My husband was on a training flight over Balboa park when the accident happened. He said he had been listening to the communications and realized PSA was coming into Lindbergh. He looked up and saw PSA coming down in flames. It felt so close they took evasive action. He keyed the mike and told the controller who told him they had everything under control. He flew back to SEE and canceled the rest of his flights for the day as he was not feeling well after witnessing the crash. A few hours later he got a phone call from the FAA. He always wondered if he was the mystery airplane that PSA thought was the traffic.

My father had a reservation on this flight. He decided at the last minute to drive instead because it was a nice day that day. My stepmom had to tell my mom my father had driven instead because my mom was freaking out about what was she gonna tell us. My dad cheated fate that day. He lived another eight years before dying from brain cancer when I was 18.

@@rc70ys I agree with you 100%. As a business owner, when you do things with many employees at the same time, there is a lot of "kids-Play" in my experience. I think that the video showed this going on in the back, which I could imagine it going on in the cockpit also. What do you think. Sad story for all, but mostly the ones on the ground. I thought I might try to nose down into a street, so I didn't take out so many innocent people and property on the ground, if possible ???

I live 2.5 blocks from the impact site in North Park. There is a curb area that is still messed up. The explosion of the crash seemed to have bounced over the two closest houses and landed on the houses past that. Whenever I hear a low plane over my house I run outside to check on what is going on. Every anniversary I go to the site and say a prayer.

I lived in mission beach at the time, did time everywhere around from OB to PB LJ and so forth and own a triplex in clairemont now but all this time I’ve been doing 17yrs as ground control for delta AA virgin America and Alaska at SAN so I’m very familiar and know the airspace and airport well. It’s a tight spot and airfield.

Thanks for the "re-run" of these season 11 episodes from 2012! In the noughties and 2010s, Mayday was one of the best shows on TV. It was educational and extremely well produced. Some of the younger viewers here seem to believe these are new episodes, which goes to show how well they stand the test of time.

I feel they shouldn't have allowed training flight Cessna into the busy routes. Probably, this is the main reason for collision. Traning flight and passenger jet shouldn't be allowed on the same zone.

In 1984 I flew a Cessna 150 with my instructor from Riverside to Lindberg and it was quite hectic landing in between two big jets, taxi back for take of and taking off avoiding wake turbulance etc.

yep, i was 18, and had arrived after a 3 day drive from detroit. I was so tired i just slept in the car, when I woke up to the radio saying a plane just crashed, I looked out and saw the column of smoke about 5 or 6 miles from where i was! Can't believe it was 46 years ago! yikes!

The Real question is Why did the Tower controller have a Cessna anywhere near the approach path to the runway while a passenger aircraft is landing on that runway. The Cessna should have been given a turn away from the extended runway centreline.

I live about 4 miles from the crash site in North Park. Last year, I was driving around that area, kind of lost, and I came to the exact intersection where the plane crashed. It was September 25, 2023, which was the 45th anniversary to the day. Freaked me out a little because I'd never been there before. Spirits calling me to visit?

I used to live about 1/4 mile west of the PSA jet crash site, just off Dwight street. Sometimes I would walk by the site and there were still broken pieces of concrete missing from the sidewalk and curbs where the jet hit.

The aviation industry and government had only known see and avoid was a failure since at least 1956 with the Grand Canyon collision. See and avoid is only a backup if other collision avoidance fails.

I’ve worked McDonnell Douglas Boeing for a long time. Back in the MDC days, we had the FAA crash transcripts on our mainframe system. It was amazing to read the last words from the pilots on crashes.

