From our 2022 NYGH EM Update conference, Dr. Justin Morgenstern gives us his evidence-based medicine bottom line for 18 recent ED-related papers. In this talk he reviews literature surrounding cardiac arrest care including the routine use of calcium, temperature management and placement of defibrillator pads. He discusses that ketamine may be faster, but not safer when used in agitated patients, and that using the YEARS and age adjusted D-dimer can reduce CT use and improve the miss rate for pulmonary embolisms. When it comes to intubating in the ED Dr. Morgenstern reviews the BOUGIE trial and concludes that we should strive for excellence with both bougie and stylets to improve first pass success, while the ED-AWARENESS trial reminds us to order sedation for our patients who have received paralytics. Dr. Morgenstern also provides EBM answers to questions like: Does contrast hurt kidneys? Is asymptomatic hypertension in the ED an emergency? Should we be using antibiotics (delayed or otherwise) in uncomplicated diverticulitis and viral infections, or TXA in nosebleeds?
