Been on finasteride for almost 7 months now, started back in March! Watched one of Kevin’s videos and finally got over the statistically irrational fear of growing man boobies; or ya know, dying or some BS, like the PFS boards would have you believe will happen 100% of the time. Getting on finasteride was one of the best decisions I’ve ever made, my hair is so much more thick in just 7 months, no sides. Thank you Mr. Mann! Hello, thank you so much for another informative and very well explained video. That thing with the wasting of precious time in the treatment of alopecia thinking that inflammation is the cause happened to me too. I wasted about three to four months total, time when my hair was getting bed instead of at least stabilizing, because I thought a so called inflammatory diet was the key to solve all my problems, like some KZitemrs said. Of course it’s not a bad a thing that I changed my diet and now I eat more healthy, but for sure I couldn’t relay on just a diet to cure the androgenetic alopecia. I started searching for videos about hair loss on KZitem and instead of watching first what actual specialists have to say, I wasted time with shitty videos until from one video to another I got to the channel “The Hairloss Show” and then some guy there in the comments recommended your channel and thanks to him I ended up here. For about three months I am on proper medication and I begin to see some stability. Also it’s very encouraging what you say about sheddings, not to panic and keep on with the treatments. I tend to be freaked out anytime I lose more hairs in the shower than at the previous washing. Thanks again for your very good videos on hairloss.

What happened to me was ridiculous. Got diagnosed with AGA in 2018 from the nurse practitioner at a dermatologist's office after a ten second inspection of the problem. Spent subsequent years with periods of stability, and then major sheds. I have diffuse thinning. In the midst of my current shed, I went to a different derm, and the NP immediately took a scalp biopsy. I've been diagnosed with Alopecia Areata Incognita and prescribed a double-dose of Allegra daily, oral steroids 2X a week, and a steroid/Vitamin D foam. I was told to also continue with Minoxidil and Finasteride even though there is no indication of AGA (the sample was taken from the back of my head). I am so glad I did not shell out big bucks for a laser helmet, cause that was going to be my next move.

@@robertdipietro991 all other conditions were ruled out by either the pathologist or the derm. I also had other signs of AA like white hairs that would pick up color over time, patchy areas of hair loss on the parietal scalp, and extreme sheds with regrowth. I passed a pull test so no TE. Thyroid levels are fine. In the biopsy report, the hairs were terminal and there was inflammation around the hair bulbs.

@@Jonathan-gy5hk No, no shedding at all. Hairloss just stopped on fin. I did start scalp massages in 2021, which gave me some shedding after 4 months, but that hair grew back after about 2 months after that and I have been fine after since!

Very intriguing topic. In my personal experience, being a guy plagued by Norwood and atopic dermatitis reapers, when the inflammation flares up, I keep losing hair despite using finasteride. Once it is calmed down hair comes back.

Exactly the same here, I have not used finasteride but as soon as I start using corticosteroids to treat my skin issues, hair loss stops completely. I have experienced it multiple times when restarting treatments, and my dermatologist says that inflammation could be linked in my case, but corticosteroids have to be used very carefully :(

I've been doing everything slow and steady, started at 0.25mg of fin every other day, and am now up to 0.75mg eod. No sides, and my 3-monthly hormone panels are looking great! Increased test with only a maginal increase of Oest. I tried jumping in head first at 1mg a day and got sensitive nipples, so my belief is that for most, they can avoid sides by letting their body acclimatise to the changes in hormones slowly by upping the dose every 4-8 weeks. I havent had a whole lot of regrowth yet, but that may come with higher doses, but I have noticed a dramatic decrease in hair fall, and my "strong" hairs are lessy frissy, darker and look thicker. I am also starting (topical) Minox at 1 dose a day and derma-rolling 1x a week, which may help with regrowth. Thank you Kevin for helping to educate us on hair loss treatment and how to do it safely :)

@@redaar1713 All good so far. I use 0.5mg ever other day. No sides. (I got some on higher doses, so this is the sweet spot for me) Hair loss has been halted and even had a little regrowth (although derma-rolling and minox helped more with the regrowth).

@@redaar1713 Lower the dose. 3 weeks seems very fast to develop breast tissue. I would advise working out and losing weight if your body is so susceptible to oestrogen. I'm not a doctor though, you should consult whoever prescribed you the medication.

