Righto! I was doing mcgill at the advice of my physical therapist who has been very helpful in the past. No results. I started your two exercise for lower back, and recovery was almost immediate. Off to Amazon to buy your book!

I never meant to say the exercises don't work, it's just that performed alone (as they are often prescribed by many Doctors/physios, NOT necessarily by Dr McGill) they are often not sufficient to fix most cases of back pain. I didn't mean this as a slight to Dr McGill (who I respect). Sorry to not be clearer in the video.

Car seats can be a huge issue for me, a lot of brands are removing lumbar adjustment in their base and mid level models, i don't like Ford (poor after sales) but their seats are great (for me), Hyundai are not too bad and Toyota is St-t, terrible, my back aches after 1 hour driving, yes tried back cushions, after market lumbar adjustment seems very expensive, i know get another car but $$$ is a issue. Thanks for your tips etc.

Same here Steve. I managed to get hold of a higher spec Kia Sorento ( 2nd hand btw ) with electrical adjustment of seats. I can now take on longer drives again with very little issue, I do still require stops for stretching after around 3 hours but usually need a toilet break by then anyway - old man prostate 😅

Car seats are a nightmare!!! I had a Toyota Sienna van for 15 years and I absolutely hated driving it every single time I got in I was raising a young family and could not afford to change my vehicle. I got a VW Westfalia when my kids grew up and the seat is a slight improvement. But then my son bought a PT Cruiser and a good deal, and the seat is absolute heaven I take his Cruiser out to boot around town for groceries and stuff and leave my van at home. I have even decided to buy it off of him simply because of the seat because he bought a truck

Will, I'm 79 and have osteoarthritis in my spine and torn ACL and MCL in my right knee. I have been doing exercises for many years to manage these problems. I am happy to say that thanks to the exercises, I am not significantly handicapped and rarely need to take any medicine for these conditions. However, since finding your channel I have modified several exercises and replaced a few in accordance with your advice. This has clearly led to improvements. Thank you. About this particular video, I have one question. Due to both the torn ligaments and OA in my knees, I can't do side planks. Is there another way to strengthen the same muscles?

Thank you for your very informative video. My husband (age 69) had really bad lower back sciatica for a while. Initially his doctor thought it was a herniated disc. A MRI ruled it out and he was told he only had a small amount of arthritis. He went to a physio who determined the sciatica was caused by extremely tight lower back muscles as a result of tight hip flexors (he walked like you demonstrated, I recognized it immediately). He started doing side to side knee bends and glutes bridges. These exercises helped stretch his hip flexors and strengthen his glutes and regular use has made a huge difference.

I had lower back pain for years until I found my current physiotherapist who found i had very tight quads and hip flexors and weak glutes which along with hyperlordosis have caused me years of pain. she gave me stretches to do which fit in with your recommendations.

Fantastic info. The brain link with potential risk is fascinating, and ties in with so many issues that people have. You're the first I've heard bring this into pain management (other than amputees and mirror box). I'm 51 and suffering from what I believe is an SI issue, but also lower back pain - but I am highly flexible - so that has always confused me... I do lift and stretch daily, so the tight muscle groups are my next focus.

Oh my goodness……when you mentioned over sensitised nervous system my heart skipped a beat. I have never heard anyone mention this before and this is exactly what l have. Just wondering if there is any chance you could please put up some stretches or exercises to work on this issue? l know you mentioned one move but I’m hoping there are more. Excellent explanation, Dr. Harlow. I've seen the McGill 3 in so many videos recommended for back pain, but my pain was so severe that those exercises were out of the question. Now that my core is stronger, the backbpain still persisted until i started doing glute exercises. Thank you.

Your head leaning forward with poor posture can result in over curvature of the neck, as well as the mid and lower back. This was a problem that I had and it took weeks of strengthening exercises to correct the problem and relieve back pain.

@@bminney The 3 key movements for the neck and upper back posture that helped me are: chin tucks, the band exercise that Will showed where you keep your arms outstretched and squeeze your shoulder blades, and a movement lying face down, doing the same arms stretched out, thumbs up squeeze with 2 lb weights. I also do the classic lower back and core work.

Spot on! I had a pretty severe anterior pelvic tilt. So bad, I almost felt like I was going to fall back while walking up the stairs. I have to stretch my hip flexors and quads daily, or I'll be in a world of hurt. Also had very weak glutes, and had to work on that.

