Never ceases to amaze me how in the cholesteamia contain such a great amount of debris in the ear that's u have to wonder how an ear can get the enormous amount of and how actually damaging they can be ,I feel it's a great learning tool it is for us to discover the human body and how the problems arise and the bodies reaction to clear them .we truly have an ability to eradicate different issues that can cause so much suffering even felt by the elderly.the asian philosophy of dealing when it arises to minimise trauma for these patients as quick an less invasive rather than the consultant in these countries china,Japanese, India,to list just a few. These wonderful doctors have so much respect for their patients young and old is something the drs in the west could learn a lot from xx

TRANSLATION: This patient is presented with a cholesteatoma recurrence after a lapse of 2 or 3 years as the EAC no longer has its natural expulsion properties. Most cases require annual debridement with an exceptional case that was needed after 6 months. The patient’s EAC has been circumferentially extended by the disease, enabling the usage of the facial nerve hook, normally used in middle ear surgeries. Too narrow a canal that cannot even permit an otoendoscopic examination will need widening through surgery. There is perforation on the pars flaccida and some calcification of subepithelium on the pars tensa. Low pressure from suctioning around a perforated tympanic membrane will be more intense, causing short term disequilibrium in the inner ear, usually lasting just a few minutes. The ossicles are located posteriorly from the red spots. As the cholesteatoma is quite advanced, it has eroded down to a skin flap of a divide from the surrounding bone structures. A CT scan will be done to determine if the disease has breached through into the middle ear.

Considering how bad the ear initially looked, I am surprised at how much damage there was behind the cholesteatoma. I expected much more damage to the ear canal, but Dr. Zhao was able to remove a lot of the infected tissue. You could still make out where a portion of the eardrum was located, as well as the outline of the hammer bone. Good work Dr. Zhao.

It starts off white and the longer it sits in your ear it darkens in color most of the time when it’s white that means it’s fresher the the surrounding yellow skin eventually it turns black and hardened

Hi Ellen. A translator explained that there is some reason that he is unable to show the contents. For some reason his videos are censored and he's only able to show so much. It's of my understanding that if he was able to lay it out and show it to us he would. That's why you see him leave it hanging by the ear for a while so that we can at least get a bit of an idea of what it looks like. If this sounds rather confusing it's because I was a little bit confused by the explanation. LOL

It doesn't matter how many of these I see I just cannot get my head around how aggressively these grow and the amount of contents that is in the ear. I would think it would burst and literally be hanging out of the person's ear. God forbid. I've stopped using Q-tips or putting anything in my ears since watching these videos but I remember using a Q-tip putting it in too far and just touching my eardrum and the pain was excruciating. I just can't imagine what this must feel like. I'd give anything to be able to have a Q&A with the doctor and ask him some questions.

Hi Dixie Rae! How are u hon? I was thinking the same thing as I was watching this video...it just looks so aggressive & overpowering. I feel bad for anyone who has this condition. I'm sure the pain is horrible. Dr Zhao is amazing for being able to clean this up for his patients. And he has said b4 people come to him when they cant take the pain from other ENTs...so he must have a light touch. I hope all is well w/u my friend. Always good to see u here😊💙

I couldn't imagine that happening in my ear I've had so many ear infections growing up as I seem to remember last time I had a major ear infection, I was 20 years old, it was during the Christmas holidays and we were at a party. But then the pain started getting really bad even as intoxicated as I was I remember the pain sobering me right up. I went to the outpatients right away crying the whole time, I remember begging them to see me sooner oh, because I had already been waiting for hours and the pain was just getting more and more intense. I just have a lot of issues with me is and I've had 3 surgeries the last one being when I was 20. I have not had many ear issues since but like you said when you use ear buds and they poke your eardrum it really hurts. So I cannot imagine my eardrums looking like there's. I want to thank the doctor for helping all these people , a lot of people do come from far away just to see him , he must be very gentle and very good at his job

@@jennyl6649 I'm sure Jenny that I read or heard that there are people that can afford anaesthesia and what he does is he simply goes as far as the person can stand it. So yes I believe there are people that goes through this procedure without any anaesthesia. I wish I could remember what comment section it was in but this was a long time ago. Now he seems like an extremely kind man and I'm sure that there are times when he makes an exception but I remember reading that and just being shook by the thought of someone going through a procedure like that and even having a partial removal without anything for the pain. This is a totally different situation but I used to work with a woman that was Cantonese and she said that her whole life she went to a dentist without getting any freezing. It was extra and it was so expensive that most people couldn't afford it and the majority of the dentist didn't even have it available in their office. I said how could you stand it and she said anything was better than the tooth pain so they managed. Very scary. Anyway honey yes unfortunately it started raining right at the beginning of September and hasn't stopped since. I woke up yesterday morning and it was frozen outside and I brought out the extra comforter and my little bedside heater, I refuse to start paying the hydro company so soon LOL our summers are short but this one was really short. We went from 90 degree weather down to 65 within a few days. This makes it tough because your body doesn't have a chance to adjust. So yeah that's what's happening. I'm very glad to hear that it's still nice and warm and that brings me to not even remembering where you're from. Surrey girl the memory she's not what she used to be. I have to laugh I've gotten so comfortable in these comments actions I just chat away like I'm on messenger. I'm surprised I don't have trolls Galore at my door. I probably scare them away he he he. What’s crazy is that it’s actually made up entirely of dead skin, not wax. It’s just insane to me that dead skin can become that way and look like that. No matter how many times audiologists explain in their videos how a cholesteatoma is formed, I still just can’t picture how layers of freakin _skin_ turns into THAT. 😳✌️☺️

@@_birdie but yeah that is a lot of skin. But I heard skin goes like a conveyor belt in the ear. It grows and it's supposed to come out but when it doesn't it layers on top of each other so you can have layers and layers of skin on top of each other obviously dead skin. The longer it stays in it becomes like a long ribbons I know that Mr Neil the wax Whisperer talks a lot about dead skin in his videos and it's in English

@@hunbun28 Yea ur right it’s a lot. Not all, when removed, come out in ribbons, (tho these here are very different ribbons than just the plain regular dead skin buildup for sure haha). But I do like those kind of cholesteatomas. They’re like the evil twin of regular dead skin buildup haha. Yet even tho I know how it’s formed, I still am always just shocked when I actually see the massive insane ones. Like, “dead skin created THAT???” Our bodies are weird as hell!. Btw right after I read the words “conveyer belt” I went “she watches Neel too!” Haha. Do u also watch Rhys Barber???

fruit salad in fact anesthesia is only available to people who can afford it. At least in Dr Zhao's practice. That's why he is sometimes unable to go past a certain point of cleaning out the cholesteatoma, because it is too painful for the patient who must go without anesthesia.

Antibiotics wouldn't work here as this is not a bacterial infection. I would best compare it to an epidermoid cyst, where a wall is created from the skin tissue and gets filled in over time. The difference being the most cysts don't completely destroy the surrounding tissues.

Its not wax but a type of tumour. This tumour eats skin and bone and has been known to damage the inner ear and even eats its way through to the brain. The good thing about this type of tumour is that it doesn't spread to lther parts of the body

He goes as far as he can. Most of the time his patients can not pay for general anaesthetic, so this procedure is done in the chair while awake. So he will only go as far as the patient can handle or how far the ear can tolerate

Dr. Zhao does not like to edit his videos, that's why we see the black parts in them ... He announces it in one of his videos..... I don't like it either but I've found when i tap the 10 second tab (if you have it on the video) it's timing is perfect for the next thing happening!! Or i just tap the actual video for fast fwd ... That works well too!!!