Really appreciate all you do Kevin. Very reliable and in depth-analysis of everything. I want, as a fellow medical professional, correct you on one thing. At 2:49 you talk about how prostaglandin increases the nerve's sensitivity to pain. This is incorrect. Pain is not something that is picked up by nerve cells. Instead, what happens is that nociceptors become more sensitized and fire quicker. Whether this is painful or not is ultimately decided by brain output. Obviously, this will often happen because, as you rightly point out, the area should not be touched if it wants to heal properly.

Thanks for the correction. This is one of the risks of trying to simplify complex biological events for the purpose of creating a digestible KZitem video. However I think the basic idea is that inflammation is painful and that's a useful protective mechanism in the healing process.

I laughed out loud when I heard the word "Nutraceutical"... Interesting video, as always. Regarding Ketoconazole, I came across a study a while back that suggested Piroctone Olamine may have comparable effects on hair growth. Not that either are worth relying on to combat hair loss. Piroctone Olamine is a common ingredient in Anti-Dadnruff shampoos here in Europe, as I believe the E.U. banned the use of Pyrithione Zinc in cosmetic products a few years ago.

I have gut inflamation (ulcerative colitis), I try to address inflammation since protein, vitamins and stuffs are all absorbed there. OF COURSE, minoxidil, finasteride are the main element... but I don't ignore my guts as a secondary element.

I will say from low calorie diets or overtraining/inflammation like over a month, my hair can get kinda thin(this was before Fin), and maybe fall out, I think that's valid though, once calories back up or overtraining reduced, hair returns thickness

Hello Kevin, I hope you are having a prime week ! Good video as always, you are my favorite source of hairloss inforamations on KZitem ! But I am surprised you didn't really talked about inflamations which really causes hairloss besides alopecia aerata such as lichen planus pilaris, frontal fribrosing alopecia or Quinquaud's follicilitis. Those aren't androgenic alopecia but still cause hairloss and in an even more cruel way because those are scarring alopecia which means that once a hair is destroyed by them it is replaced with fibrosis and it cannot regrow... When I showed my dermatologist some thinning in the sides of my head he said that the in the worst case scenario it could be frontal fibrosing alopecia. We did a biopsy because I was affraid of that but the result from the laboratory went negative for lichen planus (involved in frontal fibrosing alopecia) but the laboratory wrote that it was a slightly inflamatory androgeninc alopecia because they found some lymphocyte trace. I don't know what they mean by "Slightly inflamatory androgenic alopecia". Maybe those lymphocytes are there due to my seborrheic dermatitis. Do you have any idea on how to actually use 2.5 mg per day of dutatsteride ? The only available capsule are 0.5 mg so if one swallow 5 capsules a day one should finish the prescription 5 times sooner... Let say you have a one year prescription, you will kill it in less than 3 monthes 😅

I probably should have made it clearer, but the video was really about the role of inflammation only in androgenic alopecia. There are a number of infections that can affect the hair and cause inflammation, but that was not the topic of the video. Maybe that's something I could tackle later on. I think the lymphocytic infiltrates are very nonspecific, but they are found in many people with AGA. They are probably just a sign of the low-grade inflammation that often accompanies AGA, as I pointed out in the video. This inflammation though seems likely to be just secondary to the AGA process and not independent of it. Regarding 2.5 mg of dutasteride, unfortunately unless your can get a pharmacist to compound it, you are stuck taking 5 capsules of 0.5 mg daily. You'd need to actually get a prescription from a dermatologist to cover it.

My dermatitis got a lot better from drinking this smoothie I found in an old health book from the 90s. Basically it’s 3 raw eggs and a decent amount of raw tomatoes blended together. Apparently dermatitis is is a detox of the lymph and the skin.

and even if people are actually losing on finsasteride or dutasteride which rarely happens, its not cause of a another reason unrelated to dht, rather they are extremely sensitive and the finasteride and dusteride only block 40% and 50% of scalp dht. Its rather cause of the dht left in the scalp.

That's a good point. We know this because people born with a genetic deficiency in 5AR never lose hair. If you are losing hair on a 5AR inhibitor you need a stronger 5AR inhibitor.

