Hello Dr. Joe! I am 42 years old and I partially tore my left Achilles (or at least I believe I did) playing basketball around 14 years ago. When I performed the Thomson’s test I did get movement, however the foot was weak as I could not stand on my tippy toes. At the time I did not have insurance so surgery was not an option. Now, at 42, the leg in noticeably smaller than the other and the weakness in still there. So my question is what can I do to be able to stand on my tippy toes and maybe perform a layup in basketball. I still play from time to time and although I can jump for rebounds and jog, I cannot perform layups smoothy or get blocks that I once could.

Unfortunately, I don't know since I can't evaluate you. The above video might help, but if you can, the best option would be to go see a physical therapist for an evaluation to get specific exercises for your level.

Really damaged my achilles about 12 weeks ago , thought I'd give it a month then if no better I'd see a physio , then lockdown happened. Since then I've been in agony , tried everything and anything , and then found this video. Done the calf stretch for 2 days now and can't believe the improvement , the pain's gone and so has the limp, so thank you very much. Nigel

I fully ruptured my AT playing tennis about 15wks ago. After my non-surgical (conservative) treatment my physiotherapist also gave me these exercises Dr Jo beautifully demonstrates. The towel pull (I used wrapped towel) seems to be most effective in my healing mainly because my ankle was so swollen, stiff and painful after 8wks in an airboot. I can now finally walk without a limp & pain, although still hurts achilles if I walk too fast and when its towards end of working week. Recommend that anyone with same injury speaks to physiotherapist if possible. I require tissue work (massage) on my achilles which grew back (bridged) as a lump near inside of leg/ankle - quite common apparently with conservative healing. Gradual exercise over a good length time seems to be best advice. Anyone with a AT rupture stay positive and take your time with your rehab. Good post Dr Jo!

When you say fully ruptured is that the same as a complete tear? I completely tore my Achilles and it wasn’t connected at all anymore. I couldn’t do these exercises because when I flexed my calf my foot wouldn’t point foreward because there was nothing connecting them together. I’m just wondering if we have the same injury or if a full rupture is different then a complete tear. Glad your feeling better! I can’t wait to be at the point of doing physical therapy exercises!

I have been limping for 6 weeks and finally been seen by doctors. Due for an x-ray and physical rehabilitation therapy but this video has been helpful so far. I can't do tip toes quite yet but I'm getting there. Thank you.

This is excellent! I will do these to strengthen the heel that needs surgery before I have surgery to repair the one that's been fixed and ruptured again. The recovery was brutal since I've been so inactive due to the pain and swelling for more than two years. Also gotta practice my one leg squats so I can get up from the toilet LOL. I do wish someone had warned me how difficult that would be!

I had a complete tear on my right Achilles Tendon. I had to have surgery because it wasn’t connected at all. I definitely wasn’t able to do any of these before surgery because obviously it wasn’t connected at all there was nothing to stretch or flex 🤣. But I’m very much looking forward to when I get my cast off so I can start rehabbing and doing these and I can get back to my life!

Thank you for your video, it is clear and simple! I started my rehab following your instructions this week. I injured my left leg playing badminton in mid May. A doctor said I had torn my tendon, so I went to hospital for x-ray and ultrasound checkup. The result is ankle sprain with rupture but no broken bones nor ligament, no surgery needed. It took a month for the swell and bruises on the foot and lower leg to diminish. Now it's the fifth Sunday since I injured, I can walk without pain but my left leg is very weak, I cannot semi point with weight on the ball, thus cannot walk and go down stairs properly. Also, it is obvious that my injured leg is slimmer than the other one. I wonder: 1) slimmer injured leg is normal? Can it be trained back to normal size? 2) cannot stand on the ball is normal for achilles rupture? 3) shall I focus on the training of the injured leg or better do the same exercises on both leg?

You're welcome, and thanks! It is pretty normal for it to be slimmer. That's mainly from you not being able to work that muscle. Same for standing on the ball of your foot. If you can, I would exercise both. Good luck!

