Pembuluh darah yang mengalami kerusakan akan mengalami proses blood clotting dengan mekanisme: 1. Vasospasm/Vasoconstriction (vasodilatasi terjadi bila non-injury cell) Otot polos pada pembuluh darah akan terkonstriksi untuk mengurangi aliran darah. Signalling vasokinstriksi dilakukan oleh : - Cells around area sends signalls - Nerve reflex - Myogenic Spasm 2. Platelet Plug Formation (platetet membentuk temporary plug untuk menghentikan pendarahan). Dilakukan dengan 4 cara : - Plug Adhession Von Williebrand Factors mengarahkan platelet/trombosit untuk menempel pada sel yang mengalami luka, platelet akan menempel pada daerah luka tersebut - Platelet Release Reaction Platelet mengeluarkan adenosin bifosfat (untuk menarik platelet lain) & serotonin protrombin A2 (meningkatkan Vasospasm & menarik platelet lain) - Agregasi Platelet Pengumpulan platelet pada area luka sehingga membentuk platelet plug formation - Plug Formation Penutupan daerah luka oleh platelet sehingga menghentikan pendarahan 3. Coagulation, berfungsi untuk memperkuat plug yang dibentuk platelet - Intrinsic & Extrensic Pathway (clotting factors) Protrombinase (aktivasi protrombin) yang berperan sebagai ‘lem’ untuk memperkuat plug - Protrombinase dibantu dengan Ca2+ mengkatalasis : Protrombin (inaktif) -> Trombin (aktif) - Trombin dan Ca2+ menjadi enzim dalam perubahan : Fibrinogen (soluble) -> Fibrin (insoluble) Benang-benang fibrin akan menempel di seluruh yang kemudian menutup luka tersebut. Platelet mengeluarkan adenosin bifosfat untuk menarik platelet lain ke daerah sel yang terluka serta mengeluarkan serotonin protrombin A2 untuk meningkatkan vasospasm dan agregrasi platelet. Agregasi platelet. Terjadi saat platelet dalam jumlah yang banyak berkumpul di daerah sel yang luka dan membentuk platelet plug formation. Plug formation. Daerah luka akan tertutupi oleh platelet untuk menghentikan pendarahan. Platelet plug formation akan menyebabkan aliran darah yang keluar menjadi semakin melambat. Namun penghentian pendarahan ini hanya bersifat sementara, sehingga perlu adanya mekanisme lanjutan. • Coagulation Pada proses ini dibutuhkan clotting factors, protrombin, dan fibrinogen yang terdapat di dalam darah. Koagulasi berfungsi untuk memberkuat platelet plug formation. Terdapat 3 tahapan mekanisme koagulasi yaitu: Intrinsic dan extrinsic pathway, dimana jalur keduanya menghasilkan protombinase berfungsi untuk mengaktivasi protrombin. Protrombin berperan untuk memperkuat platelet plug seperti sebuah “lem”. Protombinase dibantuk oleh Ca2+ yang berfungsi sebagai katalis yang akan merubah protrombin menjadi trombin. Trombin dengan Ca2+ akan mengaktifkan enzim untuk merubah fibrinogen menjadi fibrin. Benang-benang fibrin akan menempel di seluruh luka dan akhirnya menutup luka tersebut dengan stabil. Setelah proses penutupan luka selesai, pembuluh darah tidak akan terjadi vasokonstriksi lagi dan aliran darah kembali normal. Thank you for the explanation