⚠️ San Diego has explored relocating its international airport, San Diego International Airport (also known as Lindbergh Field), several times over the years due to concerns about capacity, noise, and expansion limitations. The airport is located near downtown, and its single runway is one of the busiest in the U.S., making expansion difficult. One of the most significant efforts to relocate the airport occurred in the early 2000s. In 2006, a ballot initiative proposed moving the airport to the Miramar Marine Corps Air Station, located north of downtown San Diego. The idea was that Miramar had more space for expansion and could better handle future passenger and cargo growth. When I first started flying I thought It would be best to learn at the biggest airport around (San Diego Lindberg Field) WRONG! Instructor ALWAYS took me 15 miles away to nearest practice area right next to Brown Field. When I soloed I soloed at Brown Field. One day the wind shifted while I was out and the landing changed from the usual 13 to 31. As a new solo I heard what I was accustomed to hear 13. Fortunately he added follow the Beach your number 2 to land. I Said, Beach not in sight am I still 2 for 13? Controller said negative. Runway 31. Pull up and enter left traffic for 31! That's when I saw the Beach landing in the opposite runway I was preparing to land on. So Yes indeed I agree the mix of general aviation with commercial airliners not good. Especially student pilots. ( the 😢runways 13 and 31 no longer in use there and smaller aircraft discouraged with landing fee)

PSA fell over a neighborhood named North Park. My friend was in school not far from where this happened. He told me body parts were everywhere and people found parts months later. It was the worst air disaster San Diego had ever seen then and since.

I worked for Pacific Southwest Airlines’ Frequent Flyer department as the airline’s second tragedy occurred, when an armed, irate passenger caused a crash near San Luis Obispo with the loss of everyone aboard. My supervisor volunteered to be on the team that notified relatives of the people who died, something I always respected her for. PSA was sold to U.S. Air - now itself just a memory - not long afterwards. For my time there, it was a great place to work.

This has always been kind of a strange accident. Everyone involved did something that was slightly wrong. The PSA pilot wasn’t clear enough when he lost visual, the pilot of the Cessna drifted just a little off course, the tower thought the PSA pilot had seen the Cessna and the controller thought the crash avoidance system was faulty and that the pilots still had a visual on the Cessna. Nobody did anything egregious but 4 small mistakes from 4 different people ended up turning into a disaster. I still think the PSA pilots are mostly to blame out of everyone cause they lost sight of the other plane but it’s really sad how much had to line up wrong for this to happen

Wife and I lived appx 1/4 mile where it went in. Wife home and heard collision with 172, looked out window and saw airliner on fire. She got on ground with dog and heard it hit and ensuing explosions then turned on t.v. to get details. When she heard the 172 was from Montgomery field she immediately called me because I had been working on my instrument rating there. I had to call my instructor to make sure he was o.k. (he was) and assured me that we were still scheduled for the next day. I also knew and was friends with the Air Trafic Controller. Bad day all around. Condolences to all that lost people.

@@carriebizzNot that dangerous. The non flying pilot has the responsibility to see and avoid. I thought it was a little odd they made such a big deal out of it. The jet said they had traffic in sight - so I’m sure the Cessna had no clue they were about the get run over. A rear view mirror might have been helpful.

Aeromexico 498 in 1986 in Cerritos, an LA suburb: A very similar crash as it was clipped in the tail section by a small private plane. 64 people on the plane and 15 people on the ground were killed.

I saw it when it was nearly out of sight. No report nor show like this explains what the damage was that caused 182 to crash. The fire is explained by a ruptured fuel tank from the Cessna's position at the collision. But what exactly happened that caused the PSA to crash?

I aint sh*t, but...why is it the trainer didn't realize they were off course?? Wouldn't that be number one...besides fuel? I think it could have been prevented if the smaller plane had been paying attention. Just a question from a person scared witless to fly anyway.... Were the controllers able to tell if a plane goes off course...back then, when this happened?

That tracking was being done and monitored at another location than where the Lindbergh approach controller was. IIRC there was only 17 seconds between when the deviation was detected and the impact -- not enough time to relay the information to the tower and then the aircraft.
It seemed to be many things combined, ATC,inexperienced flyer and a potential distracted jet crew while ATC ignoring warnings was a disaster waiting to happen. I'm surprised it was only once.