I spoke to my surgeon about stretching before surgery and he recommended against doing this as it could make the injury worse. how does stretching a torn tendon help it?

Many people don't have surgery, and this is to help lay down the scar tissue so you don't have so many issues while you heal. I would follow what your surgeon says, but in many cases I've seen, the stronger and more flexible people are before their surgery, the better they do with there recovery.

Hi Doc Jo. I have pain in both Legs achilles tendons. Im only walking, no proper exercise since 2 years back. I have no recollection of any injuries. However the pain makes it hard to walk in general now. My Doc has given me painkillers. I will start doing your streches and see how i get on. Question is is this normal that both legs hurt? When it comes to ankle/achilles strengthening exercises, how often should they be done? How often should I rest? And what is a good sign that I can start increasing intensity.

Thanks for the video. I’m 15 weeks out from achilles repair surgery and am doing the same or similar stretches. No excessive pain while stretching however I do have sharp pain on the bottom of my heel and outer edge of my foot when weight bearing / walking. Is that pain common? Any advice on treatment?

Hey Mike, not sure if you’ve had an answer, but I had the same issue. Foot just wasn’t used to walking and weight bearing so the muscles had wasted away. Stick a towel on the floor and scrunch it up with your toes...worked for me dude.

I frayed my Achilles tendon 23 years ago. I didn't need surgery but the specialist told me never to go on my toes - ever. So, I followed his advice. As a result, if I ever do go on my toes, my calf muscle tightens up terribly. I've torn them a couple of times too. Would doing your exercises - particularly the ankle raise with very slowly lowering yourself - help me after all these years? Or did I wait too long to strengthen my muscle?

So I had surgery about a month ago broken bone an fractured ankle tore my achilles all from a gun shot. I haven’t went to therapy just yet hoping to go soon but I’ve been doing my own therapy in home. Now I can’t apply pressure to my foot yet I can’t walk but I’m needing some insite for make it the ankle stronger an also my achilles. I’ve never had this severe injury before.

Hello. I had rupture of achilles tendon. I also had conservative healing, leg was 10 weeks in airboot. Now, i menage to walk without limps or pain, but i have dorsiflexion. I dont have full motion of foot. Will this excersise help me, and can you give me any other advice? Thank you.

Can you recommend when is the best time to start these exercises after an Achilles rupture when a person is not having surgery? I am predominantly, wearing a walking boot with heel inserts and using crutches, but would like to start doing exercises. I’m just not sure how long I should wait after the rapture happened to begin gentle exercises. Is there an average number of weeks a person can start to do these?

Unfortunately, I don't know that answer since I can't evaluate you. Everyone is different, and it just depends on how big the tear is and how your body reacts, so that would need to be something to ask your doctor.

Yeah this video is wrong for recovery. You shouldn't be moving foot past neutral. Just point it forward.

I rolled my ankle about a month and a half ago landing a sidekick in Taekwondo. I went to the doctor, took and xray and he told me to let it rest a few days and I'm good to go. I kept training with quite a bit of pain. It has gotten better but there is still pain in my ankle when I lift my foot up and pain in my Achilles tendon when I do a calf raise or straighten my foot. Would these exercises benefit me? Or do I need to go and see the doctor again?

Hi there. I ruptured my Achilles yesterday playing basketball. Got a scan and it says “7cm proximal to insertion. The resulting defect due to retraction is about 3.5cm in cross section.” Is this a low/ medium/ high degree injury? Am booked in to speak with a orthopaedic surgeon in 2 days. Dr. Jo, I ruptured my Achilles about 3 weeks ago and my MD is recommending surgery. I really can’t do the surgery option right now because I’m my elderly parents caretaker. Wait so you can fix your Achilles without surgery ? Basically I’m in a dire situation where I’m abroad and the doctors here basically checked my Achilles and said the tendon is still there even though I can’t flex my foot down. But they won’t MRI it and I’m in a race against the clock to fix it or I’ll live my life like this forever. Please help

I had my surgery in January and the 2 major tendons were completely severed. My doctor assigns me different things to do every 2 weeks, but I didn’t have a cast. I got a boot. I’m walking, awkwardly and slowly, but walking one the less. He told me to put your foot into a 90 degree agree, while your foot is dangling over your couch or some pillows, then let your foot flop down into its natural position. He said do this 4 times a day for 10 reps. I’ve been doing that and am more mobile. Granted I can’t walk normally by any means, but it’s still progress. Hope this helps!

