And on memorial day also have some thoughts for the many war reporters - members of the free press - that embedded (as it's now called) or not, reported in field conditions sharing many of the same dangers in real time and afterwards so that in concert with our veterans own stories we can appreciate their hard given gift to their time and their projected futures that many did not get to experience or obtain a full reward within. I had classmates from my university that worked at the airport. One of the people there knew the guy, apparently he was a fresh CPL with a bit of an ego and not a whole lot of instrument experience. Bad combo.

Great video Juan, as usual. If I'm not mistaken, I believe his destination was PBI. First thing that stuck out to me, was his very unprofessional radio skills. Sounds to me like he never should have been on that cockpit in the first place. I don't understand what his wife was thinking, or the rest of his family for that matter. Looking forward to future updates.

Not strange at all. This was not a *_PROFESSIONAL_* career pilot. This was one of those idiots with too much money and not enough brains! One of those businessman, lawyer, doctor, athlete, "insert your choice of celebrity", .........who thinks just because he has bags and bags of money, should be able to jump into a complex machine like this and "drive it" like they bought a Ferrari at a local dealership (rollseyes). If I counted the number of accidents I've seen involving THIS EXACT scenario (and by that I mean "rich-idiot flies his own plane" scenario), and got a dime each time, I would be a millionaire by now! (rollseyes).

Just read this: “According to FAA records, the 1982 model Citation has been registered to JL&GL Productions of Brentwood, Tennessee, since March last year. FAA records also show that no one aboard was qualified to fly the Citation. The agency's airman database shows Joe Lara having a multi-engine commercial pilot certificate and instrument rating but no record of a current medical. Another occupant held a private pilot certificate with multi-engine and instrument ratings.”

They were fighting for the U.S. as they were looking to colonize parts for the French so the English would not get it. Look at all the other countries they colonized. Louisiana purchase was from France then they wanted returned. Lewis was killed by them, he did go by suicide.

I am type rated in the Citation series. Flight Safety teaches that single pilot you should engage the autopilot to allow you to monitor the airplane better in those high load times such as right after departure in busy traffic or heavy weather.

I’m not typed on the CE....do these models have “Auto-Throttles”? And..you are absolutely right. Single pilot, in the weather, USE the Automation. Did the “Automation” have a failure here which lead to the “chain of events.” I can’t make an accurate judgement with no time in type. What do you think?

@@rodcoulter997 no sir it doesn’t have auto throttle. There is a possibility that the autopilot might have failed hope fully that information will be uncovered during the investigation. The other question in my mind would be the level of Aviation education of-the pilot in command. Was he unsure of how to use the functions of the autopilot? The very viable questions have been raised about the pic’s instrument currency? The CE501 is. actually a relatively easy airplane to fly and that can lead to complacency.

Well, the only person on the plane that was type rated for the Citation was Joe Lara but his license was invalid from his medical certification being expired (last updated in 2017). There was another person, Brandon Hannah, on the aircraft who had a valid license but wasn't type rated on the Citation according to FAA records. To me, it sounds like the pilot knew how to fly a plane, just not that plane.

@@roji556 To me it looks like as if the one pilot (Mr. Hannah) knew how to fly a plane, just not that plane - and that the other pilot (Mr. Lara) knew at least in the past how to fly this plane but was for years out of flying business (given that he had no valid medical certification and the last update in 2017) and was therefore unable to manage the situation. So to me there´re a couple of open questions in this case: Was Mr. Lara the PIC? Was he a single flying pilot or sat Mr. Hannah with him in the cockpit? Do we hear Mr. Lara on the radio? And why was it possible that an airplane departed the airport with no pilot with a valid license on board? Especially the last question is important if we want to learn lessons from this crash. I hope that the Preliminary Report will bring us at least a step forward to answers.

I am a physician and pilot. Practice, practice, and practice more. Practice emergencies and practice more. Mother Nature loves complacency. I am a trauma anesthesiologist. Just like flying, hours of boredom interrupted of moments of terror. Plan A, Plan B, Plan C, PlanD, etc! Induction of anesthesia and waking up patient two most dangerous times. Just like takeoffs and landings are very critical to flight. Either profession not very forgiving if things go wrong. You be safe. ER’s/OR’s are not happy places to be. Very costly to health and the pocketbook

Woodring, 2nd Lt, U.S. Army Air Corps, a "Mustang" as he had come through the enlisted ranks, KIA in France on July 10, 1944. He was one of the "old men" in his squadron at 22 years of age.