YES! At last this is here. I've been asking for this for quite a while now. 34:30-35:40 - I think that was one of the most critical points of the investigation. The eye reference thing was a good idea, but the manufacturers forgot that, as they said at 35:00, "pilots come in all sizes and shapes." Being legally blind in my left eye, I probably would never had had a pilot's license. But if I did, and I flew that 727, I would want to make certain I can see the instruments. So I set the seat with two things in mind. First priority - being able to see and read all the instruments clearly. Second priority - being as comfortable as possible. Would probably sacrifice comfort for being able to see the instruments. So I would ignore the eye reference thing, even if it was stated in the manual that I needed to do that. And if my supervisor got on my case about that, then I would explain why I ignore it. Being able to see the instrument increases being able to ensure the passengers' safety. If the manager doesn't my reasoning, and if the higher-ups don't, then I guess I would have turned in my pilot badge. The eye reference thing might have been a good idea at the time, but I think this investigation proved that it was more or less useless.

I was working building stairs at the Valencia Views subdivision and walked up to the second floor that had no framing yet. It was Monday at 9 am and heard an explosion above my head. Looked up and saw a white cloud of smoke with the PSA coming out of the smoke with it's wing on fire and something falling from it which was the Cessna. I watched the PSA go below the treeline and wanted to cover my eyes. The local radio made a mistake and announced the names of the flight crew right after it happened. One of the guys on the job site heard on the radio his wife's name which was a stewartist on board. We watched him get in his truck and speed away to the site. I think the PSA didn't see them because the Cessna came up from underneath them from the right.

No General Aviation should have been in the TCA zone of a major airport . Prince Philip also wandered into the approach path of a BA aircraft into LHR .

I have to ask why the instructor in the Cessna was not communicating with ATC? Was he not listening to communications? Another is why fly under the hood in airport airspace? Under the hood is not normal procedure for an ILS approach? Practice areas are usually miles away from the airport.

I lived on L st just west of the 805 freeway and about 2 miles south of the accident in north park, I remember it like it was yesterday, was the 1st disaster I have witnessed so far in my life, and of course theres been many since then, 911, oklahoma city bombing, waco to name a few, but this happening so close to my house, well it's hard to describe the feeling, those unsuspecting people on the ground in their houses getting ready to go to work or whatever just never knew what hit them, was such a tragic day, I'll never forget it.....Javi G.

I don't understand why the Cessna didn't get out of the way of this huge commercial airline....Its the equivalent of being in a small car driving on the Interstate in close proximity to an 18 wheeler...I dont rely on the trucker seeing me...I let the truck pass and leave enough distance so I know I'm out of their blindspots and also out of harms way in case of any sudden lane changes or unforseen obstacles...

My high school’s gym (St. Augustine High School) was used as a morgue for the bodies. Some of the students who witnessed the tragic accident said it looked like the plane falling towards the campus

TCAS was not solution at that time. It was confusing. Result, 2002 uberlingen mid air collision where Tu-154 and DHL B-757 collided.

TCAS works just fine when it’s used properly. Trouble in the Uberlinger collision was the Russians didn’t use it properly and obeyed the ATC instead of TCAS.

36:11 in Video Duration... and they were possibly traveling at the same speed; or it could have seemed like that as the bigger plane was declining in altitude so it could have caused the smaller plane to blend in with the stuff underneath it... as it was heading right towards it. It could have been like an allusion as well as the smaller plane was painted white above it along with the wings, that's probably something else to consider in the aviation industry painting planes that wont be disguised with the ground, cities, forests, snow, fog, etc.

Sorry, you have it wrong…. Retired airline captain here …. The cockpit voice recorder had other pilots from other airline in the jumpseat and flight attendants on the flight deck… too many distractions… pilots lost their situational awareness…. We have a sterile cockpit below 10,000’ and airspace changed after that crash. I hate to see inaccurate depictions on this flight. You should do your homework before making a KZitem video

These planes need a Blindspot monitor with with rear cross traffic alert. They also need a pre-collision system for the front
