Today's session is based on the 2020A13 Question, "Describe the time course between an intravenous injection of a general anaesthetic agent to loss of consciousness. Explain the delay using pharmacokinetic principles."
The time course involves:
1. Factors that influence Plasma Concentration (Pharmacokinetics)
2. Factors that influence transport to the Biophase (this explains the delay or hysteresis)
3. The Concentration-Effect Relationship (Pharmacodynamics)
Factors that influence transport to the Biophase:
1. Cardiac Output / Cerebral Blood Flow
2. ke0
Factors that influence ke0 are factors that affect the "diffusible fraction": MW, solubility, ionisation, protein binding. ke0 is however independent of the concentration gradient.
The most challenging concept is to understand the influence of Cardiac Output on:
1. Onset
2. Peak Effect / Peak Ce(t) / Emax
3. Time to Peak Effect
through its effects on Cp(t) via distribution and the delivery of drug to the biophase.