RIP Lt. Woodring. I lost an uncle late in the war, too, who was a bombardier for the US Army Air Corps. His Liberator broke up as a result of the aircraft debris above it got shot down in 1945. His remains were finally located and returned to our family in 1948. RIP 2Lt. Robert Lanoice Stedman, all those who gave their lives, as well as all the MIA, lest we forget.

I flew into MQY about an hour after that accident for a fuel stop in an Eclipse. It was 1,200 ft ceiling and smooth air as I did vectors for the ILS 32. Nothing too challenging. They warned me of the mower as well! On the way out I took off on 01 with the same vectors. Not very noticeable winds aloft at 3,000 ft and below. Thanks for the analysis as always.

One of the big problems in early Citations was the Artificial Horizon required a long time to spin up the gyros prior to flight in order to become effective. Several years ago, another Citation crashed shortly after takeoff going into IMC because the pilot didn't allow enough time before takeoff for the AH to "uncage." It would interesting to know how soon after engine start this aircraft went airborne.

Juan, The airport is actually just Smyrna, Tennesse (KMQY), NOT “NEW” Smyrna Airport as you continuously refer to it. I believe that you may be confusing the name of THIS airport with that of the New Smyrna Beach Airport (KEVB) in Florida, which is just south of Daytona Beach International Airport (KDAB).

Every pilot should go in the FAA somatogravic simulator at Oshkosh, or wherever they have it. Understand how intense the sensation can be, so a pilot can disregard it, and fly the instruments.

Until I watched Scott Perdue of Fly By Wire channel do a flight using a tennis ball hanging on a string, I just did not understand it. I was truly amazed to see that tennis ball barely move as he banked, rolled, and inverted. He used physics to illustrate as he explained the laws of motion and though I’m not a pilot, it was interesting and gave me yet another reason to admire and respect pilots, as if I needed any more.

When I was a low time pilot I got myself into similar situation and knew I was in trouble. Fortunately I had auto pilot to get me out. Always felt bad about having to use the auto pilot, not anymore.

Will be curious to see what happened in this one (in a year or whenever). I've taken off in a Citation into night black-on-black (technically VMC, but no reference to a horizon with the deck angle after takeoff) for a return to service flight with a failed primary gyro roll input to the left seat's AI that went unnoticed for several seconds. I was monitoring on the right side and was wondering wtf the pilot was doing with the crazy roll inputs and about to take over when he finally got an external visual reference and realized what was happening. We terminated the flight without issue. You can't always trust your instruments and, in this case, it would've screwed the AP as well since it took roll data from the same faulty gyro. After the flight, we realized the connector between the gyro and the bulkhead had a bent pin for the roll data. This left the AI without any flags - it happily assumed the roll was at zero degrees and displayed only pitch. Without a second pilot and if it was actual IMC, this flight likely would've ended up cratering and, depending on the state of that connector in the wreckage, resulted in a "pilot failed to maintain control" PC.

Juan, you always blow me away. Insightful accurate analysis without inappropriate emotion and fear selling of the masses. I think you are probably spot on in the suggestion of a pilot disorientation. That turning and climbing entering a cloud layer has disoriented all of us a time or 10. You have to really maximize your efforts on setting that adi perfect and doing whatever it takes to hold it there. Even rolling out would have helped this pilot immensely. Pitch and power in a slight climb until your head clears out. Such a huge responsibility for pilots to take people up in the air that don’t fully understand what is happening. Don’t take these responsibilities lightly. Stay proficient and know your own personal limits and abide by them. Use strategies to sequence things and keep the workload down. Never forget that your passengers are counting on your skill, not your ego.

If anyone is interested in turbine engine igniters, AgentJayZ just did a video with a nice hands-on demo of one working on the bench. Very much like a multi-thousand-dollar barbecue igniter. Experienced or not, current or not, anyone can get "the spins" entering IMC, so anticipating that problems can arise when entering IMC seems like a good idea - being prepped and ready to engage the autopilot sounds like a good idea.