Thank you. Agree totally. Even being charitable to McGill, his approach denies the existence of non-specific drivers. I've worked for years in pain management and to deny biopsychosocial factors in 2024 looks like dogmatic adherence to a particular model. PS Just seen KINDLE version of book! PRE-ORDERED. 👍👍

I have your book (thank you), I watch your videos (thank you again) and now I’ll go to the link you’ve created to see if it’s a routine I can do on a daily basis. You’ve motivated me to try new and different things to slow down the degradation circling a 68 year old body. I find standing on one leg for a minute while my electric toothbrush does its thing is a fun balance challenge, though the risk of falling with a sharp object in my mouth may not be conducive to seeing 69. Maybe I’ll just stick to your videos. Keep up the great work. It’s appreciated.

Will, thank you for your posts and your book - I find both very helpful. I do struggle with lower back pain. I am 65, I rock climb 3-4 days a week, I train regularly so am pretty fit, but when I go for an easy walk with my wife on a level concrete path for an hour or two I get pretty continuous lower back pain that lasts for weeks. I think I may be struggling with anterior pelvic tilt - my butt sticks out. You mentioned stretching your glutes in this video provided you relief. I didn't see a description of this stretch in the video - did I miss it? I always warm up for a half an hour and then stretch before climbing, I focus on loosening up my neck, shoulders, waist, hips and hamstrings. My hamstrings are quite tight, I have pretty good range of motion in my hips. I wonder if tight glutes could be the cause of my lower back pain? Interestingly, if I am hiking up a mountain I don't get the same pain, the pain seems to come from relatively should walks on level, paved paths. Thank you for all your content! It took me awhile to develop an awareness of the angle of my pelvis, I do exactly what you suggest - when I am walking I focus on tucking my pelvis to reduce that angle. It feels better when I am consciously doing it, however I don’t get any long term relief. But it’s a great suggestion and I am focusing on trying to tuck my pelvis to a neutral position. Thanks for taking the time to respond and sharing your experience!

I'm not part of the target group but I came across your video because, while I do have back pain, it's mostly if not all due to my si joint dysfunction. Not the joint but the ligaments, particularly my long dorsal sacroiliac ligament. It affects the lat. The lat stabilizes the pelvis. If you're pelvis is unstable due to injury like me, it can affect your mid and upper back! And your core runs from your head to your pelvis. Core stability is spinal stability from head to pelvis. If there are any instability in that chain, you can have pain anywhere along that track and obviously it could give you pain in your lower extremities. Core stability is not just in your abs or your ribcage. It's bigger than that. And in my case, SI ligaments are causing me pain seemingly all over. What I've learned on my long journey to figuring out my pain is find the source of sources of the tissue causing pain. For me, I found the particular ligaments. And the pain is worse because of my postural misalignment and muscularskeletal imbalances. To get out of pain, you need to focus on breath including diaphragmatic breathing, get stacked--all three diaphragms in alignment--engage your side abs, address your imbalances and, most importantly, rebuild your CNS. A Strength and conditional coach who I work with that works at the pro level with sports teams/leagues and athletes told me the secret is CNS. None of the other tools will help if you dont rebuild your CNS and get your brain to control your body, not the other way around.

Yeah, you could've been describing me. I am a super-flexi person who herniated my L5 raking leaves last Fall. Saw a PT who had me doing bird dogs, side planks and glute bridges, etc. Like a lot of your viewers, I did my exercises religiously. I was moving & feeling fantastic! Then, our annual YMCA shutdown happened and I hurt myself working out outside on concrete. Super frustrating. Am back to square one. How did you help the gymnast?

Such good sense, thank you Will. What to do when it seems chronic back pain straddles various categories like injury, hyper sensitivity and SI joint instability. The smallest wrong move and the viscous cycle starts all over again. I’m 72 and in the gym with a conscientious personal trainer X 3 weekly

these videos are excellent. from what i have seen of you and the advice you are giving; i haven't done the core exerises, i have just used ibrophen vibrator machine. the person who offered core exercises is very good not the one you mention. i lie on accuppressure mat and put two pillows under my back at night. i amnow going to see if i can do a walk on the hills with my stick i have for a couple of weeks

I often have quite bad pain to the left of my lumbar spine. I assume it is the hip joints (no idea how they are called) back there. Dead hanging from a bar helps a bit. But what really makes it go away completely for 3-5 days is deadlifting heavy weights. But I am sometimes a bit afraid that I might make it worse in the long run with that. But it really really helps in a big way. I do have tight hip flexors but stretching them makes it worse. They are strong though.

I discovered the McGill Big 3 after about 4 years of lower back pain. They fixed me in about a week. I include them in my regular exercise routine now and have never had a recurrence of back pain. Stuart McGill has both a lot of clinical experience and has trained a large number of competitive athletes, getting them from back injuries to world class level. He is well aware that these 3 exercises are not the only way forward. As a side note, your demonstration of the curl up is incorrect. You've turned it almost into a sit up, which McGill specifically argues against. Just my two cents.

