Thats NOT an Aussie accent. That a New Zealand accent. The difference between the Aussie and the Kiwi accent is that the Aussie accent is more British sounding. The Kiwi accent sounds alot like a White South African accent. So these two are NOT Aussies. They are Kiwis because they both speak like White South Africans. The dude in the video is definitely of British and English descent. He could easily become a Hollywood actor. He has the facial features of an Anglo Saxon from ancient Britain. I came to this video to learn about medical examinations but i stayed purely because of him. (Im Blasian).

How do you differentiate spastic paraplegia with parkinson disease ? I have bilateral babinksi, hofmann and romberg sign. I also have iron deficiency. Rhanks

Spastic paraplegia has upper motor neuron (UMN) signs with spasticity, weakness and hyperreflexia. Parkinson disease is an extrapyramidal disorder (the motor system outside of that which passes through the pyramids of the medulla (i.e. the UMN)); it does not cause true neuromuscular weakness on bedside testing (except for from disuse later in the condition) but rather slowness, tremor and rigidity (a different type of hypertonia, as compared to spasticity). Hello doctors my body twitching all the time palms thighs ribs muscles moving around the body. when thighs moves or twiches different area will twitch ribs or biceps muscles. Implied verbal consent for “examining [the] lower limbs” is gained at the beginning of the video and any more detailed a display of this is beyond the scope of the video and would make it quite long for the target audience. If you need tips on how to introduce yourself and commence physical examinations, I would suggest more basic videos to start with, as these videos are for more experienced medical students, doctors and neurologists. @@Meducation101 hi doctor, I'm 33 healthy and I am a automotive mechanic by trade. Months ago I hurt my right wrist and the pain was extremely painful! On and off from about may to August it hurt. I noticed i get twitches in fingers from working out but my right ring fi ger twitches side to side like a tremor. A few day before it started, I was doing heavy barbells curls but was going very heavy. Is is possible to have a tremor caused from lifting weights that were too heavy? I thought maybe a strtched nerve ?

@@tomfletcher2345 speaking in general as I do not know your case in detail through formal clinical history taking and examination (which is what I would primarily advise you to have performed) - the diverse range of tremors that can occur, originate in the central nervous system, most predominately the basal ganglia but other areas such as the cerebellum are sometimes implicated. Hi larneyful, thank you for your question, I am unaware of an association between the photic sneeze reflex (where people sneeze when they look at the sun or have a slit lamp/ophthalmoscope shone in their eye(s)) and ASD. I do, however, recall an interesting poster at the Neuro-Ophthalmology Society of Australia 2017 conference which demonstrated that sustained philtrum pressure can abort this reflex. Most importantly, this condition has the best back-ronym I've heard in medicine: Autosomal Compelling Helio-Ophthalmic Outburst (ACHOO) syndrome! TURN ON SUBTITLES, which provide clarity at times that I was less clear than I had hoped. Now, a few clinical “pearls”: 1. The platysma is innervated by the cervical branch of the facial nerve. It is helpful in cases of Bell's Palsy, as it receives contralateral innervation, similar to the mid and lower face. NB. Don't forget the 5 branches of VII: Ten Zebras Bit My Cheeks - Temporal, Zygomatic, Buccal, Marginal mandibular, Cervical 2. The sternocleidomastoid (SCM) performs contralateral rotation of the head (this is obvious when one thinks about the difficulty seeing the jugular venous pressure if the patient turns their head too forcefully away from the area of neck inspection; however, it has primarily ipsilateral cortical input. This means that a supratentorial lesion causing SCM weakness will affect contralateral head rotation. 3. The Whispered Voice Test has a sensitivity of ~95% and a specificity of ~80% for picking up hearing impairment. Ref: Pirozzo S, Papinczak T, Glasziou P. Whispered voice test for screening for hearing impairment in adults and children: systematic review. Bmj. 2003;327(7421):967. 4. An abnormal “Head Impulse Test” is indicated by the eyes drifting off the target when the head is turned, followed by a corrective saccade back to the original point of fixation. If this occurs then it suggests a defect in the vestibulo-ocular pathway ipsilateral to the to the direction of head rotation. Ref: Halmagyi GM, Curthoys IS. A clinical sign of canal paresis. Arch Neurol, 1988;45:737-9