Could poor circulation affect recovery? I have ongoing tendonitis in both ankles. I was told several years ago that I had poor circulation in my legs, but my doctors wouldn't recommend any treatment other than to quit running. I would like to keep running, but no matter how much I do these stretches the pain does not go away. The only thing that works is inactivity which unfortunately leads to weight gain and other health issues.

Try swimming, it’s very low impact but the resistance from the water makes it just as much exercise as running. Best way to build up stamina and lung capacity too, a lot of long distance runners use swimming to practice breathing and build endurance

I had both my Achilles lengthened about 15 years ago (I was a toe walker as a child so they never stretched and eventually were too short)… since the surgery I have always been too scared to over stretch them. I’ve recently started horse riding which requires heels down and toes up which I struggle to do… Will these be safe to do please?

Second stretch is what injured me. I was pushing my moms car into the garage. The initial push off is what did it.

Often a quick motion like that is what causes the injury. However, once you are cleared by our doctor for stretches and exercises, it's important to gently go back in to those movements to get your flexibility and strength back.

Hi Dr. Jo I believe I tore my achilles. Went in to have it checked and x-ray.. no rupture and doctor said that it was only a sprain?...I was given a walking boot and have been using that since. It's been about 6 weeks since injury. I already know that some of the exercises in the video I may not be able to do. Would you recommend me doing some of these excercises or wait longer to start?

Unfortunately, I really don't know since I can't evaluate you, but you can not see a rupture or a sprain in an x-ray. Did your doctor recommend physical therapy?

5 weeks ago, i jumped about 15 feets high for a reason. My left foot was unable to move. Xray said no bone had been broken. Ligaments also dont hurt. But the joint position of calcenius and achilles tendon hurts if i press it(Back right of the left foot). Is it a sirious ussue and should i follow these excercises?

Great video. I had surgery for fully ruptured Achilles. I just got out of the splint and into the boot a few days ago. I’ll start doing the stretching but should I be doing the balancing exercises right away too or more massage and stretching ?

Thanks. If you are still in the boot, you should hold off on any standing activities until you are cleared by your surgeon. Good luck!

Today is the 5th week of my post OP achilles tendon surgery.. I have started exercises. Bt why is my back feeling stiffness?? Just above the heel i am feeling stiffness

I am having pain in my achilles nearly 15 months just recently started do i have rupture or more severe than that. I can walk with a bit pain. Do i need to this stretch too ? Thank you

AskDoctorJo Hello. From yesterday I'm in normal shoes (without wedges) still limping a lot. I have a question this morning I noticed that my ankle is very swollen. Is that normal? I dont have a pain , but I was walking quite a lot yesterday. I'm equally 6 weeks post operation

Some swelling is normal at each stage, but you really need to talk with your physical therapist about this. Since they can see you in person, they will have a much better idea of what is going on.

I started running about a month and a half ago, suddenly I started developing this lump in my Achilles tendon which was painful after my runs and would hurt if squashed. I stopped running about a week ago and it’s now painless. Can I return to running at least twice a week?

Hi Doc, when I did these stretching exercises, I had ankle pain from the outside of the foot when walking and especially when going down the stairs. what should I do now?

If you haven't, you should really go see a physical therapist in person. Unfortunately, I don't know what would be best without getting to evaluate you. Goo luck.

