My heart goes out to these patients. I wish there was somewhere we could help with anesthetic payments. I barely can make ends meet otherwise I would love to send money to help. Thank you doctor for all that you do to help these patients.

I have watched so many other medical proceedures but i have never felt so upset for the patient as I had while watching this proceedure. I hope the patient was helped after sitting through this clean up.

Was the patient awake for this ? It looked very painful and can’t imagine be awake for this. The ear was a mess and looked infected on top of it. I sure would like to see after video of what the ear would like like when it’s healed. OMG you truly are an amazing ENT. Thanks to another amazing video.

For those who don't know what a cholesteatoma is; here is some info for you: This is due mainly because of a retracted eardrum from repeated middle ear infections and/or swimmers ear. The eardrum becomes retracted there for creating a small pouch like home for dead skin to build up. A cholesteatoma is nothing but dead skin buildup non cancerous cyst and if it's not removed it can damage the three tiny bones in the middle ear you will also lose hearing and infection will Persist if not taken care of you will drain continuous smelly fluid and surgery is the only option to get rid of the cholesteatoma. Cholesteatoma got its name from (cholest) from cholesterol since a cholesteatoma resembles chunks of fatty tissue. Cholesteatoma a can grow at the bottom of the eardrum this is called safe ear since it's no near the delicate bones. Or a person could develop n attic cholesteatoma which is known as an unsafe ear this is because it's growing near the bones and facial nerve and will destruct anything in its path. Some people will require a mastoidectomy where the doctor will drill and clean out any debris in the tiny air holes within the skull. And some people could require reconstruction of the eardrum and or the tiny bones within the middle ear. In rare cases, long existing cases could cause meningitis of the brain if cholesteatoma continues to grow. It is rare but also possible for cholesteatoma to grow near or into the brain. That's why it is so important to act fast and don't delay cholesteatoma once diagnosed, you won't die overnight nor in a few months but depending on how far cholesteatoma has grown or spread will determine how soon you will need an operation. Dr. Zhao has said that a lot of his patients can't afford the anesthesia, so they will have as much as they can tolerate removed. If there is anything left behind there is a reason unless it's an infection because the removal of wax is to get rid of the surplus. Think of your ear as a self-cleaning oven, you only step in when there is a problem; like you've never cleaned it and there is a ton of build-up.

Thanks - is this horrible condition very prevalent in China for some reason? Because this guy's videos seem to show nothing but cholesteatoma cases, yet a British audiologist who posts similar ear treatment videos is mostly clearing wax from healthy ears and it is very rare to see the terrible cases in the UK that we see in China.

@@jacquelyngorman1626 Not always. Anaesthetic is very expensive in China and often the patient can’t afford it, many times the patient is completely awake and feeling every bit of the procedure. I’m sure this person hasn’t had a general anaesthetic, normally under a ga the patient has a green covering over themselves with only the area to be operated on uncovered as shown in the intro do Dr Z’s videos.

This was the grossest of all of the ear wax removals I have seen. Not that I am going to complain because it is totally watchable but still was a little bit on the gag side. Love this channel though! couldn't live without it!!

The video is cholesteatoma which requires annual check up on life time basis as it is skin disease to the ear being unable to remove on its own.

Clean the debris first, then wait the inflame canal to calm down and the last after the canal at least 'healthy', they can cut down the polyp.

He mentioned in the video that normally they would surgically remove the polyp first, but doing so would require anesthesia and a hospital stay which would take a lot of time. The patient was also 90 years old, so the family was hesitant to put them under anesthesia. Apparently, this patient came to him for a cleaning before and seemed to really trust him, so he agreed to try to remove as much as the patient was comfortable with.

I would speculate that the polyp contributed to this ear not being able to evacuate the wax and keratin plugs. Despite the title containing the word cholesteatoma, this is a keratosis obterans case as there is no evidence of breach of the meatus or tympanum from the middle ear and no signs of tympanic retraction.
