied to the scrotum, but it's not that critical. The GREAT advantage of cremes is you can use testosterone proprionate, which has a short half life of 19 hours. This mimics the natural diurnal ryhthm of testosterone fluctuations. The protocols promoted here will, over time, result in saturation of estrogen receptors despite serum estrogen being within normal ranges. The result is loss of libido despite high free testosterone levels. Bottom line is try to mimic what the body does naturally in terms of testosterone levels fluctuating over a 24 hour period.

Another very good video. I am 71 and my last Testosterone level check was something like 274 and my Primary was still telling me that my testosterone level was considered Low Normal. Obviously, i was buying that. I have been using Testim 1% for the last severl years my levels were somewhere between 374 and 423. In fact in December my level was at 424 and when measured again in March, i was at 274 and still being told that levels were still Low Normal. This cannot posstibly be right!!! Thanks

I am 44 and I had all symptoms of low T. I also have high cholesterol. A local male clinic run blood test and my total testostrone came back 420. I decided to give testostrone injections a try even though I was technically within the normal range but on the low side. I was suffering from low/no sex drive, depression and, lack of concentration, foggy brain and wanting to take naps all the time. Doctor started me on 150mg weekly injections, i started to feel much better a few days after my first injection. I had better mood, more energy and motivation. I am in third week and I noticed an immense pressure in my eyes that is unbearable. I checked my blood pressure today and it was around 157/97! I had no history of high blood pressure before starting TRT. My blood pressure used to be around 120/75. I contacted the clinic but the doctor is out of town for a few days. They asked me to stop the next injection until he is back. Could having a high cholesterol cause blood pressure to go up after starting TRT? Will my blood pressure normalize eventually since my body is adjusting to testostrone getting injected every week? Not sure where to go from here. Can a lower dosage reduce my blood pressure? I had informed the doctor about having high cholesterol but he did not think that will be an issue but I think that could be the cause of high blood pressure. My arteries are probably a little plagued and testostrone is possibly thickening my blood and causing my blood pressure to go up. This is my theory but I am not an expert. Has anyone experienced this?

Make sure you take a baby aspirin because TRT thickens the blood. Take some l citrulline for your BP. It will lower it. 😎. Works for me😁

@johnheueisenIII Yes, you are absolutely correct. I stopped TRT for 4 months. I saw a new doctor and she put me back on TRT since my total T was 400 and free T was barely making the lower end of the range. She put me on Testostrone Enanthate 100mg/Week (Xyostead). Bp was stil around 135/90 but not as bad. She added Cialus 5mg/day, and my BP is now 115/75! My T is 1100! I am thinking about dropping my dosage to 75mg/week since I have acnes on my chest and back but no other side effects.

Using bioidentical testosterone, progesterone &DHEA in cream once or twice a day applied to perineum area over 20 years ,with no adverse effects. Regular blood tests monitor testosterone levels & maintain optimal to higher levels of hormones. It's a joke for endocrinologists to claim that injections of non bioidentical hormones is preferable. Creams are safer & effective. I suspect they want financial benefits of patients requiring their services or they are culpably ignorant. Try the cream for effective bioidentical hormone delivery. The proof is in the pudding.

58 year old male; I have done both pellets and injections. Prefer the pellets but my insurance no longer covers them so I am back on injections. Any chance in the near future that the price of pellets will go down ? Any secrets on dealing with BCBS and Aetna on getting them to cover the pellets ? Thank you for listening and sharing this all too often & unfortunate scenario. Pellets definitely more beneficial esp when there are erection changes. It takes the hormonal fluctuation out of it, allowing us to do regenerative procedures if stronger erection still needed. Most traditional doctors do not do pellets as typically insurance does not cover it. We consider hrt, when done in the right way to be, the non lease covered items or the non insurance covered items. We have insurance so that there are not large out of pocket expenses for acute, unexpected or serious issues. We lease cars for the same reason --so no large out of pocket expenses for unexpected issues. But the breaks and tires are not covered because we all need them to drive so people have to pay for them. HRT is similar ---it is life changing, necessary and makes people feel better so mainly it will be an out of pocket expense and not covered by insurance. Insurance dictates what traditional medicine can do ---which traditionally is disease, diagnosis and treatment of the diagnosis. It is so unfortunate but true. We wish we can say we are making progress but we have not seen real progress traditionally in the arena of hrt in decades.

Pellets are the worst. You feel great the first month and it’s three months of downhill hell. Plus it cost way more than injections. Which is why these doctors push them. I switched to injections three times a week and it’s steady as can be. Who wants to get cut open three times a year? I still have a large bump a year later where I had it done.done once

Thank you for sharing your experience and listening. All treatments are customized to the patient. Definitely there are men that do prefer and then are treated with injections. We guide but ultimately it is always the patient choice. On the flip side, there are many men that are not consistent with injections or hate getting or giving the shots---then they do not feel better. One size fits all is what does not work. We appreciate your feedback about pellets.

While a triathlete, I was on HRT large dose injections to balance my T numbers from 49-52, then had a blood clot in my spleen, instantly no more HRT, not even low doses. Now, doctors just shrug their shoulders at low T levels and ED. I stopped triathlon, took up jiu jitsu, lift heavy, healthy, clean, eating and have HITT sessions. All of that has little to no effect on my T levels or ED. Now what? Cialis does its job, but is there anyway to get the job done without the chemicals?

Not accurate info. Diet has a huge effect on testosterone as men age. I have eaten a whole food, plant based diet for 17 years. I'm 62 and my total T is 639 ng/dl which is at the very high end of the normal range. High T is normal for males who eat like me.

8:03 I love this podcast, Dr Durst & “the grrrlz”(the other employees on the cast-get your head right) & Dr Ibrahim. He is obviously an expert on all things “T” and seems to certainly command an amazing knowledge of many other facets of GP healthcare. With that being said, this episode, while chock full of vital information and award worthy analogies, is what I imagine all podcasts are unknowingly working towards; two AI manikin robots that look pretty damn convincing and definitely on point with their specialties. However, the perfectly “slick” forehead’s coupled with the obvious “blank,” “dead,” “1000 yard stares” that they both proudly possess is slightly scary to me, a man who admittedly looks older than my 52yrs. If this is the only alternative to cleaving off a few aesthetic years, then we need to seriously reconsider our ideas of AI, the future and most importantly…BOTOX. LY Rvtlz for all that you do & provide for the local community and ultimately the world.

We're sorry to hear you're experiencing symptoms of low testosterone @messiii.007. You need to find a provider that specializes in Hormone replacement therapy -that can assess numbers, symptoms and provide solutions. Assuming you mean 21 for TT you would fall within a normal range but not optimal if having symptoms. An HRT specialist would be helpful for further directing you on next steps! Hope this helps. Ideal Testosterone levels is factored on a few things like age, medical history, medications, and more. Optimal could be anywhere from 800 - 1200.

Hello, I have been doing injections for 21 years as a result of extremely low testosterone. I have never been told by my Urologist about the pellet therapy. Is there a higher cost involved with the pellet implantation? Would the pellet procedure be covered by Medicare and supplemental insurance?

We love pellets in the right patients. There usually is a higher costs and not typically covered by insurance. There are some other ways that your testosterone production can be boosted to help maintain fertility ---when young and fertility is a concern. Correct. Possible prostate enlargement and shrinking ot testicle are possible side effects with BHRT. They also don't mention you need to take DIM supplement with the therapy. Capitalism for you, go figure.