My wife heard his plane when it was making its turn off the departure. It crossed about half a mile from my house. There have been unconfirmed reports of an explosion before he hit the water.

Juan spot on as always. There was a second pilot (Brandon Hannah) on board, but not C500 type rated. The radio voice from 66BK sounds like a younger voice. Just a hunch. I’ve sent your video to one of the Nashville TV stations. Your 11 minutes has more good information than all the other coverage combined. There are relatively recent pictures of Mr and Mrs Lara in a Hawk XP. It’s a pretty big leap to a Citation. I have a friend who flies a CJ for a company. He said the insurance for single pilot was significantly more than the cost of a second pilot, and he’s extremely experienced.

I’ve thought about that. I’ve come to the conclusion that when hitting a sudden bank of fog that you don’t come out of pretty quickly is better because you can at least slow up, turn flashers on, take the next exit and actually pull off somewhere and stop and wait it out, if possible. We don’t have that luxury in a plane, you have to keep going and fly the plane while assessing and evaluating everything else. I’m not a fan of either scenario!

I was amazed to read that it happened to that pair. They own a gigantic church off of I-65 and Concord Rd in Brentwood. The church seizes all of your assets and they put you up in shared houses with other families, with the church's advertised dogma being healthy eating and weight loss. With how much money they rake in, I'm amazed they didn't have a newer airplane.

Thank you for bringing out the point of turning on the autopilot. When I learned to fly the autopilot was poo-pooed by instructors as something a REAL pilot never used. Over the years many lives would have been saved by just turning it on. I hope training has changed by now.

autopilots are not infallible as I have had one almost roll a 727 when it was switched on. So it stlll comes back to good cockpit resource management, being ahead of the airplane, and not flying into conditions that are beyond your skill set. And you had some bad instructors if they thought autopilots are never used.

Apparently his medical was not current, by many years. He married a wealthy woman who bought this old Citation 501SP. You can get them for as little as $250,000, depending on engine time remaining. I have a CE500 Type and +300 hours in Citation I and V. It was my first jet. I loved flying them. It was so easy to fly. An old citation is better than an old piston twin. Yes they are slow jets but fly high and faster than piston and most turboprops. *It is possible it had instrument failure? I don't know what flight instruments it had. I wouldn't be surprised if it still had original 40 year old mechanical/electrical flight instruments.*

@@Matt-zc1qs I don't trust the altitude on mine implicitly, but for everything else it's far more reactive than the steam gauges in some of the aircraft I fly in. Better than a G1000 or even the single gauge retrofit glass gauges? No. but reasonable enough. Check how you have yours mounted as that does make a difference.

Smyrna’s airport is the busiest civil aviation airport in the state and home to The Great TN Airshow (50 years this coming weekend). Most recent update that I saw today is that a barge and crane have made it out to the crash site, might be seeing a recovery effort soon of a larger section of the airframe.

Growing up and living in Nashville through my 20s, Percy Priest Lake area was our stomping grounds. It’s bad enough to hear of such a tragedy, but even more so when you have any sort of connection to the area in which it happened. I sold Mac tools years ago, and one of my stops was this airport. It's also used by the Reserve base there. They worked on the Blackhawks, and I would spend too much time there just looking at the different aircraft. The coolest thing I ever saw was around 6 Blackhawks taking off. They were in a row and hovered for a while then just like in the movies. they formed a line following the leader to the runway and took off heading down the runway. The hair stood up on my arms.

we saw that same sort of crash with N610ED. The pilot of the 610 didn't have a functioning autopilot. What was said was the owner of the plane was broke and alot of things didn't work, but the pilot took off anyway. maybe the second pilot would have helped. you hate seeing this happen, if there was anywhere ego should be put aside, it seems like the air would be the place. Especially in light of the living history crashes

That first one where Nashville departure checks with him, he did reach out to departure. That's not in this audio but if you download it from liveatc. He switches and calls nashville departure and nashville is talking to someone else and this was them getting back to him.

Professor, here is one for you. When the L1011 was first being test flown, the FMS (flight management computer) at altitude, the FMS flew past the TOD (top of descent) where the engineers thought it should be. Well, the the airplane got over the the waypoint, the computer tried to point nosed the airplane straight down. The FMS had computed that it was most economical to stay at altitude then go strait down. The computer was reprogrammed.