I think that inflammation may have a role in hairloss, however I believe it is completely different to typical MPB and also extremely rare. That being said, if you’re taking care of your DHT and still experiencing hairloss then having an autoimmune condition isn’t exactly far fetched in todays world of shitty food, high stress and sedentary lifestyles. I read a great book by Dr William Davies called super gut which talks about some of these issues. Although I don’t agree with all of it, I think is some strong evidence that due to the recent change in our diet (highly processed foods, mass farming and pesticides) gut issues could be a real thing and compromise certain peoples immune systems. If you’re a healthy person without any prior medical conditions like eczema, celiac disease, lupus etc then the Big 3 is more than likely going to work. That being said, if you have a underlying issue or a history of heavy antibiotic use and poor diet, then perhaps this is something to look into. It could also be a reason as to why people get “worse” when using 5ar inhibitors. Hairloss progressing when you’re suppressing DHT doesn’t make any sense. However, if the hormonal levels in the body were already unbalanced and the immune system is compromised, disrupting it even further may make the underlying condition worse.

Yeah I am always confused about inflammation and what it even is in terms of hair loss. I hear the term chucked around a lot but no one actually explains it in detail what it is. I suffer with an itchy scalp and dandruff and when I don’t use a dandruff shampoo or a Cetirizine tablet I can definitely notice a difference in my scalp health. Also, when I eat greasy food and alcohol my scalp always itches like crazy. I think there’s a small link there but it’s literally so hard to study to know for sure.

I agree ! I would feel safer with a stronger blocker but I don't see how to do so... Getting a prescription for dutasteride against hairloss is a complicated mission and 0.5 miligram of dutasteride only blocks about 50 percents. For arroung 75/80 percents you need 2.5 miligrams of dutasteride which I don't know how to do in real life as there are no 2.5 miligrams capsule

I want to know how to get them if they do somehow exist. And taking 5 pills a day just isn’t doable. Price would be nuts. If I could get my hands on 2.5mh dut that would for sure tell the Norwood reaper to fuck off. I’d keep my hair till the day I die.

40 percent scalp DHT suppression doesn't seem significant but the clinical trials and follow up data all show that finasteride stops hair loss in the vast majority of people, including follow up data that goes up to 10 years that shows that 90 percent of patients still had gains. 2.5 MG dut is the strongest safe 5AR treatment option but it is really only for people with the most extreme androgenic alopecia.

Are you even having genetic hairloss? If not then finasteride or dutasteride isn’t going to anything for you. You could just as well have a different hairloss condition. Do you have a very itchy scalp? So yes then you should probably treat inflammation. Just because its not a part of aga doesn’t mean it doesn’t prevent hair regrowth.

Hey, unfortunately yes it was my dermatologist who connected the dots and noticed it. She seen me 1 month prior to my break up (and a good few times prior to that) and during my visit under 2 months after my break up she was shocked at the rate of increase regarding my hair miniaturisation. She did a full mental and physical evaluation and the only thing that changed was the break up during that period. I didn’t have any increased shedding, just miniaturisation according to her. I would have understood if it was shedding as you can chalk that up to Telogen Effluvium but I didn’t know a stressful event could have this effect on AGA. Thankfully that significantly accelerated rate of miniaturisation has started to slow since August, which is also when I started to mentally get over the break up. My dermatologist said she’s seen this before but a shedding period is significantly more common than increased miniaturisation. In summary, it sucked! Had I of known potentially a cortisol blocker would have helped but no point crying over spilt milk as they say.

Bro dedinitely use 2% ketoconazole. I need to keep using it othereise it comes back. What I do notice tho is takinh 3000mg NAC daily and doing the Wim Hof method makes it much less aggressive so I would definitely give that a try as well.

