From our 2022 NYGH EM Update conference, infectious diseases specialist Dr. Isaac Bogoch discusses non-COVID infectious diseases in the ED. He reviews which antibiotics we should be giving first in patients we suspect of having bloodstream infections, and whether we can stop antibiotics early in patients with pneumonia who are improving clinically. He reminds us that when we are thinking about our antibiotic prescribing in the ED, we should know the bugs in our area and think about disposition, drug and duration (shorter is better), and that oral antibiotics are just as good as IV options. Dr. Bogoch highlights effective, often under-utilized antibiotics fosfomycin and cefadroxil and stresses the profound human and system impacts of antimicrobial resistance and the role we all play as antimicrobial stewards.