I find it assuring that professional commercial pilots are almost always partisan to those who fly non commercial planes and will know exactly who is obeying the rules and who are not. (I am waiting to hear how Mr. KLAS ended up after busting that damn Bravo and was not polite to either the ATC or the local SWA inbounds).

@@KCautodoctor The local news is wrong. They probably used a wrong name to search for the information (There is a unrelated Bill Lara. The pilot in question is William Lara). He had a type rating for the aircraft but his medical had expired.

With his speed and oscillation in altitude, I would expect a failure in the control of the automation system coupled with spatial disorientation. That pilot was _stressed_ .

Yeah, agreed. I doubt it was just a simple spatial disorientation. Probably an automation failure of some sort and it caught the pilot way off guard in imc.

In April '97, a headhunter representing FedEx Express called my home in Nashville one night and 20 minutes later I was hired over the phone to implement a Comprehensive Fix to a VDR concerning overflown compressor wheels. I spent the first 30 days working that problem and then was put on the road buying MD-11's for the company. Then, in '02 was appointed an FAA Liaison representing FedEx to FAA. Being constantly homesick for Nashville, I subscribed to both the Memphis & Nashville Business Journals. One morning in 2008, there was a headline article in the Nashville Journal quoting the New York Times as having just called Nashville "the new 'it' city." Nashville has never been the same. We've lost our lovely community to all the transplants coming here. 'Nuff said. Moved to Bedford Country 3 years ago and have not been happier since returning home in 2015.

Ryanair itself is a pretty shady out fit, the Belarus government probably had some one on the inside to let them know when the target was aboard and paid Ryanair to land there. This! Non pilots tend to be so impressed with anyone who knows how to operate an airplane that they think the pilot must be highly skilled. There really needs to be required FAA safety seminars for passengers of GA airplanes, or family members of airplane owners.

@@jiyushugi1085 Kinda sorta - then there are people like me. I have never flown in a private aircraft, or anything that isn't a scheduled service, and never will. Our neighbor invited me and my young daughter to fly in her and her husband's plane, however I politely but firmly declined her offer. A few months later while our neighbor was on a training flight to obtain her own license, she crashed (rolled) the plane on landing. She and the instructor were lucky to walk away as the wreckage was a ball of scrap, but she did have to have reconstructive surgery on her face as she had several broken bones, including her eye socket. I have no doubt that private aviation is fun, and safe _most_ of the time, but when it stops being safe it is too often "curtains" for my liking.

You can do a "go around" at any stage of flight not just on short finals. Go wings level and climb out, if you're disorientated, why not? Make your apologies to ATC later.

Yep, fly the airplane. When ATC says to do something, report "Unable". It's going to make departure mad, but better to fly straight and level, get control, then make the turn.

"Wings level" in IMC is easier said than done if you're not proficient. The first time I flew under the hood my inner ears said "left bank" while the flight instruments said "straight and level".

Seems like it might boil down to single pilot, IMC in a fast plane with a low redline...pilot not fully prepared? There must have been something that came up soon after takeoff that threw his whole plan off the timeline. Maybe he forgot a checklist item or to set an instrument, and in playing catch-up with that got behind the aircraft AND throw in spatial disorientation...he maybe lost track of the attitude and airspeed while in a turn? Could he have pulled the yoke back too hard at that 300 knots or so? Would overspeed result in wing or stabilizer flutter? But I’d think there’d be much more room for error...something like 330 kts, no? Also, was this aircraft somewhere around 40 years old? Maybe not so forgiving of overspeed anymore? I was thinking the other day, seems like this is going to be bad year for general aviation. Fuel prices are bound to rise and following the pandemic hibernation, lots of pilots not current. Maybe there should be a proficiency training drive going on. Maybe the FAA and aircraft manufacturers and air safety organizations should be emphasizing “currency, proficiency...train, train, train.” Or something like that. Although, maybe they already are.

I should clarify my question. Does a CFI need to eyeball the medical document of his student before he signs off on the student? In other words, if the student fails to provide his CFI with proof of medical “fit to fly” clearance, is that reason for the CFI to withhold the FAA application for licensure until or unless the student complies?