Hey, i my hair started thinning about 10 to 11 months ago on the middle of my head and then my hairline also started to recede more and more every week a noticed more thinning or receding at my scalp and it didn't stop now. I lost probably 60 to 70 of percent od hair on my scalp. My father is in his mid 60s and he has 40 percent hair on his head and my brother is 40 and he also has about 60 percent hair that is not thinning. But he doesn't have a bald spot. I am trying nizoral and i am going to see a doctor as fast as possible so i can talk about which things i should test in my bloodwork to see if i am sensitive for side effects of finasteride and i hope that im one of the majority that isn't sensitive to them. I got told by a doctor that inflammation can increase the strength of male pattern baldness. I know this video says that its not true, but i am so shocked that my hair is falling out so fast, do cases exist were people with aga went bald in 1 - 2 years ? And if it would be true that inflammation speeds up hairloss, would it also speed up the dying off of the hairfolicles. Because i am looking for a treatment to regrow my bald spots without minoxidil because i did read that it decreases in efficiency after 5 to 10 years and that it isn't a help forever. What do you think about microneedling with topical finasteride, but i guess it is going more systemic this way right ? This is what my doctor prescribed me (5mg) - I'm just curious, I've looked everywhere and nobody seems to take the 2.5g, doesn't it exist in pharmacies? Does the company make it in a larger dose than the 5mg? 2.5g could become interesting if a person tolerates the 5mg very well.

5 MG of dutasteride? That is 10 capsules of avodart per day. Are you sure he didn't mean finasteride? People will often quarter 5 MG finasteride pills into 1.25 MG pieces to take daily, I do this myself. The highest known dose of dutasteride that can be taken daily is 2.5 MG.

Must be the hairspray. Yeah, calcification is basically just the final stage of androgenic alopecia. It happens after fibrosis when the hair follicle has long died. It isn't a cause of hair loss, it is a consequence of it.

Your skin looks great, I need to do whatever you're doing. I use finasteride and Nizoral every day and my scalp itches. I think I need dutasteride and oral minoxidil. Plus a hair transplant with verteporfin. I also seem to have lost spots of hair in my facial hair. I think I have some kind of autoimmune condition. My immune system responded too aggressively to the covid vaccine as well and I had to be put on prednisone to try to prevent Bell's Palsy and inflammation pain in joints and loss of feeling in extremities.

Sounds like something your dermatologist could diagnose, if you think you have an auto-immune condition causing your hair loss. The scalp itching could actually be a sign of some kind of infection, or maybe severe seborrheic dermatitis. Your dermatologist could prescribe something more powerful than Nizoral for that.

Another excellent video Kevin. Kevin, does it matter if I use Garnier Fructis Stemoxydine or Serioxyl? Both have 5% Stemoxydine. Right now, I am using Serioxyl and mix it with alfatradiol in morning and with Minoxidil in the evening. Will Garnier Fructis Stemoxydin can be used as solvent as well? any suggestions

independently of DHT it is true that androgenic alopecia has an inflammation component to it as well as stress (cortisol).

Inflammation is the end product of tissue injury mediated by DHT and is not an independent phenomenon. It is linked to DHT, as I outlined in the video. Extreme stress can cause hair loss in the form of telogen effluvium, but there is no good evidence that mild stress is important in androgenic alopecia.

Hey Kevin, good video yet again. Quick question though I’m a P.E teacher and I’m outside a lot of time recently I’ve been noticing my hair becoming thinner even though I’m on 5ar inhibitors. I had a bit of crown thinning but I was able to reverse that with pharmaceuticals. Other than that it was just trouble with my hairline, no diffuse thinning. Appreciate that and let us know how the hair transplant goes!

from my personal experience, inflammation can cause many autoimmune diseases such as - Hashimoto which can cause hair Thinning/loss, leaky gut which means very bad nutrient absorption, Eczema, and many many more diseases. personly I will never downplay the nasty effect of autoimmune diseases which are typically caused by inflammation, so having a "Normal" immune system should be one of the priorities besides suppressing DHT and using minoxidil.

Thanks, glad I helped save your hair! Regarding Rob, his theories aren't new. There are ads for devices to massage the scalp and improve blood flow that date back 100 years. In fact tight head bands from hats were a leading theory of hair loss for years, until it was realized that DHT and its effects on the hair cycle are the real culprit and this has nothing to do with blood flow to the scalp. In fact the head in general (because it contains the most important organ, the brain) and the scalp in particular are very vascular, so there's no reason that blood flow should enter into the equation. When faced with these objections, the scalp massage proponents shift the goal posts, and talk about scalp tension, or skull expansion, or any excuse to keep their pet theory alive. Anyway, over the last 100 years of hair loss research, there is more and more evidence for the interaction of genetic predeterminism and DHT as the root cause of androgenic alopecia, making the blood flow theory not only wrong, but unnecessary to explain hair loss.

