The first question and answer about the challenges of staying true to your design and vision while also accommodating players opinions and desires seems like the most challenging aspect of game design if you actually have a hit on your hands. I feel like with almost anything else there are right and wrong ways that you can reference and examples to look towards, but when it comes to this dilemma I can never say that I've seen that there is an objectively better decision to make. I'm watching this happen to Dark and Darker right now, which is a game I love and a development team who I'm very much inspired by. Being in a situation where your game becomes popular and then trying to manage expectations of a devoted playerbase, while also staying true to your vision is truly a perplexing task.

"The amount of work that's necessary for this kind of stuff". Can definitely relate to this Andrew bloke as someone also developing games outside my day job. I've just reworked a reasonably simple menu screen where the player selects, builds, or equips a space ship. I already had a lot of the systems built for it, but it took me about 16hrs to do the new menu. Stuff takes ages!

Where are all the women making games? Are there any? Yeah, I know, there's at least one, and she appears here, but the other 16 where men. Is the disparity really this big?