David i am not a joyride pilot but i would assume that you wont be able to file an insurance claim on this one. I would expect that adjusters take pilot error and unlawful certificates (like medical) seriously.

Great video, I got my pilot's license flying out of the Smyrna Airport years ago. A great private/public airport that used to be an AAC and AFB so it has long runways. Priest Lake is basically all that separates it from Nashville International hence the turn and climb and maintain 3000 until the hand off to BNA out of runway 32. Sadly, do not think the pilot was experienced enough in the Jet and Conditions, will be interesting to see how many flight hours he had total, not to mention in a high performance aircraft. Sad, really sad.

great report. I would think the first thing one would do is rely on the horizontal gauge and the airspeed gauge and altitude gauge , granted I'm no pilot, just person that love's aviation and only Fly's R/C aircraft

Eyes must be on the panel before entering IMC. You've got to acclimate to panel flying very early. No sight seeing while in the clouds unless you have turned on Auto pilot.

Definitely a task saturated pilot by the way he sounded on the radio. I’m thinking the same thing...spatial disorientation. It’s either that, or maybe an attitude indicator that went bad and he failed to recognize it. If he had an attitude based autopilot system, that could’ve prevented the autopilot from being engaged. He could have been confused by the instruments, tried to activate the autopilot, found that it wouldn’t engage, and then lost control of the aircraft when he couldn’t figure it out. Fear also plays a big part and can keep us from seeing the obvious when things go wrong. May the pilot and passengers Rest In Peace. I love your channel, and thanks for honoring our fallen soldiers, and thank you for your service as well.

Looks like pilot was not proficient for ifr. It is so fast, how those ifr skills decorate. PIC have all the rights to cancel the flight if he not feels good about it. My condolences to all family members. Thanks you Juan for great report.

It's hard to believe how a pilot could be current on this type of aircraft yet not proficient in IFR. I wonder what will come to light. I personally am not ready to throw the pilot under the bus here but if it turns out that pilot error was a significant factor I wonder what kind of training/oversight "swiss cheese layers" had to line up this time to allow this to happen.

Way to early to speculate. The reviewer mentions the pilot's currency several times however I don't believe that information is know. Could have been an equipment failure. Plane was pretty old.

Joe Lara has a C500 type rating but not a single pilot endorsement. There was another pilot on board, private pilot MEL with Instrument Rating.

Can a pilot get a right seat rating/endorsement in the Citation? The SIC had spent 8 hours in the Citation the day before. Was that for a type rating for PIC or just right seat?

@@keithvick1432 There is not Right seat endorsement. An SIC type rating in a C500 would be required if it was an international operation. Because this was a domestic Part 91 flight the right seat pilot only needs to be appropriately rated, in this case Muti-Engine Land and instrument rated.

Vne speed on a Cessna 501 appears to be 355 knots or 409 mph. Yes he exceeded the 250 max below 10,000 ft but as far as I can tell it was well within its max speed. Before I learned about low IMC conditions I was sure that it was suicide. Please verify the max Vne. I found several listing the 400MPH. Thanks

My company SOP is to engage the autopilot at 1000ft AAL after departure. In a single crew operation I definitely would not be practicing my manual handling.

Great breakdown. I’m a student pilot. I am from that area. My grandfather worked at that airport back when it was Sewart AFB. Been fishing many, many times where the plane went down. Was home for a visit when it happened. It’s very disconcerting. There, but for the grace of God, go I. Just poking at you, brother…but everyone from “SmurNa” wincing every time you say “SmeerNa”. ;)

The message received by Gwen’s daughter, whose husband was on the plane, might give a clue to what happened here. The message she reported on initially said that they had a problem and were going to do a controlled landing and that they were ok. From that I’m assuming ( and I know, one should never assume), that they had encountered a problem and were intending to attempt to ditch on the lake. Unfortunately it takes a lot of experience to be able to do that, and even then it is risky..but I believe that is what happened.

@@StevieWonder737 yes, you can look up Elizabeth Shamblin’s initial message to the people of Gwen’s church. It is somewhat bizarre, I never knew that Gwen Shamblin existed until two weeks before the accident, when another KZitemr sent me down a rabbit hole. It’s actually left me quite shaken.