Hey Kevin, I have been on oral dutasteride for 7 months now and I have been shedding like crazy. Also my hairline has receded almost an inch at this point. I got all my blood work drawn and everything looked good. My DHT level was at 6 ng/dl which is very low. I guess I am curious if it is working not because of the shedding but because my hairline has receded so much. Do you think my hairline will grow back or maybe I should consider going back to fin after giving it a couple more months? Any insight would be great! FYI I have been on minoxidil 5% for years at this point.

Stop setting timers for yourself. Don’t assume within two months it’ll get better. It’s a very long process. Also, if your hairline has aggressively receded like that, shouldn’t you assume it’s a shedding phase??

I'm a super rare bird who has spent the last two years listening to lectures and reading journals. I'm a male with FFA which is a woman's autoimmune disease. I joined two blogs and the biggest contributor of info was asked by a few women if I'm past or experiencing menopause. 🗣️Dude I have a important query to ask... You mentioned in one of your video that you used to consume oral minoxidil for 6 month then you quit it.. But you did not mention what exact dose did you use to take ?? One more question - in that transition period from oral minoxidil to topical minoxidil did you face extra shedding??

I was using doses as high as 40 mg if I recall, very stupid in hindsight. Truth be told any benefit I thought I had was likely just placebo, didn't notice any shedding from it.

Yes, you'll still see filler material in the solvent from the inactive ingredients but finasteride dissolves in alcohol so the active ingredient should mix.

Kevin, I've been on finasteride for 8 months and then switched over to dutasteride the last 6 months and my hair loss (diffuse) still progressed especially in the hairline (more specifically the middle of my hairline, sides untouched). Do I perhaps not have androgenic alopecia but another form of hair loss like scarring alopecia?

How can testosterone levels not be correlated with androgenic alopecia when the highest T levels in the normal range are up to 5-6 times more than the lowest, and up to 10% of T converts to DHT? Also, a common side effect of TRT is hair loss...

Started finasteride in 2021 and switched to dutasteride a year ago and yes, am still losing hair and yes, I have photos lol. I’d like to think it’s much slower than if I hadn’t ever started the therapy, but it’s present nonetheless. Also have been on oral minoxidil for a year, before which I was on topical for 9 months.

I'd consider 2.5 mg of dut, it will suppress about 30 percent more scalp dht than 0.5 mg. It is the strongest thing out there that is still safe. I talk about it in my perfect dose of dutasteride video.

Amazing job! Could you make a video about hair transplant on black men. I have noticed the result are amazing on them because (I beleive) 1: the shape of their hair allow to cover more ground per hair 2: the shape of their hair doesn’t make it as difficult to create a natural angle relative to the skull

Maybe, I think you're right though that the results are typically pretty good. I think the reason why is because most of them have darker hair and that blends in well with darker skin so you don't see the scalp as easily. It makes giving the appearance of density much easier.

Hey again Kevin! I've heard you talking about how 0.5mg dutasteride inhibits only 51% of scalp DHT and 2.5mg of duta inhibits about 79%. There is one study that shows that but is it actually good to use a single study and believe it and give advice according to it's results? I've seen countless studies about other things showing that thing A does thing B, but when you actually do a meta-analysis of quality studies, it turns out that it's not the case. Also that study says that finasteride inhibits only 40% scalp DHT but there are studies such as "The effects of finasteride on scalp skin and serum androgen levels in men with androgenetic alopecia" that say that fina inhibits scalp DHT much more than than. I'm not saying fina or duta inhibits more than in the study that you frequently quote, i just want to know what's the truth. Also Derek from MPMD said (he said it, i take everything with a grain of salt) that he took duta 0.5mg, did a scalp biopsy and found that his DHT was not detectable let alone reduced only by about 50%.

Hey Kevin, thx for the helpful content. I’ve been prescribed finasteride by a dermatologist but have not taken it yet. Recent blood test revealed that I have slightly elevated estradiol and progesterone, testosterone was normal. Now I’m worried that the chances of me getting sides is quite high. I’m otherwise healthy and by no means overweight. My doctor said “take the pill and let’s see what happens” although she literally had to google finasteride lol. What is your opinion?