I'm 40 ( just turned ) I been doing these and every other stretch you mentioned but for three years now I can't seem to heal this bulging/herniated disc . I'm so fed up but I won't give up.. something has to give..I'm a very fit person who wants to get back to competing in my sport so I'm very disciplined with PT and stretching and strength the muscles but nothing. 😡

Great video but one question comes to mind. How on earth do i work out which reason is mine for my back pain? I was given planks for my abs, lock clams and bridges for my glutes. I find that eveyone tells me my glutes are weak but trying to strengthen them gives me pain in the glutes which just adds to my back pain. So i have no idea what to do or who to believe 😢😢

I agree that the McGill big 3 on their own are not the only fix for back pain. Back pain is different for every individual, as is the “cure.” In many cases one can only find good ways to manage their pain, depending on the root cause. In defense of Dr. McGill, he does outline this in his book “the Back Mechanic “ which needs to be read in its entirety to be of any help. He also discusses movement habits which can play a huge role in managing back pain. It’s a great book and it’s a pity that some PT’s just extract the big three if they are not addressing overall spine health and habits. With all due respect, I’m not saying that you are one of them. Just please perhaps mention that it’s the isolation of just using the big three that can be the issue, some may have interpreted that to mean McGill’s methods are wrong, when in fact they are very helpful.

I was doing this very bird dog exercise and wondering why I was getting pain... The only other thing I wished they did is have links to the other videos. I am sure Will likely has a tight glutes video, tight quads video, tight hamstrings video etc... I will look them up.

100% agreed. Most of back exercises given here by doctors specifically same stupid exercises for last 30 years is such a waste of time. If you have backpain than think outside the box to resolve them rather than doctors given stupid exercises which doesn't do anything.....

The biggest drawback of McGill system is lack of flexibility! Core stability is very important! But so is flexibility ! I’ve also found out that flexion is not your enemy what you mentioned is true that the brain can process it as a pain trigger for me, sitting on a floor with my legs spread and flexing forward, naturally, has been therapeutic for my lower spine sensitivity, thanks.

Yes your brain can sometimes be your worst enemy just by overprotecting you. I even think that problems with discs, pain from real structural damage can last up to 2-3 months at the most. And rest of the chronic pain comes from brain. Buz this is kind of hard to get over it. Same as with anxiety. You have to slowly expose yourself to "potential threaths" and by time you can desensitize your nerves and brain

Hi. I am 57 & having renal transplant some 24 years ago.currently using Lifetime immuno suppressive drugs including light steroids Currently living a home life after 25 years of banking job I am having muscle weakness issue, specially whole back & legs,.Doing some back exercise alao but not getting rid of it.can you guide me & send me some tips & whole body exercise.Is it better to start some weight bearing workouts also?

the general definition of core as a location is fallacious since it is all about adaptation to gravity...core is not a location, it is a response to gravity usually embodied in balance.

Does your book suggest routines for getting started (again). I'm 72 with sarcopenia and although I do a fair amount of moving around I don't do any regular disciplined exercise.

Same! I was doing all the things and was feeling great. Then, my YMCA annual shut down occurred and I re-injured myself. Super frustrating. Am back to square one.😢

I have very tight and painful hamstrings in one leg and the calf muscle which is also making my foot tingle, I first thought my back, but absolutely no pain there, so unsure if I've upset something in the nervous system, which is quite possible as these muscles in this leg haven't really engaged in years, it wears off when doing something, and when I use the massage gun?

The "big 3" exercises aren't even sufficient for stability. The trunk muscles in these exercises are working against only partial body weight. If one's task involves, say, lifting 80# from the floor, one's back strength will be insufficient with only the "big 3" for training. Rather, one should train to do more than what the task will require. One should practice the task and more with good form, so that the strength and good form will be there in actual performance of the task. Don't go out on the field and expect to be able to do what you aren't doing in training.

There is a related issue. Some fitness influencers, even ones who are very respected, say things like, you don't need explicit back training or grip training for the deadlift -- you can just progress the deadlift and the back and grip strength will come along for free. Or, they say, just do suitcase carries, which is an example of expecting something that requires less strength, e.g. a carry with a dumbbell weighing x#, to stand in for deadlifting 4 times x#. IMO what is actually happening is that these influencers are thinking of young folks and / or men, who gain muscle easily, and for whom progressing the actual task might work, even if it's always borderline unsafe. For older folks, especially those dealing with sarcopenia, that "getting it for free" isn't safe and likely won't work. We do need to work explicitly on the supporting muscles, like the back.

We need more educated persons to analyse the systems ..and many of the fisio therapists will not do that.