Are these really safe for a ruptured tendon? I feel like these would be better rehab exercises during recovery phase. I currently have a ruptured Achilles tendon and I feel that if i tried doing these it would make it worse by tearing any fibers that might be left or cause the muscle to retract and roll up.

It does depend on what stage you are in. If your doctor has cleared you, you can do these. Gentle stretches and exercises won't tear the fibers more. They help prevent the fibers from balling up and causing pain and scar tissue later. Good luck.

I had Achilles' tendon repair surgery 6 weeks ago and just recently able to walk normally with an immobilizing boot on. I'll be starting PT soon, but the doctor made clear that I can't go past a 90 degree angle with my ankle, so basically none of these exercises are ok. I'd advise anyone post-op to ask their own doctor if these are okay before doing them.

Thanks doc jo... may i ask is this good for a person who has leg anthropy? I notice my daughter leg is getting thinner. We consult some doc and theraphist already for a year now but still no improvement in the size of her leg. Irely on the youtube some exercise in leg antrophy. I found your channel... pls help some exercise good for leg antropy. Many thanks again more power.

I have painlot & tendon prt gets redish bluish so should I do this exercises. Please lete know doctor. I can't raise leg on toes asif I do so I lose my balance so guide me

Doc is it ok to do that exercise because im already at 12 weeks post operation of achilles tendon?Is it also ok to walk inside our house (for the tendon to stretch) without the aircast boot? Prior to this im also having my therapy every other day..

Also, I’m 3 months into injury and am trying tho heal naturally without surgery, do you think these exercises are still good for me..... I’m 45, if that matters

Have not been for rupture, I was in pt for fractured ankle (weight barring), which led me to this..... some of these were same in building strength..... I also have that deflated calf..... thank you

I would be very careful with that. There are reasons why you have to let your body heal for a certain amount of time. You could re-rupture it if you don't give the anchors time to heal to the body (even if you are feeling good. If you haven't started physical therapy, I would recommend going. They can assess you and help you progress safely. Good luck.

Hey I been In a cast for 6weeks natural healing. now I'm in a boot for about 5 day but I walk around without the boot is that safe. I have not started physically therapy yet but I can stand on it and everything. But it is still very tight so I can't do a full calf raise. Should I stick to the boot or am I find without it?

Hi doctor jo, I got pain in muscle which is backside of the hill but not in tendon. Its like both side of the tendon just little up from the hill. It has happened while running then I took rest for 5 days but still I did not get well. Could you tell me if I do these exercise how long will it take to get well.

It really depends on what kind of surgery, and if you are cleared by your surgeon to start stretches and exercises.

With a non-surgical Rupture. My injury snapped away from the calf muscle. How long would you think that I’m free from rerupure and don’t have worry, to do sports?

It really depends on a number of things, like were you or are you in a boot? Did you have physical therapy for it? How does it feel now? It is different for everyone.

Thanks for the reply! I was injured on the 23rd of September the cast wasn’t put on correctly in the emergency room. So when I was seen by the consultant 3days later the correct cast was put on so I would say the healing started from then. The Physio started this month on the 6th with the boot advised to stay on until this Tuesday the 19th I was told to take the boot off and use my normal shoes. I’ve been doing these exercising and still feel pain and discomfort but I would imagine this is part of the process... can you give any indication of how long more. Until I’m 100% Thanks

Thank you very much, I have a question about my surgery. After my 2nd and 3 rd extensor digitorum brevis tendons rupture operation. Can I kick sandbag or any kickboxing competition, I am wondering can I kick with the fixed tendons area anymore.

Thank you! I was getting ready for football (American) and they said I couldn’t play if it was still injured. This helped a LOT! I did this every day, and in about a month, I was back to normal. This was really helpful and Jo is a great advisor. Thanks!

In 5th grade i tore my achilles and ever since then my left ankle hasnt been able to be put in a 90 degree angle (very contracted) and it makes walking quite hard.

Brother can you do calf raises ???????....Coz I am not able to do even one. While walking, I am not able to push throught the ball of the foot. Hence getting hypertension on knee.