Well I would probably agree with her about taking the pill and see what happens, though it's not too reassuring that she had to google finasteride! The thing is, blood tests haven't been shown to predict sides, unless you have markedly abnormal tests. It sounds like your tests were borderline normal/abnormal. It would be a shame to lose your hair just because of the remote possibility of sides.

Only possible explanation I can think of is because of the increase from testosterone that finasteride causes, although the fact that it is happening locally on the scalp seems kinda weird. I would speak to a dermatologist, it could also be a reaction to whatever shampoo you're using.

Cuz it’s not a lot it’s like 2-3 but they move around a little I was also using a bar of soap on my face cuz I ran out of actually face wash and I started breaking out cuz of that aswell

hey kevin, quick question. Sometimes people get side effects on fin, however, with continuous use of the drug, the side effects "dissappear". Do they really go away? or is it just that people adjust to their new normal? And if the negative side effects truly go away with time, why doesn't the positive effects (hair growth) go away as well? what's the mechanism of action there?

Research shows the side effects usually do go away although maybe it is possible that for some the side effects are so minor that they quickly learn to adjust to them and forget about them. It's an interesting question but for sure the side effects do go away for most people with continued as confirmed by research and always go away when stopped.

I have retrograde alopecia and i am 20 years old, from the last 6 months I have been on 1 mg Fin and 1 month ago I added Dut 0.5 mg twice weekly alongside 1 mg Fin and Min and thankfully I had no sides. Still the areas where hair have been miniaturized haven't thicken up yet so I was wondering to switch to Dut 0.5 mg daily as I don't want to go bald. So what do you recommend is suitable for me, either stay on the same regimen of 1 mg Fin 5 times a week with 0.5 mg Dut twice weekly or entirely switch to 0.5 mg Dut daily. My hairloss was started around a year ago and I got into treatments really quick, My hairline is really good but i have hairloss from the sides of my head. Any suggestion would be highly appreciated as i am going really depressed with my situation.

Give it another 6 months. The first year of treatment is not a good time to assess the efficacy of any hair loss stack.

What do you think of the study that stress also increases male hairloss? There is a study that stress increases histamine production in the body which can add to hairloss and that taking an anti-histamine daily can help prevent hairloss

Sounds like BS. Extreme traumatic triggering events that cause extreme stress can cause temporary hair loss but the idea that daily stressors we all face are making our hair loss worse is an unsubstantiated claim.

I dont think fin will be enough for all men. The 10year bernstein medical study showed that 10% of men dont maintain their hair. The other all maintained or imporved. I once missunderstood the results since they said 50% didnt improve, but this means 50% maintained/ worsended in the other figures they showed that only about 10% worsended.. This mininterpretation caused me to believe that i didnt respond to fin and was why i quitted fin.

I don't think that there is any treatment of any disease that works for everyone. Maintaining results long-term in 90% of men though is pretty damn good. There were similar results in the 5 year Korean study too. So as drugs go, finasteride is very effective.

Isnt finesteride with combo of minoxidil superior? I think of starting minoxidil. Just begin with finesteride. But im afraid to start minoxidil if finesteride was enough for me. Then in stuck with minoxidil. Did you first start with finesteride only? Studies shows that a combo is superior tho.

I'm using minox in combination with diclofenac (strong anti inflammatory) and it seems to work very good. It's hard to tell wich one does the heavy lift but in my view diclo has a significant impact. Maintaining on it at the moment.

You can easily get self myo-facial release using a foam roller or even just a tennis ball so in my opinion it isn't worth it unless you have a really difficult to pinpoint trigger point causing tightness. If a foam roller isn't cutting it try a PVC pipe, that will leave bruises.

Wait some more time or switch to dutasteride? My (diffuse) hairloss is quiet agressive at 22 years old. (I also have been using minox for more than a year with no success, very few new hairs but essentially didn‘t stop the progression)

I'd say switch to dutasteride but 9 months is a bit too soon. Give it at least a year and if things haven't stabilized then dutasteride is the next logical choice. If necessary you can titrate it all the way up to 2.5 MGs but start with 0.5
