I legit had a friend who loved puke and thinks characters puking themselves while wearing a mask is endearing. I once did an art trade with her and she wanted a character wearing a gas mask and puke overflowing. Did I draw it? Yes, yes I did. Weird kink aside, her art skills were KICKASS, and she even illustrated for a popular video game.

Zach and Michael actually talk about it in the DVD commentary, how they got their start on newgrounds where the trend was being edgy and over the top, so they wanted to give the characters in smiling friends softer moments like that. They also pointed out how Pim's recurring fantasies of just wanting a nice family to take care of is meant to be a sort of cute juxtaposition to the chaotic shit that happens in the show.
